Therapeutic efficacy of granulocyte-macrophage colony-stimulating factor in patients with idiopathic acquired alveolar proteinosis.

Alveolar proteinosis (AP) is characterized by excessive surfactant accumulation, and most cases are of unknown etiology. Standard therapy for AP is whole-lung lavage, which may not correct the underlying defect. Because the hematopoietic cytokine granulocyte-macrophage colony-stimulating factor (GM-CSF) is required for normal surfactant homeostasis, we evaluated the therapeutic activity of GM-CSF in patients with idiopathic AP. Fourteen patients received 5 microg/kg/d GM-CSF for 6 to 12 wk with serial monitoring of the alveolar-arterial oxygen gradient ([A-a]DO2), diffusing capacity of carbon

Effect of epinephrine infusion on chest pain in syndrome X in the absence of signs of myocardial ischemia.

Eight female patients (aged 51 to 65 years) with New York Heart Association class II angina pectoris, normal coronary angiograms, normal hyperventilation, and abnormal exercise stress tests (chest pain and ST depression), and 5 sex- and age-matched controls participated in this study. Epinephrine was given intravenously to both patients and controls at 5-minute intervals in doses of 0.1, 0.2, 0.3, 0.4, and 0.6 nmol/kg/min. After rest (15 minutes), the alpha-adrenoceptor antagonist phentolamine or placebo was administered intravenously to patients in a double-blind, crossover study on 2 separat

Sensitization to aztreonam and cross-reactivity with other beta-lactam antibiotics in high-risk patients with cystic fibrosis.

The immunogenicity, allergenicity, and cross-reactivity of aztreonam were investigated in 21 patients with cystic fibrosis (CF) (aged 5 to 39 years) with well-documented histories of allergic systemic reactions (SRs) to penicillin and/or cephalosporin antipseudomonal beta-lactam antibiotics (BLAs). Skin tests (STs) with penicilloyl-polylysine (PPL), penicillin minor determinant mixture, and antipseudomonal BLA were positive in 19 patients (90%). The BLA causing the most recent allergic reaction, minor determinant mixture, or PPL, was positive in 89%, 53%, and 32% of ST-positive patients, respe

[Clinical studies of the antiarrhythmic efficacy of disopyramide phosphate (Norpace) (author's transl)].

The antiarrhythmic action of Norpace was investigated in three clinical studies. 1. A single blind placebo controlled study in 10 centers confirmed the antiarrhythmic action of Norpace in 120 patients with ventricular or supraventricular cardiac arrhythmias. It was shown by means of continuous ECG recordings that Norpace produced a statistically significant reduction in the frequency of both ventricular and supraventricular extrasystoles. Paroxysmal ventricular tachycardia was prevented in 70% of the patients. 2. In a double blind comparative study of Norpace and quinidine sulphate a similar a

Multifactorial and multiple component interventions for preventing falls in older people living in the community.

RATIONALE: Multifactorial fall prevention interventions are those where intervention components are tailored to each person's individual fall risk factor profile. Multiple component interventions provide the same components to everyone. The evidence on the effects of these interventions is inconclusive. This review updates one published on the Cochrane Library in 2018. OBJECTIVES: To assess the effects of multifactorial and multiple component fall prevention interventions compared to usual care or attention control, exercise, and falls prevention advice or education in community-dwelling olde

Blood flow restriction with different loads versus moderate intensity resistive exercises in patients with knee osteoarthritis: a single-blind randomized controlled trial.

BACKGROUND: Knee osteoarthritis (KOA) is a common condition causing pain, muscle weakness, and functional impairment. In addition, many patients cannot tolerate resistance training due to pain. Blood flow restriction (BFR) has been proposed as a low-load alternative strategy to improve muscle function, but the effectiveness of passive BFR application remains unclear in KOA. Therefore, this study compared passive BFR at 50% and 70% limb occlusion pressure (LOP) with moderate-intensity resistance training (MI-RT) in KOA. METHODS: A three-arm, parallel groups, single-blind randomized controlled

Adjuvant Treatment with 5-Fluorouracil and Oxaliplatin Does Not Influence Cardiac Function, Neurovascular Control, and Physical Capacity in Patients with Colon Cancer.

BACKGROUND: Adjuvant chemotherapy with 5-fluorouracil (5-FU) and oxaliplatin increases recurrence-free and overall survival in patients with colon adenocarcinoma. It is known that these drugs have been associated with cardio- and neurotoxicity. We investigated the effects of 5-FU ± oxaliplatin on cardiac function, vascular responses, neurovascular control, and physical capacity in patients with colon cancer. METHODS: Twenty-nine patients with prior colectomy for stage II-III adenocarcinoma and clinical indication for adjuvant chemotherapy were allocated to receive 5-FU (n = 12) or 5-FU + oxal

WITHDRAWN: Trimetazidine for stable angina.

