Impaired catecholamine secretion as a cause of diabetic autonomic neuropathy.

Human and animal studies were performed to investigate the causes of diabetic autonomic neuropathy. Human diabetics, with and without autonomic neuropathy, were measured for plasma catecholamine response to insulin hypoglycemia and for urinary catecholamine excretion. In streptozotocin-diabetic rats, plasma catecholamine response and tissue catecholamine concentrations were measured at various stages of the disease. As the duration of the diabetic state lengthens in rats, there is a time-proportional stepwise decrease in plasma catecholamine response. This is similar to the clinical course obs

The potent pressor effect of phenylpropanolamine in patients with autonomic impairment.

We determined the pressor effect of phenylpropanolamine hydrochloride, a widely used sympathomimetic agent, in 14 patients with autonomic failure and orthostatic hypotension. Blood pressure was monitored every five minutes in the sitting position with an automated device. Oral administration of 25 mg of phenylpropanolamine hydrochloride in nine patients produced significant increases in systolic (32 mm Hg), diastolic (15 mm Hg), and mean (21 mm Hg) blood pressures. The pressor effect appeared within the first 60 minutes and was still present at 105 minutes. There was no change in heart rate. B

Increased respiratory frequency and variability in high risk babies who die of sudden infant death syndrome.

We have tested the hypothesis that autonomic instability, reflected in increased variability of heart rate and respiratory frequency, characterized high risk babies who died of sudden infant death syndrome. Using computer-based methods, we compared the power spectra of instantaneous heart rate and respiration on coded tape recordings from seven asymptomatic siblings and 10 babies with symptomatic apnea who died of sudden infant death syndrome to 34 age- and sex-matched controls. We confirmed our previous observation of increased respiratory bandwidth, an index of variability in respiratory fre

Autonomic neuropathy after rubella infection.

A woman with autonomic neuropathy after rubella infection is described. She had features of both sympathetic and parasympathetic impairment, the major problems being postural hypotension, abdominal and parotid pain, and fatigue. Autonomic dysfunction persisted 38 months after the rubella infection with minimal response to treatment. The literature relating to autonomic neuropathy is reviewed, and the similarities of this patient's condition to the Guillain-Barré syndrome are discussed.

Paucifascicular congenital sensory neuropathy in identical twins.

A male infant had sensory and autonomic dysfunction, and his identical twin had a similar clinical finding. One twin was extensively studied, utilizing sural nerve, skin, and conjunctival biopsy specimens, to evaluate the status of peripheral sensory axons. The results support an antenatal neurodevelopmental disturbance in axonal growth that affects sensory neurons and limits their distal extension. Neuropathologic studies of this patient closely resemble findings in hereditary sensory neuropathy type II; clinically, however, this patient resembles patients with congenital autonomic dysfunctio

Autonomic effects of centrally administered dermorphin in conscious rabbits.

The administration of 1 microgram dermorphin into the mesencephalic ventricle of conscious rabbits modifies some autonomic functions, heart rate, respiratory rate and temperature decreasing in a statistically significant way. Naloxone completely reverses such effects. Pretreatment with drugs which inhibit serotoninergic, catecholaminergic, Gabaergic and cholinergic systems does not affect these central dermorphin actions.

[Status of nonspecific brain systems in various hypothalamic syndromes].

The nonspecific systems of the brain and their role in neuroendocrine regulation were studied and the nature of some neuroendocrine and metabolic disorders was specified. Examination of 41 women with the hypothalamic syndrome showed that they had ergotrophic insufficiency expressed both in the specific hypothalamus-pituitary system (hyperprolactinemia) and in nonspecific systems (deficiency of the activation systems of the brain sympathetic activity at the periphery and the emotional syndrome), which suggests the deficit of dopamine not only in the tuberoinfundibular but also in mesolimbic sys

Autonomic responses to glucose ingestion in elderly subjects with orthostatic hypotension.

The cardiovascular and plasma catecholamine responses to oral glucose (50 g) ingestion were investigated in five elderly subjects with orthostatic hypotension and five elderly control subjects. All the orthostatic hypotension subjects showed blood pressure falls after glucose ingestion, as compared to only one of the control subjects. Significantly greater falls in the orthostatic hypotension as compared to control patients were observed for systolic blood pressure (P less than 0.01) at 60 and 90 min following glucose and mean blood pressure (P less than 0.05) at 60 min following glucose. Heart rate response to facial immersion and apnea in quadriplegia.

Profound bradycardia is a common complication in the early posttraumatic period following cervical spinal cord damage. It is thought to be due to temporary inactivity of the sympathetic nervous system after separation from supraspinal control, coupled with unopposed parasympathetic dominance because of vagus nerve sparing. Hypoxia, underventilation, and tracheal suctioning appear to intensify the bradycardia. This study examined the effect of vagal stimulation using facial immersion and apnea on heart rate in patients with quadriplegia. Ten patients with quadriplegia (eight males and two femal

[Classification and symptomatology of migraine].

The different variants of migraine can be classified in common migraine, classical migraine and complicated migraine. Since objective methods of diagnostic proof are not available, diagnosis and categorisation have to be based on the patient's history and clinical criteria including hard and soft symptoms. Fundamental conditions are the evidence of recurrent headache attacks and the exclusion of other causes of headache.

Effects of digitalis on the human sick sinus node after pharmacologic autonomic blockade.

To study the effects of digitalis on the sinus node and the mechanisms involved, 16 patients with the sick sinus syndrome had electrophysiologic assessment of sinus nodal function during (1) control study, (2) after pharmacologic autonomic blockade with propranolol (0.2 mg/kg body weight and atropine sulfate 0.04 mg/kg intravenously), and (3) 10 minutes after 0.01 mg/kg of intravenous ouabain. The study was completed within 30 minutes of pharmacologic autonomic blockade. During the control study 50 percent of patients had an abnormal corrected sinus nodal recovery time or abnormal sinoatrial c

Inappropriate renin secretion and abnormal cardiovascular reflexes in coarctation of the aorta.

The response of the renin-angiotensin system to high and low sodium diets, to standing, and to saralasin infusion was assessed before and after surgical correction of aortic coarctation in a 27-year-old man. The cardiovascular responses to tests of autonomic function were measured. The heart rate responses to the Valsalva manoeuvre and standing were abnormal before operation, and plasma renin levels were high and renin secretion responded poorly to changes in dietary sodium, to standing, and to saralasin. Renin responsiveness and cardiovascular reflexes returned to normal after operation. The

Autonomic responses of transsexual and homosexual males to erotic film sequences.

Penile volume and galvanic skin responses to nude female and male film sequences were studied in 10 transsexual patients, 44 patients requesting treatment for homosexual impulses, and 60 heterosexual students. Student controls and homosexuals showed significantly greater galvanic skin responses to the preferred than to the nonpreferred sex. Transsexuals tended to show larger galvanic skin responses to females than did male homosexuals. No strong relationships were found between penile volume and galvanic skin response to the preferred sex. It is concluded that transsexual patients differ signi

Prevention of hypertensive crises in patients with high spinal lesions during cystoscopy and lithotripsy.

Three patients with high spinal lesions, admitted to the hospital for cystoscopy and lithotripsy, showed signs of severe hypertension due to mass reflex during this operation. This symptom was controlled with an intravenous drip of Arfonade¿, and the patients felt quite comfortable without any other anaesthesia. Other possibilities for handling such patients are discussed.
