Neuroplastin expression is essential for hearing and hair cell PMCA expression.

Hearing deficits impact on the communication with the external world and severely compromise perception of the surrounding. Deafness can be caused by particular mutations in the neuroplastin (Nptn) gene, which encodes a transmembrane recognition molecule of the immunoglobulin (Ig) superfamily and plasma membrane Calcium ATPase (PMCA) accessory subunit. This study investigates whether the complete absence of neuroplastin or the loss of neuroplastin in the adult after normal development lead to hearing impairment in mice analyzed by behavioral, electrophysiological, and in vivo imaging measureme…

Novel Usher syndrome pathogenic variants identified in cases with hearing and vision loss.

BACKGROUND: Usher syndrome, the most common form of inherited deaf-blindness, is unlike many other forms of syndromic hereditary hearing loss in that the extra aural clinical manifestations are also detrimental to communication. Usher syndrome patients with early onset deafness also experience vision loss due to progressive retinitis pigmentosa that can lead to legal blindness in their third or fourth decade. METHODS: Using a multi-omic approach, we identified three novel pathogenic variants in two Usher syndrome genes (USH2A and ADGRV1) in cases initially referred for isolated vision or hear…

Central auditory development in children with cochlear implants: clinical implications.

A common finding in developmental neurobiology is that stimulation must be delivered to a sensory system within a narrow window of time (a sensitive period) during development in order for that sensory system to develop normally. Experiments with congenitally deaf children have allowed us to establish the existence and time limits of a sensitive period for the development of central auditory pathways in humans. Using the latency of cortical auditory evoked potentials (CAEPs) as a measure we have found that central auditory pathways are maximally plastic for a period of about 3.5 years. If the

There is a price in terms of hearing function, although this may compare favourably with the deafness associated with the natural history of the disease, and with surgery. In deciding on therapy, patients should be aware of this treatment option.

New window for cochlear implant insertion.

Nine patients with profound bilateral hearing loss received a cochlear implant via the middle fossa. Subjects presented with a bilateral radical mastoidectomy cavity, fibroadhesive otitis media, otosclerosis, autoimmune inner ear disease, previous cranial trauma and genetic prelingual deafness. A classic middle fossa approach was adopted. A small cochleostomy was performed on the most superficial part of the basal turn. A Nucleus CI24M cochlear implant system (Cochlear Corporation) was inserted in four patients, a Lauraflex implant (Philips Hearing Implants) in three patients and a Combi 40 + …

Implantation of a transcutaneous, bone-anchored, piezoelectric bone conduction hearing aid on the squama temporalis.

Bone-anchored hearing implants, such as the Osia® system (Cochlear Deutschland GmbH, Hanover, Germany), are used to rehabilitate conductive hearing loss, mixed hearing loss, and single-sided deafness. The surgical technique presented anchors the OSI300 supraauricularly and subperiosteally over the squama temporalis, thereby avoiding the creation of an implant bed in the mastoid and reducing cosmetic contour deformity. Cranial-parietotemporal placement allows bypassing absent or previously operated bone mastoid structures. Postoperatively, the implant was proved to be stable and inconspicuous, …

Pregnant woman with Jervell and Lange-Nielsen syndrome: a case report.

Jervell and Lange-Nielsen syndrome (JLNS) is a rare autosomal recessive disorder characterised by congenital sensorineural hearing loss and prolonged QT interval, predisposing to ventricular arrhythmia and sudden cardiac death. We report a 32-year-old woman with congenital deafness and recurrent syncope since childhood, diagnosed with JLNS due to a homozygous KCNQ1 exon 14 (c.1716-1719del) variant. She had an implantable cardioverter-defibrillator and was maintained on propranolol. During pregnancy, she developed fetal growth restriction and delivered a healthy neonate vaginally under continuo…

Clinical gene therapy restores hearing: a paradigm shift.

Recent breakthroughs in gene therapy for autosomal recessive deafness 9 (DFNB9) caused by OTOF mutations have transformed treatment paradigms for hereditary hearing loss (HHL). To date, eight clinical trials targeting DFNB9 have been registered in 51 centers across eight countries, demonstrating the rapid progress of gene therapy in auditory medicine. These pioneering studies establish the framework for the clinical translation of gene therapy targeting HHL. This review synthesizes progress in OTOF-related clinical trials, highlighting translational foci such as inner ear drug delivery, trial …

Chudley-McCullough syndrome: A report of a rare syndromic sensorineural hearing loss.

Chudley-McCullough syndrome (CMS) is a rare autosomal recessive disorder. It is characterized by the association of early-onset sensorineural hearing loss and typical brain malformations, in contrast with preserved or only mildly affected psychomotor development. The first cases were reported in 1997 by Chudley et al. in a brother and sister born to consanguineous parents. It was not until 2012 that Doherty et al. identified mutations in the GPSM2 gene (G-protein signaling modulator 2) as the cause of CMS. We present the case of CMS in a 3-year-old girl with bilateral sensorineural deafness an…

Hepatic manifestations in VPS53-related pontocerebellar hypoplasia type 2E: A case report.

Pathogenic variants in VPS53 are associated with pontocerebellar hypoplasia type 2E (PCH2E), characterized by microcephaly, severe neurodevelopmental impairment and epilepsy. We present a case of a female neonate with VPS53 pathogenic variants exhibiting the classic phenotypic features along with liver disease and deafness, which had not been described in previously reported cases. Similarly, while liver abnormalities have been reported in patients with mutations in other genes coding for proteins of the GARP or EARP complex, of which VPS53 is a subunit, liver disease has not been described in…

Stroke Strikes in Silence: Bilateral Medial Medullary Infarction in a Patient With Complete Hearing Loss.

Bilateral medial medullary infarction (BMMI) is a rare stroke subtype that accounts for less than 1% of acute strokes. Common manifestations of this stroke include quadriparesis, bilateral hypoglossal palsy, bilateral sensory loss, and respiratory failure. We present the case of a 39-year-old male with deafness and mutism who was brought to the emergency department due to acute onset of altered mental status and generalized weakness, further decompensated, and was lately diagnosed with bilateral medial medullary infarction. This case hopes to illustrate a differential diagnosis to be considere…

[Kidney involvement in rare hereditary diseases].

Various rare inherited disorders can be associated with kidney involvement, including glomerulopathies, tubulopathies, multiple cysts, congenital anomalies of the kidneys and urinary tract, urolithiasis, malignant and benign tumors. Genetic nephropathy should be always considered in children, adolescents and young patients with the kidneys or urinary tract disorders and/or patients with positive family anamnesis. Extrarenal manifestations can be a valuable clue for diagnosis of certain hereditary diseases, e.g. neurosensory deafness in Alport syndrome or photofobia in nephropathic cystinosis. …
