External fixation of open femoral shaft fractures.

OBJECTIVE: To determine whether external fixation proves to be a sensible technique for definitive stabilization in open femoral fractures. DESIGN: Retrospective clinical study. MATERIALS AND METHODS: From 1985 to 1989, 18 patients (mean ISS 25.4) with open femoral fractures (type II 11%, type III 89%) were treated by primary and definitive external fixation. After failure of closed reduction procedures, open reduction via debrided soft tissue wounds was employed in 72%. Supplemental internal fixation of large wedge fragments was required in 66%. External fixators were removed after a mean o

Factors influencing surgical outcomes of fixation with locking plate for three- and four-part proximal humerus fractures in patients aged 50 Years and older.

BACKGROUND: Treating complex three- and four-part proximal humerus fractures, especially in the elderly, remains contentious, with internal fixation using locking plates and shoulder arthroplasty being primary options. Although proximal humerus locking plates are more commonly used than shoulder replacements, they have a high complication rate. Factors like low bone density, advanced age, multiple fragment fractures, and medial cortical support loss negatively impact treatment outcomes. This study evaluates the functional and radiographic outcomes of using locking plates for treating these fra

[Treatment of extra-articular malunited fractures of the distal radius using osteotomy with interposition of a graft].

Malunited fractures of the distal end of the radius are corrected by an osteotomy transversely oriented at the level of the angulation (15 cases reported). After correction of the deformation (length and angulation) compact iliac bone graft is inserted. 5 malunited fractures with a volar deformation were corrected by a palmar surgical approach and AO-T-plate fixation. 8 cases with a dorsal displacement were corrected by a dorso-lateral surgical approach and fixed with K-wires (7 cases) and without any internal fixation (1 case). 2 cases with dorsal displacement were corrected by a palmar appro

Intertrochanteric valgus osteotomy and sliding compression hip screw in fractures of the femoral neck.

Twenty-four patients ranging from 28 to 56 years of age with Garden grade III or IV fractures of the femoral neck are reviewed after treatment by intertrocherantic valgus osteotomy and Richards sliding compression hip screw. The rationale behind this treatment, the operative technique, and the choice of internal fixation device are all discussed. All patients were followed up after more than 2 years so that any late complications could be documented. The results were evaluated using both the Merle d'Aubigné procedure and radiographic examination: 12 (50%) were rated excellent, 5 (20.8%) good,

Sinus Tarsi Approach With Distractor-Aided Reduction of Displaced Intra-articular Calcaneus Fracture.

This review describes the sinus tarsi approach for operative fixation of intra-articular calcaneus fractures, using a fixed distractor for assistance. The sinus tarsi approach has been shown to decrease wound complication rates and allow for earlier surgical intervention compared with the extensile lateral approach, while still achieving satisfactory anatomical restoration in the operative management of calcaneus fractures. The case presented involves a 46-year-old man who sustained a displaced intra-articular calcaneus fracture after a fall from height. The discussion includes operative indic

Mechanical comparison of different plate-and-screw configurations for repairing forearm diaphysis fractures: a systematic review.

PURPOSE: This is the first systematic review of all studies that adjusted plate and/or screw variables to determine the most mechanically stable implant configuration for forearm fracture repair. METHODS: PubMed and Scopus archives were searched using the phrase “biomechanics” AND “fracture” AND “plate” AND (“forearm” OR “radius” OR “ulna”). Eligibility criteria were applied: (i) studies that mechanically analyzed implant performance by modulating plate and/or screw variables, such as plate geometry, plate material, screw number, etc.; (ii) forearm diaphysis “shaft” fractures of the radius and

Thumb Fractures and Associated Injuries: An Evidence Based Review to Guide Treatment.

Fractures of the thumb encompass a diverse spectrum of injury patterns, each requiring nuanced consideration of various treatment strategies. Associated conditions include injuries to the ulnar collateral ligament and radial collateral ligament of the metacarpophalangeal joint, which can significantly impact thumb stability and function. Although extra-articular fractures of the thumb metacarpal and phalanges are more tolerant of residual deformity due to compensatory movement of the carpometacarpal joint, intra-articular fractures, including Bennett and Rolando fractures, often benefit from s

Optimizing Outcomes in Zygomaticomaxillary Complex Fractures.

