A multidimensional approach to frailty compared with physical phenotype in older Brazilian adults: data from the FIBRA-BR study.

BACKGROUND: Frailty is a predictor of negative health outcomes in older adults. The physical frailty phenotype is an often used form for its operationalization. Some authors have pointed out limitations regarding the unidimensionality of the physical phenotype, introducing other dimensions in the approach to frailty. This study aimed to create a multidimensional model to evaluate frailty in older Brazilian adults and to compare the dimensions of the model created among the categories of the physical frailty phenotype. METHODS: A cross-sectional study was conducted using data from 3569 partici

Influence of Gerontology Services on the Physical and Psychological Health of Elderly in the City of Zagreb.

BACKGROUND: The World Health Organization (WHO) defines the quality of life as an individual perception of personal position in the cultural and social system within an individual's environment. Elderly people, in general, prefer to live in their own home as much as possible, and moving means for them losing social relationships, changing routines and lifestyles and losing independence. The research aimed to gain insight into the use of gerontology services for the elderly people, their availability, diversity of information, a range of services they provide, and impact of these services on qu

Is mini-nutritional assessment a reliable tool in detecting malnutrition in elderly with body weight excess?

PURPOSE: The rising proportion of elderly population in high-income societies has resulted in increasing number of subjects with chronic disabling diseases and nutritional deficiency. Elderly's nutritional status is usually assessed through the mini-nutritional assessment (MNA®). However, its effectiveness may be influenced by weight excess or obesity. We tested the performance of MNA® questionnaire in subjects aged ≥ 65 years from Northern Sardinia, Italy, according to overweight/obesity, and we tried to identify the factors associated with malnutrition. METHODS: A modified version of MNA® (

[2018 ESC/ESH Guidelines for the Management of Arterial Hypertension: What's New with Regard to Our Older People?].

In the "2018 ESC/ESH Guidelines for the management of arterial hypertension" there are new blood pressure (BP) recommendations with regard to older patients based on randomized controlled trials and well-conducted meta-analyses performed during the last years. It is recommended to base the diagnosis of hypertension on repeated office BP measurements under standardized conditions and in older people in out-of-office BP measurement with ABPM and/or HBPM if logistically and economically feasible. Furthermore lying and standing BP measurements should also be considered in subsequent visits in older

[The effect of 24 weeks of moderate-to-high intensity strength training on the elderly].

INTRODUCTION: Strength programs have been seen to be useful in minimizing the effects of sarcopenia, although intervention protocols may vary in their content and characteristics. The aim of this study was to demonstrate the influence of a particular strength protocol for the elderly. MATERIAL ANS METHODS: A total of 35 individuals took part in the study, with 18 in the exercise group (4 men and 14 women), and 17 in the control group (4 men and 13 women). The average age was 73. The exercise group carried out a strength training program at moderate to high intensity over 24 weeks. Strength wa

Biological markers in older people at risk of mobility limitations.

Due to the progressive ageing of the worldwide population, prevention and treatment of late-life dysfunctions, including functional decline and mobility limitations, represent leading targets of scientists and clinicians, but are also receiving growing attention from governments and healthcare systems. The early identification of elderly patients more prone to physical decline represents a crucial step for establishing preventive measures. Although functional capacity can easily be assessed, the use of additional criteria that anticipate the onset of mobility limitations seems much more advant

Pay attention! The critical importance of assessing attention in older adults with dementia.

Attention is an important cognitive domain that is affected in Alzheimer's disease and other dementias. It influences performance in most other cognitive domains, as well as activities of daily living. Nurses are often unaware of the critical importance of assessing attention as part of the overall mental status examination. This article addresses an important gap in nurses' knowledge. The authors present a brief overview of attention as a critical cognitive domain in dementia; review instruments/methods for standardizing and enhancing the assessment of attention; and offer ways to help ensure

Safe and effective pain management in elders.

The elderly are often either untreated or undertreated for pain. The consequences of undertreatment for pain can have a devastating impact on health and quality of life, resulting in depression, anxiety, social isolation, cognitive impairment, immobility, and sleep disturbances. Reasons cited by healthcare professionals for inadequate pain control include lack of training, inappropriate pain assessment, and reluctance to prescribe opioids, however, the undertreatment of pain can be legally considered to constitute neglect, abuse, or negligence. The appropriate treatment of pain is humane and w

Perceptions and implications of violence from care home residents with dementia: a review and commentary.

AIMS AND OBJECTIVES: This paper presents a critical review of the literature related to the violence staff in care homes experience from people with dementia. It attempts to identify the psychological consequences abuse has on care workers and the implications this has for practice. BACKGROUND: Abuse in the form of violence or aggression against healthcare professionals is not new. However, when this is from residents with dementia in care homes it is considered to be part of the job. Consequently it is largely underreported and ignored which masks its true extent. CONCLUSION: The magnitude

[Anemia in the elderly - a diagnostic and therapeutic challenge?].

Anemia in elderly patients should never be regarded as a normal physiological response to aging. The main categories of anemia in older patients are the nutritional anemia attributed to iron deficiency, including blood loss, folate and vitamin B12 deficiency, anemia of chronic disease in patients with cancer, infections and other chronic inflammation. A further category is the unexplained anemia due most probably to impaired corrective mechanisms to stress in older persons. Investigations such as a complete blood count, red cell indices and morphology, reticulocyte count, iron parameters, vita

Ethical challenges in the care of older people and risk of being burned out among male nurses.

AIM: The aim of this study was to illuminate the meaning of being in ethically difficult situations when caring for older people, as experienced by male nurses. Male nurses are a minority group in the nursing profession, thus it is important to listen to their lived experiences of the ethical challenges they are faced with in their work. METHOD: The study is part of a comprehensive investigation into the narratives of female and male physicians and nurses, c
