Low-Dose Donor Dopamine Is Associated With a Decreased Risk of Right Heart Failure in Pediatric Heart Transplant Recipients.

BACKGROUND: Previous studies in adults have suggested that donor dopamine treatment may improve recipient outcomes in organ transplantation; in this analysis, we aimed to determine if donor dopamine reduces the incidence of postoperative right heart failure (RHF) in pediatric heart transplant recipients. METHODS: Data for recipients aged 18 years or younger transplanted at our institution between January 1, 2000, and June 15, 2011, and their respective donors were obtained. The presence of postoperative RHF was assessed for in all subjects. Donor dopamine dose was stratified into 3 groups: no…

Characteristics and outcomes of patients with heart failure and discordant findings by right-sided heart catheterization and cardiopulmonary exercise testing.

There are limited data integrating findings on right-sided cardiac catheterization and cardiopulmonary exercise testing in ambulatory patients with heart failure. In this study, 187 outpatients with HF referred to the Duke Medical Center for consideration of advanced HF therapies were retrospectively evaluated. All patients had undergone right-sided cardiac catheterization and cardiopulmonary exercise testing; the median cardiac index (CI) was 2.0 L/min/m2 (interquartile range 1.7 to 2.3), and the median peak oxygen consumption was 11.3 ml/kg/min (interquartile range 9.2 to 13.8). Despite aggr…

Usefulness of the Seattle Heart Failure Model to identify adults with congenital heart disease at high risk of poor outcome.

Our objective was to determine whether the Seattle Heart Failure Model (SHFM) differentiates patients with adult congenital heart disease (ACHD) at high versus low risk for cardiovascular outcomes and poor exercise capacity. The ACHD population is growing and presents increasingly for care in the community and at tertiary centers. Few strategies exist to identify the patients with ACHD at high risk for heart failure and mortality. We studied 153 adults with transposition of the great arteries, Ebstein anomaly, tetralogy of Fallot, double outlet right ventricle, and single ventricle from 2 ACHD …

Medication adherence, self-care behaviour and knowledge on heart failure in urban South Africa: the Heart of Soweto study.

BACKGROUND: There is a paucity of data on treatment adherence in patients with chronic heart failure (CHF) in Africa. METHODS: We examined the pattern of treatment adherence, self-care behaviour and treatment knowledge in 200 consecutive patients with CHF attending the Chris Hani Baragwanath Hospital, Soweto, South Africa via a combination of questionnaire (100%, n = 200) and pill count (41%, n = 82). RESULTS: Mean age was 56 +/- 14 years, 157 were black African (79%) and 109 (55%) were male. CHF-specific treatment included loop diuretics (93%), beta-blockers (84%), ACE inhibitors (74%), spi…

Overlap and distinctiveness of psychological risk factors in patients with ischemic heart disease and chronic heart failure: are we there yet?

OBJECTIVE: Growing evidence supports the importance of psychological factors in the etiology and progression of cardiovascular disease (CVD). However, this research has been criticized due to overlap between psychological constructs. We examined whether psychological questionnaires frequently used in cardiovascular research assess distinct constructs in a mixed group of ischemic heart disease (IHD) and chronic heart failure (CHF) patients. METHODS: 565 patients with CHF (n=118) or IHD (n=447) completed the Type D scale (DS14), Hospital Anxiety and Depression Scale (HADS), Beck Depression Inve…

Stratification of pediatric heart failure on the basis of neurohormonal and cardiac autonomic nervous activities in patients with congenital heart disease.

BACKGROUND: Stratification of pediatric patients with congenital heart disease (CHD) has been based on their hemodynamics and/or functional … tients (P<0.05 to 0.001). CONCLUSIONS: CANA and NHA indices are useful to stratify mild and severe heart failure in stable postoperative CHD patients, respectively. However, careful attention should be paid to age- and surgery-related influences on these indices.

Neonatal lupus erythematosus with congenital heart block and severe heart failure due to myocarditis and endocardititis of the mitral valve.

We report a case of neonatal lupus erythematosus (NLE) with congenital heart block and severe myocardial failure, which was followed from the 25th week of gestation because of fetal bradycardia. The child was delivered at the 37th week of gestation by elective cesarean section because of echocardiographically documented heart enlargement, pericardial effusion and moderate insufficiency of the mitral and tricuspid valves. In spite of immediate pacing, intubation and supportive treatment, the newborn developed progressive heart failure. Echocardiography showed endocarditis of the mitral valve an…

[The role of the renin-angiotensin-aldosterone system and vasopressin in the development of heart failure during continuous electric stimulation of the heart].

