Management of brain metastases according to molecular subtypes.

The incidence of brain metastases has markedly increased in the past 20 years owing to progress in the treatment of malignant solid tumours, earlier diagnosis by MRI and an ageing population. Although local therapies remain the mainstay of treatment for many patients with brain metastases, a growing number of systemic options are now available and/or are under active investigation. HER2-targeted therapies (lapatinib, neratinib, tucatinib and trastuzumab emtansine), alone or in combination, yield a number of intracranial responses in patients with HER2-positive breast cancer brain metastases. N…

Cognitive Control: Brain Oscillations Coordinate Human Working Memory.

A new study has found that rhythmical brain stimulation at theta frequency (5Hz) over prefrontal cortex and stimulation at alpha frequency (10Hz) over posterior parietal cortex increase visual working memory capacity, the effects being due respectively to prioritisation of target information and suppression of distracting information.

Brain metastases from esophageal cancer: A case report.

INTRODUCTION: At present, there is no uniform consensus on the treatment of brain metastases from esophageal cancer. The studies on the treatment of brain metastases from esophageal cancer by radiotherapy combined with temozolomide (TMZ) are even rarer. PATIENT CONCERNS: A 69-year-old woman was admitted to our hospital for brain metastases from esophageal cancer after thoracic irradiation. DIAGNOSES: Magnetic resonance imaging of the head showed a round, heterogeneous metastatic tumor in the left parietal lobe. Brain magnetic resonance imaging showed edema around brain metastasesIntervention…

Intensity normalization methods in brain FDG-PET quantification.

BACKGROUND: The lack of standardization of intensity normalization methods and its unknown effect on the quantification output is recognized as a major drawback for the harmonization of brain FDG-PET quantification protocols. The aim of this work is the ground truth-based evaluation of different intensity normalization methods on brain FDG-PET quantification output. METHODS: Realistic FDG-PET images were generated using Monte Carlo simulation from activity and attenuation maps directly derived from 25 healthy subjects (adding theoretical relative hypometabolisms on 6 regions of interest and f…

Noninvasive Brain Stimulation & Space Exploration: Opportunities and Challenges.

As NASA prepares for longer space missions aiming for the Moon and Mars, astronauts' health and performance are becoming a central concern due to the threats associated with galactic cosmic radiation, unnatural gravity fields, and life in extreme environments. In space, the human brain undergoes functional and structural changes related to fluid shift and changes in intracranial pressure. Behavioral abnormalities, such as cognitive deficits, sleep disruption, and visuomotor difficulties, as well as psychological effects, are also an issue. We discuss opportunities and challenges of noninvasive…

Nonstationary Model of Oxygen Transport in Brain Tissue.

The paper addresses the mathematical study of a nonstationary continuum model describing oxygen propagation in cerebral substance. The model allows to estimate the rate of oxygen saturation and stabilization of oxygen concentration in relatively large parts of cerebral tissue. A theoretical and numerical analysis of the model is performed. The unique solvability of the underlying initial-boundary value problem for a system of coupled nonlinear parabolic equations is proved. In the numerical experiment, the tissue oxygen saturation after hypoxia is analyzed for the case when a sufficient amount

Stem Cell Therapy for Pediatric Traumatic Brain Injury.

There has been a growing interest in the potential of stem cell transplantation as therapy for pediatric brain injuries. Studies in pre-clinical models of pediatric brain injury such as Traumatic Brain Injury (TBI) and neonatal hypoxia-ischemia (HI) have contributed to our understanding of the roles of endogenous stem cells in repair processes and functional recovery following brain injury, and the effects of exogenous stem cell transplantation on recovery from brain injury. Although only a handful of studies have evaluated these effects in models of pediatric TBI, many studies have evaluated …

Modeling of [18F]FEOBV Pharmacokinetics in Rat Brain.

