Endoscopic laser coagulation of radiation-induced mucosal vascular lesions in the upper gastrointestinal tract and proximal colon.

UNLABELLED: Endoscopic laser coagulation effectively controls bleeding from radiation-induced rectal vascular lesions. OBJECTIVE: To assess the outcome of endoscopic treatment of radiation-induced bleeding due to vascular lesions located proximal to the sigmoid colon. METHODS: We identified 15 consecutive patients with such proximal radiation enteropathy treated at our Institution with Nd:YAG laser between 1984 and 1991. Ten patients (66%) had gastric and/or small bowel involvement, and five (33%) had colonic involvement with or without more proximal lesions. Bleeding first appeared at a mea

[Synchronous and metachronous tumors of the colon and rectum].

Three hundred and seventy-two patients with colorectal tumours treated by curative resection between January 1982 and January 1992 were reviewed in order to determine the role of colonoscopy and the outcome of patients with multiple tumours. Thirty (8.1%) of them with a mean age of 57 (35-79) years (20 males, 10 females) had synchronous (19 cases) or metachronous (11 cases) lesions. Rectum and sigmoid colon were the most frequent site of multiple lesions, accounting for 73% of all lesions. Accurate pre-operative diagnosis was performed in 14 of the 19 patients with synchronous lesions, and in

Continent urinary reservoir in exstrophy/epispadias complex.

The outcome of 15 children and young adults who underwent reconstruction of continent urinary reservoirs (CUR) from January 1987 to 1990 is presented. Ten patients were male and 5 female with an age range of 3 to 20 years. There were 13 patients with bladder exstrophy and 2 with incontinent epispadias. In 8 cases the urinary diversion was performed for an inadequate bladder capacity following successful closure (3), failed attempted closure (3), female epispadias (1) and following successful urethroplasty in 1 case of male epispadias. A tiny fibrotic bladder plate unsuitable for attempted clos

A study in general practice of the symptoms and delay patterns in the diagnosis of gastrointestinal cancer.

GASTROINTESTINAL CANCER WAS CLASSIFIED INTO FOUR GROUPS ACCORDING TO THE SITE: stomach, caecum and ascending and transverse colon, sigmoid colon and rectum. The incidence of these cancers in general practice is as rare as three per 10,000 consultations. I report on a study in general practice of the symptoms and delays in diagnosis in 150 patients with gastrointestinal cancer. There was an interval of many weeks between the onset of symptoms and diagnosis in the majority of cases. In approximately 50 per cent of cases there was an interval of weeks between the patient consulting the general pr…

Segmentectomy of a Peripheral Pulmonary Nodule Following Bronchoscopic ICG-Coil Localization: A Case Series.

Preoperative localization of small, non-subpleural pulmonary nodules is a challenge in minimally invasive segmentectomy. Bronchoscopic placement of indocyanine green (ICG)-soaked microcoils enables intraoperative detection via near-infrared fluorescence (NIR), improving the precision and safety of resection. We report three patients with CT-suspected solid pulmonary nodules who underwent bronchoscopic ICG-coil placement. The interval between localization and surgery ranged from 1 to 7 days. All patients underwent uni- or biportal video-assisted thoracoscopic segmentectomy. The ICG coil was cle

Barolith precipitation: an uncommon sequela following barium imaging.

We present the case of a young male in his mid-30s who developed baroliths, a rare complication, following a barium contrast study. The patient initially presented with abdominal pain, vomiting and constipation. Imaging revealed two large masses of retained contrast material within the sigmoid colon and rectum, consistent with baroliths. Despite initial conservative management, including enema and electrolyte correction, the patient required surgical intervention due to persistent symptoms. An exploratory laparotomy and rigid sigmoidoscopy were performed to remove the impacted stool, leading t

Progesterone-Only Contraceptive-Induced Ischemic Colitis.

Ischemic colitis (IC) occurs when there is a temporary lack of adequate blood supply to the intestines, particularly in vulnerable areas such as the splenic flexure and sigmoid colon, which lack sufficient collateral blood vessels. Although primarily seen in older individuals with atherosclerotic risk factors, IC can also be triggered by hormonal contraceptives in young women. Although estrogen-containing oral contraceptives are known to increase thromboembolic risk, the impact of progesterone is not well understood. We report a unique case of biopsy-confirmed IC in a previously healthy 30-yea

Aortoiliac graft-enteric fistula presenting as gastrointestinal hemorrhage: A report on a complex case management.

KEY CLINICAL MESSAGE: Iliac artery-enteric fistula is a rare cause of lower GI bleeding and can cause life-threatening consequences. A high degree of clinical suspicion is needed in patients with previous aortic surgery to allow early multidisciplinary intervention. ABSTRACT: This case study discusses the staged management of a 78-year-old patient presenting with life-threatening lower gastrointestinal (GI) bleeding secondary to an aortoiliac graft-enteric fistula (GEF) into the sigmoid colon on the background of an adenocarcinoma and diverticular disease. The patient had an aorto bi-iliac sy

Robotic natural orifice specimen extraction surgery (NOSES) for anterior resection.

Minimally invasive colorectal surgery is currently well-accepted, with open techniques being reserved for very difficult cases. Laparoscopic colectomy has been proven to have lower mortality, complication, and ostomy rates; a shorter median length of stay; and lower overall costs when compared to its open counterpart. This trend is seen in both benign and malignant indications. Natural orifice specimen extraction surgery (NOSES) in colorectal surgery was first described in the early 1990s. Three recent meta-analyses comparing transabdominal extraction against NOSES concluded that NOSES was sup

Robotic Repair of a Symptomatic Perineal Hernia With Self-Fixating Synthetic Mesh.

