With the exception of stress incontinence, the most common urologic problems unique to female patients are vesicovaginal fistulas and urethral diverticula. The author describes modifications of a Foley catheter both to facilitate distention of the vagina for vaginoscopy and for urethrography and use of the visual urethrotome for urethroscopy.

We report on 47 patients, surveyed and treated by us, diagnosed of primary vesical diverticula, having thus excluded all those secondary to … Surgery was performed on 34 patients, the indication being made on the basis of the big size of the diverticulum, its clinical repercussion, or because of its association with vesicoureteral reflux or any other kidney malformation. We employ an intravesical approach for the small diverticula with attendant reflux, but for resection of the sizeable ones, irresp …

Diverticular disease of the colon is frequent in developed countries, probably due to diets poor in fiber. Prevalence increases with age, reaching 65 percent at 70 years. The preferred seat of the disease is the sigmoid colon. The disease is usually asymptomatic, but inflammatory signs are seen in 10 to 25 percent of cases. Diagnosis of diverticulitis is difficult since correlation between clinical signs and histological data is often mediocre. Surgery is carried out in 15 to 30 percent of patients presenting severe disorders, i.e. in 1 to 2 percent of the overall population presenting diverti

One hundred and thirteen cases of juxtapapillary duodenal diverticula were found in 5573 cases which were examined by upper gastrointestinal barium meal X-ray examination. Seventy-nine percent of those 113 cases had complications, while 21% of the cases were suffered from juxtapapillary duodenal diverticula alone. Among the complications, cholelithiasis showed the highest incidence. (33 cases out of 113 cases, 29.2%). Twenty-four cases of juxtapapillary duodenal diverticula of which serum amylase and serum bilirubin values were found to be over the normal range, were subjected to the ERCP and

In the elderly population bleeding from the colon is not unusual. In evaluating a patient with colonic bleeding, the rate of bleeding should be assessed, and the possibility of upper gastrointestinal and anorectal bleeding should be excluded. The most common cause of massive bleeding from the colon in the elderly patient is diverticulosis, and the next most common cause is angiodysplasia. A number of other less common causes of bleeding should be excluded when making the diagnosis. If angiography is employed and the bleeding site is identified, initial therapy can be started concurrently by ei

The Authors study the statistics of the Verona Surgical Centre, concerning 5 cases of colovesical fistula. They support the two-time operation, that is, the colon resection with colo-colic anastomosis sheltered by a decompressive transversostomy and stitching of the vesical breach. They affirm the usefulness of the operation according to Hartmann in patients facing a greater operative risk.

A case of multiple gastric diverticula with pyloric stenosis is reported here. More than 30 diverticula were located throughout the stomach except in the pyloric antrum. The diverticula were divided into two types according to shape: shallow depression and mushroom-like outpouching. This case has the greatest number of gastric diverticula ever reported.

Lateral pharyngeal "pouches" in young men are described. They are not to be confused with true laryngeal cells as often mentioned in the literature. The radiographic diagnostic of the pharyngeal "pouches" after barium swallow together with the Valsalva test is demonstrated in review of the own casuistic. Increased intraluminal pressure caused by instrument- and glassblowers may be an etiologic factor as well as an acquired defect in the lateral pharyngeal wall.

The preeminence of sonography in the diagnosis of obstructive jaundice has already been established. Earlier reports have described a variety of radiologic modalities used to establish the diagnosis of choledochal cyst. Our case illustrates the usefulness of sonography in differential consideration, though either transhepatic or operative cholangiography is necessary to further define the point and nature of the obstructive process and to determine that the cyst communicates with the common bile duct.

Coloscopy was accurate in 83% of patients referred because of a radiological suspicion of malignancy in a diverticula-bearing sigmoid colon. In 17% the sigmoid could not be reached or inspected because of deformity or stenosis. All 13 coexisting carcinomas were correctly diagnosed endoscopically except for one which could not be reached. Coloscopy is therefore a valuable adjunct in detecting or eliminating cancer in colonic diverticular disease.

Patients with diverticular disease should be categorized for therapeutic purposes into 3 groups: (1) asymptomatic diverticulosis (accidental finding; (2) painful diverticular disease (no inflammation); and (3) acute diverticulitis (pericolitis present). In asymptomatic diverticulosis a high-fiber diet should be given, and an operation is not indicated. In painful diverticular disease resection of the sigmoid colon should be considered after recurrent attacks. In acute diverticulitis resection should be performed after the second inflammatory attack. The younger the patient, the earlier an oper

Granulomatous inflammation typifying Crohn's disease was centred within or confined to appendices in six patients, two of whom developed lesions attributable to Crohn's disease elsewhere in the gut. The remaining four patients have remained symptom-free for periods varying from two to six years. Histological evidence of Crohn's disease was also present in five of 46 appendices available for re-examination in a survey of 63 cases of Crohn's enterocolitis. It is adduced that appendiceal involvement in Crohn's disease is not uncommon.
