Tranexamic acid may benefit patients undergoing total hip/knee arthroplasty because of haemophilia.

BACKGROUND: The lower limb joints, including hip and knee, are the most commonly involved joints in haemophilic arthropathy. With a higher risk of transfusion, total hip and knee arthroplasty (THA and TKA) are still the first choice after failure of conservative treatment. In the present study, we aimed to analyze clinical outcomes and complications rate after total joint arthroplasty of the lower limbs using tranexamic acid (TXA) or not. METHODS: Thirty-four patients with haemophilia A undergoing 24 TKA and 18 THA were evaluated in this retrospective study (No. 201302009). Based on using TXA

Effect of body position and ventilation on umbilical artery and venous blood flows during delayed umbilical cord clamping in preterm lambs.

OBJECTIVE: While delayed umbilical cord clamping (UCC) is thought to facilitate placental to infant blood transfusion, the physiological factors regulating flow in the umbilical arteries and veins during delayed UCC is unknown. We investigated the effects of gravity, by changing fetal height relative to the placenta, and ventilation on umbilical blood flows and the cardiovascular transition during delayed UCC at birth. METHODS: Catheters and flow probes were implanted into preterm lambs (128 days) prior to delivery to measure pulmonary, carotid, umbilical artery (UaBF) and umbilical venous (U…

Hepatitis E virus seroprevalence and molecular study among blood donors in China.

BACKGROUND: The risk of hepatitis E virus (HEV) infection from blood transfusion has aroused increasing concern in many countries. The aim of this study was to analyze the potential risk of HEV infection through blood transfusion in China. STUDY DESIGN AND METHODS: Qualified blood donations and donations with isolated alanine aminotransferase (ALT) elevations from five geographically diverse Chinese regions were tested for anti-HEV immunoglobulin (Ig)M and IgG and HEV antigen. The positive samples for anti-HEV IgM and HEV antigen were tested for HEV RNA. HEV open reading frame (ORF)2 partial …

Intravenous iron for perioperative anemia: recent advances, practical considerations, and future directions.

PURPOSE OF REVIEW: Perioperative anemia is common in surgical patients and is associated with increased morbidity, mortality, prolonged hospitalization, and higher rates of allogeneic red blood cell transfusion. Iron deficiency, either absolute or functional, represents the leading underlying cause. RECENT FINDINGS: Recent advances in patient blood management emphasize early diagnosis and targeted treatment of anemia, with iron supplementation as a cornerstone intervention. This review summarizes current evidence on mechanisms, indications, and clinical outcomes of perioperative iron therapy,

Acute Hemolytic Transfusion Reaction Due to Anti-Kpa Antibody.

BACKGROUND: Kpa (KEL3, Penney) is a red blood cell antigen within the Kell system, first described in 1957, that occurs in less than 2 percent of the population. Although anti-Kpa antibodies were identified in 2-5 percent of those with alloantibodies among patients requiring chronic transfusion, only five previously published case reports of anti-Kpa reactions were identified. CASE REPORT: Reported here is a case of an elderly female who experienced an acute hemolytic transfusion reaction due to this antigen. Following initiation of blood transfusion, she experienced a sudden onset of rigorou…

Evaluation of point-of-care versus laboratory antiglobulin-enhanced major crossmatch testing in cats.

OBJECTIVES: It is recommended that all cats, regardless of transfusion history, undergo major crossmatch prior to blood product administration to decrease the likelihood of immunological transfusion reactions. A reliable and user-friendly point-of-care crossmatch test would help make crossmatch testing more accessible. The primary objective of this study was to assess agreement between two antiglobulin-enhanced major crossmatch test methods, a laboratory-based tube crossmatch method and a point-of-care gel column crossmatch method, in cats in need of blood transfusion. MATERIALS AND METHODS: …

Chikungunya and Mayaro infective viruses in components of blood.

OBJECTIVE: Evaluate the infectivity of Alphavirus Chikungunya and Mayaro in blood products in plaque forming units (UFP/ml). BACKGROUND: Arboviruses are responsible for sporadic diseases or epidemics which cause serious public health issues. Due to the high number of asymptomatic infections and high viremia, blood donors may pass on these viruses by transfusion. METHODS/MATERIALS: This study used blood bags that would be discarded after evaluation and certification of the absence of infections. The blood products obtained by centrifuging a unit of whole blood were called blood components. Al

Modulation of erythropoietin formation by changes in blood volume in conscious dogs.

1. A possible influence of the filling of the circulatory system on the plasma concentration of erythropoietin, which is the major regulator of erythrocyte formation, was investigated in conscious dogs. 2. Over an experimental period of 5 h, the animals were subjected to either haemorrhage (hypovolaemia), blood volume expansion (hypervolaemia), or exchange transfusion of blood with dextran (isovolaemic anaemia). 3. A reduction of blood volume by 20% induced by haemorrhage increased plasma erythropoietin levels approximately 1.5-fold in the absence of significant changes in haematocrit. 4. An e

Evaluation of Rh-Hemolytic Disease in Neonates and Management with Early Intensive Phototherapy in the Neonatal Intensive Care Unit.

BACKGROUND OF THE STUDY: In neonates with Rh-hemolytic disease, light emitting diode (LED) phototherapy allows delivery of high spectral irradiance (SI). A linear correlation exists between SI and efficacy of phototherapy with no saturation point. There is scant data on evaluation and early phototherapy using LED units in Rh-hemolytic disease. OBJECTIVE: This study aimed to describe the hemoglobin (Hb), hematocrit (Hct), total serum bilirubin (TSB), phototherapy parameters and short-term outcomes in neonates with Rh-hemolytic disease. METHODOLOGY: Maternal parameters for Rh-isoimmunization w