Primary acquired perineal hernias are rare defects through the pelvic floor diaphragm. The optimal surgical technique for repair remains unknown, and recurrence rates approach 50%. We present a 65-year-old female without previous obstetric or pelvic surgical history who was found to have herniated sigmoid colon through a 2×2 cm levator ani defect. The patient underwent robotic transabdominal hernia repair with a synthetic self-fixating underlay mesh. The peritoneum was primarily closed and the patient was discharged the same day. There is no sign of recurrence to date. Our minimally invasive a

On the border of medical specialties: ovarian metastasis from colorectal cancer.

The colon cancer (CC) is the fourth most common cancer in the world. It is associated with metastatic spread in 50% of cases in the course of the disease. Common sites for synchronous metastases from colorectal cancer are the lung, liver, peritoneum, bone and brain. The frequency of ovarian metastasis from CC varies widely from 1.6 to 7.4%. This type of metastasis is difficult to distinguish clinically from primary ovarian neoplasms. We present a case of a 49-years old woman admitted to the Department of General Surgery at the 5th Military Clinical Hospital in Cracow for elective surgery for m

Laparoscopic Hartmann's reversal combined with interval lymphadenectomy after emergency surgery for perforated colon cancer: A case report.

A 55-year-old man underwent Hartmann's procedure for generalized peritonitis caused by perforation of sigmoid colon cancer, which was histologically diagnosed as tubular adenocarcinoma penetrating the muscularis propria (T3). The extent of lymph node dissection was insufficient for the advanced cancer, according to the concept of complete mesocolic excision. Two months after surgery, he underwent laparoscopic Hartmann's reversal, combined with interval lymphadenectomy. En bloc resection of the colostomy, rectal stump and associated mesentery was performed with high ligation of the inferior mes

Abdominal ectopic pregnancy complicated with a large bowel injury: a case report.

BACKGROUND: Abdominal pregnancy accounts for 0.6 to 4% of all ectopic pregnancies. Due to delays in diagnosis and difficulties in the management of abdominal pregnancy, the risk of mortality is significantly higher than for uncomplicated ectopic pregnancies. A 23 years-old gravida-II, ectopic-I Ethiopian woman was initially managed as a case of missed second trimester abortion. Later on, abdominal ectopic pregnancy was diagnosed with ultrasound and she underwent a laparotomy. Though fetus and placenta was removed successfully without significant hemorrhage, there was inadvertent sigmoid colon

Plastic bread clip impacted in gastrointestinal tract: a case report and review of the literature.

A 79-year-old initially presented to her general practitioner for a 6-week history of nausea, intermittent abdominal pain and altered bowel habits. Colonoscopy was performed to investigate her symptoms and revealed a bread clip that was adherent to the sigmoid colon mucosa, unable to be retrieved. A CT scan was performed to exclude any associated complication, which showed no evidence of perforation or collection. A repeat colonoscopy by a more experienced endoscopist again failed to retrieve the bread clip. Eventually, the bread clip had to be removed through a laparotomy and colotomy. We per

[A Case of Laparoscopic Hepatectomy and Combined Resection of Lymph Nodes and Ureter for Liver Metastasis after Colorectal Cancer Surgery, with Local Lymph Node Recurrence and Bilateral Hydronephrosis].

A 65-year-old man with sigmoid colon cancer underwent sigmoidectomy, followed by 8 courses of oral S-1 as postoperative adjuvant chemotherapy. Three years and 3 months after surgery, the patient developed liver metastasis, lymphadenopathy at the root of the inferior mesenteric artery, and bilateral hydronephrosis. The left hydronephrosis was believed to be due to invasion by lymph node metastasis at the root ofthe inferior mesenteric artery. The patient underwent laparoscopic partial resection ofsegment 5 ofthe liver, excision ofthe lymph nodes at the root ofthe inferior mesenteric artery(comb

Desmoid Tumor at the Site of Endometriosis Surgery, Coincident with the Use of Oral Contraceptives.

Desmoid tumors, which are often estrogen-dependent, frequently develop in surgical wounds. Here we report the case of 33-year-old woman with a 4-cm solid mass detected in her left adnexal area. She had previously undergone a laparoscopic surgery for endometriosis at age 29 years and had been using a combined oral contraceptive (COC) to prevent recurrence. The mass was diagnosed as a uterine myoma on the basis of ultrasonography and magnetic resonance imaging. Gonadotropin-releasing hormone agonist therapy for 3 months resulted in shrinkage of the tumor. Using a second laparoscopy, we identifie

Case of cytomegalovirus colitis in an immunocompetent patient: a rare cause of abdominal pain and diarrhea in the elderly.

Cytomegalovirus (CMV) colitis usually occurs in immunocompromised patients after undergoing organ transplantation or chemotherapy. We report the case of a 60-year-old immunocompetent Japanese woman who presented with abdominal pain and bloody diarrhea. She was initially diagnosed as having ischemic colitis with pseudomembranous colitis on the basis of her symptoms, Clostridium difficile antigen positivity, and colonoscopic findings, which showed ulcer formation from the sigmoid colon to rectum. In spite of bowel rest and administration of metronidazole, her symptoms did not improve. On follow-