The status of the renin-angiotensin-aldosterone system and the time course of vasopressin levels were studied in patients with an implanted cardiostimulator. Activation of plasma renin due to renal hypoperfusion in an inadequate stimulation was demonstrated to be followed by a rise in aldosterone and vasopressin concentrations and by an increase in blood volumes. Activation of angiotensin II and vasopressin resulted in higher peripheral resistance. Altered humoral regulation of the circulation retained 6-12 months after cardiostimulator implantation, by maintaining myocardial overload with vol

Patient phenotype profiling in heart failure with preserved ejection fraction to guide therapeutic decision making. A scientific statement of the Heart Failure Association, the European Heart Rhythm Association of the European Society of Cardiology, and the European Society of Hypertension.

Heart failure with preserved ejection fraction (HFpEF) represents a highly heterogeneous clinical syndrome affected in its development and progression by many comorbidities. The left ventricular diastolic dysfunction may be a manifestation of various combinations of cardiovascular, metabolic, pulmonary, renal, and geriatric conditions. Thus, in addition to treatment with sodium-glucose cotransporter 2 inhibitors in all patients, the most effective method of improving clinical outcomes may be therapy tailored to each patient's clinical profile. To better outline a phenotype-based approach for t…

Multiorgan failure and in-hospital mortality in urgent heart transplantation: the PRESAGIO study.

INTRODUCTION AND OBJECTIVES: Multiorgan failure is a contraindication to heart transplantation (HT). In Spain, standardized criteria have been established for its definition. The aim of this study was to analyze the association between these criteria, assessed at urgent waiting list inclusion and at the time of transplantation, and in-hospital mortality (IHM). METHODS: Ambispective multicenter study of consecutive urgent HT procedures performed between January 2020 and May 2024. We excluded patients aged <17 years, those with multiorgan transplants, and retransplant recipients (n=328). Data f

Clinical Utility of HeartInsight Score Variability in the Management of Patients With Heart Failure.

BACKGROUND: Patients with heart failure (HF) can be monitored remotely using cardiovascular implantable electronic devices (CIEDs). In Japan, the HeartInsight algorithm has been implemented in selected implantable cardioverter defibrillators and cardiac resynchronization therapy devices. This retrospective study investigated whether elevations above, and daily fluctuations in, the HeartInsight score threshold aid prediction of hospitalization for HF. METHODS: This study included 61 patients. Data were extracted from the BIOTRONIK Home Monitoring system; the number of days of HeartInsight scor…

ADAM and ADAMTS proteases as integrative hubs in heart failure pathogenesis and therapy.

Heart failure (HF) arises from multifactorial disruptions in cellular signaling, inflammation, and tissue remodeling beyond classical neurohormonal regulation. Increasing evidence highlights ADAM and ADAMTS metalloproteinases as central integrators of inflammatory, fibrotic, and hypertrophic pathways. This review establishes a dual integrative framework. Horizontally, distinct protease family members converge within each pathological axis, including inflammation, fibrosis, hypertrophy and remodeling, metabolism, and angiogenesis, to drive disease progression. Vertically, individual proteases, …

Baroreflex Activation Therapy in Patients with Heart Failure with a Reduced Ejection Fraction.

A randomized, controlled trial of baroreflex activation therapy (BAT) in patients with heart failure and reduced ejection fraction demonstrated that BAT was safe and significantly improved patient-centered symptomatic outcomes, increasing exercise capacity, improving quality of life, decreasing n-terminal pro B-type natriuretic peptide (NT-proBNP), and improving functional class. BAT was approved by the FDA for improvement of symptoms of heart failure for patients who remain symptomatic despite treatment with guideline-directed management, are New York Heart Association Class III or Class II (…

Clinical Impact of Semaglutide in Patients with Heart Failure and Preserved Ejection Fraction.

Background: The clinical impact of oral semaglutide on cardiac biomarkers in real-world patients with heart failure with preserved ejection fraction (HFpEF) and type 2 diabetes mellitus (T2DM) remains unclear. We evaluated whether initiation of oral semaglutide was associated with a reduction in B-type natriuretic peptide (BNP) levels and explored factors associated with this response. Methods: We retrospectively enrolled 27 patients with HFpEF who initiated oral semaglutide for T2DM management at a single academic center. Clinical data were collected at three time points: three months before …