PURPOSE: [18F]Fluoroethoxybenzovesamicol ([18F]FEOBV) is a radioligand for the vesicular acetylcholine transporter (VAChT), a marker of the cholinergic system. We evaluated the quantification of [18F]FEOBV in rats in control conditions and after partial saturation of VAChT using plasma and reference tissue input models and test-retest reliability. PROCEDURE: Ninety-minute dynamic [18F]FEOBV PET scans with arterial blood sampling were performed in control rats and rats pretreated with 10 μg/kg FEOBV. Kinetic analyses were performed using one- (1TCM) and two-tissue compartmental models (2TCM),

Brain Atrophy Associated with Primary Diffuse Meningeal Melanomatosis.

BACKGROUND: Primary meningeal melanomatosis is a rare leptomeningeal tumor, and the diagnosis is challenging due to nonspecific clinical symptoms and radiologic findings. CASE DESCRIPTION: A 21-year-old man presented with recurrent seizure and impaired memory. Cranial magnetic resonance imaging showed obvious brain atrophy with bilateral extensive meningeal enhancement in the supratentorial region. Diffusion-weighted imaging and fluid-attenuated inversion recovery showed slightly hyperintensive signals in the cortex. Microscopic examination revealed invasion of pigment into the Virchow-Robin …

Global multisensory reorganization after vestibular brain stem stroke.

OBJECTIVE: Patients with acute central vestibular syndrome suffer from vertigo, spontaneous nystagmus, postural instability with lateral falls, and tilts of visual vertical. Usually, these symptoms compensate within months. The mechanisms of compensation in vestibular infarcts are yet unclear. This study focused on structural changes in gray and white matter volume that accompany clinical compensation. METHODS: We studied patients with acute unilateral brain stem infarcts prospectively over 6 months. Structural changes were compared between the acute phase and follow-up with a group of health…

Studying the visual brain in its natural rhythm.

How the brain fluidly orchestrates visual behavior is a central question in cognitive neuroscience. Researchers studying neural responses in humans and nonhuman primates have mapped out visual response profiles and cognitive modulation in a large number of brain areas, most often using pared down stimuli and highly controlled behavioral paradigms. The historical emphasis on reductionism has placed most studies at one pole of an inherent trade-off between strictly controlled experimental variables and open designs that monitor the brain during its natural modes of operation. This bias toward si…

Visual working memory deficits following right brain damage.

Visual working memory (VWM) involves the encoding and maintenance of visual information over time, with the requirement that object features be accurately bound to spatial locations. We and others have shown that damage to the right hemisphere leads to impaired spatial working memory. Here, we test the notion that right brain damage (RBD) may have consequences for domain general VWM. We had eight RBD patients and a group of healthy control participants perform a VWM task under different loads (1 to 3 items to recall) and spatial competition (high vs. low). All participants were asked to rememb…

Do painters need their whole brain to excel?

Skilled professional artists are sometimes able to maintain their talents while other cognitive functions deteriorate due to brain diseases. The objective of this study is to asses the preserved artistry of a professional painter in spite of the presence of strokes affecting brain areas implicated in art expression. She had a neurologic evaluation and brain imaging after the stroke; painter-curators analyzed and compared the painter's pictorial artwork created before and after the stroke. In spite of cerebellar, visuospatial, motor, cognitive, and functional deficits likely related to strokes …

Fluid Overload in Pediatric Severe Traumatic Brain Injury.

OBJECTIVE: Pediatric traumatic brain injury is a major public health problem in the United States. Hypertonic saline therapy is a well-established treatment in patients with severe traumatic brain injury (Glasgow Coma Scale ≤ 8) who have intracranial hypertension. In children, fluid overload is associated with increased mortality, ventilator duration, and length of PICU stay, even when controlling for severity of illness. This study reports prevalence of fluid overload in pediatric patients with severe traumatic brain injury treated with 3% hypertonic saline and effect on clinical outcomes. D…

S100A6 and Its Brain Ligands in Neurodegenerative Disorders.

The S100A6 protein is present in different mammalian cells and tissues including the brain. It binds Ca2+ and Zn2+ and interacts with many target proteins/ligands. The best characterized ligands of S100A6, expressed at high level in the brain, include CacyBP/SIP and Sgt1. Research concerning the functional role of S100A6 and these two ligands indicates that they are involved in various signaling pathways that regulate cell proliferation, differentiation, cytoskeletal organization, and others. In this review, we focused on the expression/localization of these proteins in the brain and on their …