BACKGROUND: Patients with stable angina not controlled by monotherapy with nitrates, beta blockers, or calcium channel blockers are often treated with combinations of these drugs. There may be adverse effects from, or contraindications to, the use of combinations. In low risk groups, medical treatment appears to be as good an option as percutaneous transluminal coronary angioplasty in terms of averting myocardial infarction, death, or subsequent revascularization. Revascularization procedures are too costly or inaccessible for many patients in developing countries therefore effective and safe

Advances in disease management and treatment outcomes for obesity and knee osteoarthritis in primary care practice.

The lifetime risk of developing knee osteoarthritis (KOA) is approximately doubled in people with obesity compared with those without obesity. Many patients with KOA are treated by primary care physicians in the community setting. We performed a targeted literature search on PubMed to identify relevant literature describing the impact of weight loss in patients with obesity and KOA. A strong body of evidence supports the use of weight loss through lifestyle interventions, including caloric restriction in combination with exercise, as a primary intervention in patients with KOA and obesity. The effects of ontogeny in variable temperature and flow waters on the locomotor physiology and morphology of Icelandic arctic charr (Salvelinus alpinus).

Climate change is causing increases in water temperature, flow and their variability in many freshwaters. Predicting fish futures will require knowing how they respond to variations in temperature and flow. This study tested the null hypothesis that locomotor physiology and morphology of Arctic charr are independent of the temperature and flow of water experienced during ontogeny. We raised cultured charr from hatch under a 2 X 2 matrix of flow (3 cm s-1 &15 cm s-1) and temperature (5°C and 13°C) in the laboratory. Fish were compared for sprinting performance (Usprint), swimming kinematics and

Efficacy and safety of N-acetylcysteine in patients with mild cognitive impairment undergoing exercise-based cardiac rehabilitation program: a randomized controlled trial.

BACKGROUND: Oxidative stress is implicated in the etiology of mild cognitive impairment. We hypothesized that oral supplementation with N-acetylcysteine, a precursor to the antioxidant glutathione, would improve cognitive outcomes among individuals with mild cognitive impairment. METHODS: n this 24-week randomized, double-blind, placebo-controlled trial, participants enrolled in an exercise-based cardiac rehabilitation program with possible vascular mild cognitive impairment were randomized to receive either oral N-acetylcysteine (2400 mg/day) or placebo. Cognition was assessed using a neurops

Efficacy and safety of low- and high-intensity magnetic field therapies for orthopedic pain: a systematic review.

INTRODUCTION: Orthopedic and musculoskeletal pain is a leading cause of disability worldwide, and many patients continue to experience suboptimal relief despite advances in pharmacological and physical therapies. Magnetic field-based interventions, including pulsed electromagnetic field therapy (PEMF) and repetitive peripheral magnetic stimulation (rPMS), have emerged as non-invasive approaches for pain modulation and functional improvement. This systematic review aimed to evaluate the efficacy and safety of low-intensity (PEMF) and high-intensity (rPMS) magnetic field therapies in adults with

Robot-assisted rehabilitation therapy for adhesive capsulitis: A pilot randomized controlled trial on clinical and patient-reported outcomes.

BACKGROUND: Adhesive capsulitis, commonly known as frozen shoulder, presents with pain and restricted movement. Treatments include physical therapy focusing on stretching exercises, intra-articular corticosteroid injections, and surgical intervention if conservative management fails. The efficacy of robot-assisted rehabilitation therapy (RART) for adhesive capsulitis remains unexplored, necessitating clinical application and analysis. METHODS: A prospective, randomized controlled trial was conducted involving patients aged 20 to 60 years diagnosed with adhesive capsulitis. Participants were r

Clinical Use of Hypertonic Lactate-Current Evidence and Emerging Perspectives.

CONTEXT: Lactate has traditionally been viewed as a metabolic byproduct associated with metabolic acidosis, exercise-induced fatigue, and cancer metabolism. However, accumulating evidence highlights its valuable role as an alternative fuel and a key signaling molecule. Hypertonic sodium lactate infusions have gained interest for their potential therapeutic applications, particularly in neurology and cardiology. This review examines the established and potential clinical uses of exogenous hypertonic lactate treatment. EVIDENCE ACQUISITION: A literature search was conducted in PubMed and Medlin

Evaluation of preoperative cardiopulmonary reserve and surgical risk of patients undergoing lung cancer resection.

Lung cancer represents the second most frequent neoplasm and the leading cause of neoplastic death among both women and men, causing almost 25% of all cancer deaths. Patients undergoing lung resection-both for primary and secondary tumors-require careful preoperative cardiopulmonary functional evaluation to confirm the safety of the planned resection, to assess the maximum tolerable volume of resection or to exclude surgery, thus shifting the therapeutic approach toward less invasive options. Cardiopulmonary reserve, pulmonary lung function and mechanical respiratory function represent the cor
