Disrupting Helicobacter pylori Iron Homeostasis With Bismuth Nanodrug-Mediated Nutritional Trap for Targeted Gastric Infection Therapy.

Helicobacter pylori (H. pylori) is a leading cause of gastric cancer, but current antibiotic-based therapies face challenges like low eradication rates, high recurrence, and intestinal flora disruption. Iron is critical for H. pylori's survival and pathogenicity, as the bacterium relies on maintaining iron homeostasis for normal proliferation, metabolism, and colonization of the stomach. Inspired by this, we propose a "nutritional trap" strategy, using bismuth as "fake iron" to trick the H. pylori's iron-uptake system, triggering iron starvation. We then developed a bismuth-based nanodrug (Bi-…

Distribution and Clinical Impact of Helicobacter pylori Virulence Factors in Epstein-Barr-Virus-Associated Gastric Cancer.

Background: Helicobacter pylori (HP) and Epstein-Barr virus (EBV) coinfection lead to chronic inflammation and contribute to the development of gastric cancer. However, studies examining the association between HP virulence factors and EBV infection in gastric cancer are limited. This study investigated the polymorphisms of HP virulence factors associated with EBV infection and their effects on clinical outcomes in EBV-associated gastric cancer (EBVaGC). Methods: A total of 96 HP isolates from 54 patients with gastric cancer were divided and analyzed based on EBV coinfection status. Polymerase…

Multiplex PCR-Mass Spectrometry Mini-Sequencing Technology Detected Antibiotic Resistance of Helicobacter pylori to Six Antibiotics.

The abuse of antibiotics has led to widespread resistance to Helicobacter pylori (H. pylori) in the population. There is an urgent need to establish a method to detect multiple antibiotic resistance rapidly. This study aimed to construct a novel strategy for the high-throughput detection of H. pylori's resistance to varying antibiotics using multiplex PCR-mass spectrometry mini-sequencing (mPCR-MS mini-sequencing) technology. This study detected the resistance of H. pylori to six antibiotics using eight mutated sites (23S rRNA-2143; pbp1A-1667, 1684, 1240; gyrA-261, 271, 573; and 16S rRNA-928)…

Natural small-molecule compounds targeting Helicobacter pylori virulence factors: A promising strategy for overcoming antibiotic resistance.

Helicobacter pylori (H. pylori) infection is an important causal factor of gastritis, peptic ulcer, and gastric cancer. High infection rates and the increasing challenge of antibiotic resistance worldwide have prompted an urgent need to develop novel therapeutic options and antimicrobial agents. This review focuses on the potential of natural small-molecule compounds as novel anti-H. pylori agents-a promising approach that mitigates the risk of resistance development and maintains the microbiome's ecological balance. We detail how H. pylori virulence factors, including urease, CagA, VacA, and …

Unraveling the Role of METTL3 in Helicobacter pylori-induced gastritis via m6A-CXCL1/NF-κB modulation.

Helicobacter pylori (H. pylori) infection is a significant cause of gastric diseases, with its pathogenic mechanisms still not fully understood. This study investigates the role of METTL3, an enzyme involved in m6A methylation, in modulating the CXCL1/NF-κB signaling pathway in H. pylori-induced gastritis. Using both bioinformatics analysis of GEO database and experimental approaches including MeRIP, RIP assays, and immunostaining, this research highlights how METTL3 influences CXCL1 expression and NF-κB pathway activation. Results from both in vitro and in vivo models show that METTL3 increas…

Immunogenicity evaluation of semisynthetic α-(1 → 3)-D,D-heptoglycan conjugates for Helicobacter pylori vaccine development.

Helicobacter pylori is a Gram-negative bacterium that infects nearly half the global population and is associated with a range of gastric diseases, including gastric cancer. The lipopolysaccharides on the surface of H. pylori are immunogenic and have been regarded as attractive targets for carbohydrate-based vaccines. In this study, two semisynthetic glycoconjugates, CRM197-1 and CRM197-4, were prepared by conjugating a α-(1  →  3)-D,D-hepto-disaccharide and a α-(1  →  3)-D,D-hepto-pentasaccharide, respectively, with carrier protein CRM197. Compared with CRM197-1, CRM197-4 elicited a more robu…

Single-dose oral administration of drug-loaded magnetic 3D-printed microbullets for eradication of Helicobacter pylori.

Infections of Helicobacter pylori (H. pylori) affect 42.1 % of the Chinese population and 43.1 % of the world population. H. pylori inhabits the mucous sublayer at the pylorus, leading to gastric ulcers, gastritis, and even cancer. Oral antibiotics are usually used to treat H. pylori infections, whereas traditional quadruple therapy has side effects including headaches, nausea, diarrhea, intestinal dysbacteriosis, antibiotic resistance, and repeat infections. Here, a drug-loaded magnetic microbullet was designed to realize long-term retention in the stomach for one-shot treatment for H. pylori…

Non-improvement of atrophic gastritis in cases of gastric cancer after successful Helicobacter pylori eradication therapy.

BACKGROUND: Helicobacter pylori (H. pylori) infection is closely related to the development of gastric cancer (GC). However, GC can develop even after H. pylori eradication. Therefore, it would be extremely useful if GC could be predicted after eradication. The Kyoto classification score for gastritis (GA) is closely related to cancer risk. However, how the score for GC changes after eradication before onset is not well understood. AIM: To investigate the characteristics of the progression of Kyoto classification scores for GC after H. pylori eradication. METHODS: Eradication of H. pylori wa…