The management of patients with a zygomaticomaxillary complex (ZMC) fracture requires assessment of functional and aesthetic impacts, tailored to individual fracture patterns, soft tissue effects, visual changes, and other aspects. Precise reduction is challenging due in part to the three-dimensional tetrapod structure, 4 buttresses, and limited ability to expose ZMC fractures. Tools like virtual surgical planning, navigation, and intraoperative computed tomography may enhance precision and minimize morbidity. We can expect that further developments in navigation, augmented reality, and other

Increased Lateral Calcaneal Body and Subtalar Joint Visualization Utilizing a Modified Sinus Tarsi Approach.

BACKGROUND: The traditional sinus tarsi approach (TST) for open reduction and internal fixation (ORIF) of calcaneus fractures has gained popularity because of adequate fracture exposure with lower wound complication rates. Visualization, as compared with more extensile approaches, can be limited, however. This study introduces a modified sinus tarsi approach (MST) that extends the TST incision approximately 2 cm posterodorsally as well as 2 cm anteroplantarly, to enhance exposure. The aim of this study is to quantify the amount of exposure obtained with the traditional sinus tarsi approach and

Surgical Approaches to the Elbow in Fixation of Traumatic Injuries.

Elbow fractures and fracture-instability injuries are increasingly prevalent. Gaining adequate exposure to the joint surface poses considerable challenges due to the unique articular anatomy and intricacies of the neurovascular structures traversing the elbow. Laterally, the radial head and capitellum are accessed through a classic Kocher or variations of the Kaplan technique. Posteriorly, the distal humerus is exposed by retracting the extensor mechanism through triceps split, reflection, or olecranon osteotomy. The medial approach may expose the coronoid through or around the flexor carpi ul

Collapse of the proximal carpal row into flexion after distal radial fracture: recognition and treatment.

Early palmar carpal instability with proximal row collapse into flexion after a distal radial fracture is rarely reported. Fracture reduction alone may not restore alignment, whereas dorsal capsuloligamentous reinforcement may achieve stability, underscoring the importance of early recognition and combined treatment.Level of evidence: V.

Trans-medial gastrocnemius approach (Badet approach) for displaced posterior cruciate ligament tibial avulsion.

Avulsions of the retrospinal surface are rare injuries resulting from high-energy trauma. Displacement of this fracture frequently indicates a surgical treatment to restore posterior cruciate ligament function. Several approaches have been proposed in the literature, either open or arthroscopic, which can be tricky due to the fracture's proximity to the popliteal vascular-nervous elements. Badet's open approach is a medial trans-gastrocnemius approach, providing a direct access to the retro-spinal surface for osteosynthesis. In this technique, an L-shaped incision is made along precise skin li

The Safety of Volar to Dorsal Percutaneous Screw Fixation of Bennett Fracture-Dislocation - A Cadaveric Study.

Background: Bennett fractures are traditionally fixed with percutaneous K-wires from dorsal to volar, or with a volar to dorsal screw via a volar open approach. While volar to dorsal screw fixation is biomechanically advantageous, an open approach requires extensive soft tissue dissection, thus increasing morbidity. This study aims to investigate the practicality and safety of Bennett fracture fixation using a percutaneous, volar to dorsal screw, particularly with regard to the median nerve and its motor branch during wire and screw insertion. Methods: Fifteen fresh frozen forearm and hand spe

Role of finite element analysis for selection of single point fixation in zygomaticomaxillary complex fracture.

BACKGROUND: One-point fixation was superior to the two and three-points fixation in minimally displaced zygomaticomaxillary complex (ZMC) fracture regarding the cost, invasiveness, scaring, number of wounds, and operation time. Accordingly, this study aimed to predict which one-point fixation is the most stable in managing minimally displaced ZMC fracture. MATERIAL & METHODS: This study simulated the different one-point fixation approaches on three ZMC models after fracture reduction and application of all forces exerted on the fractured area. The findings were represented as stress impact on
