Chronic hepatitis B in pregnancy: unique challenges and opportunities.

Chronic hepatitis B (CHB) affects 350 million individuals worldwide. Perinatal transmission leads to high rates of chronic infection and complications, including cirrhosis and hepatocellular carcinoma. It is important to recognize and appropriately treat CHB in pregnancy, thereby reducing the risk of neonatal transmission and HBV-associated morbidity and mortality. Screening for CHB is recommended in all pregnant mothers as is universal vaccination of infants with hepatitis B virus (HBV) vaccine with or without hepatitis B immunoglobulin (HBIG). This has resulted in a lower incidence of HBsAg

Neonatal morbidity and mortality in Calabar, Nigeria: a hospital-based study.

BACKGROUND: The morbidity and mortality pattern amongst neonates admitted into the University of Calabar Teaching Hospital were reviewed from 1st June 2003 to 30th November 2004. RESULTS: The major indications for admission for inborn babies were infections (27.4%), jaundice (21%) and low birth weight (LBW) (18.4%). The out born babies were admitted largely for sepsis (26.8%), jaundice (17.7%), tetanus (13.9%) and low birth weight (11.2%). Staphylococcus aureus (61.2%) and unclassified coliforms (21.9%) were the dominant isolates of septicaemia. The overall mortality rate of 19.3% was largely

Survival after balloon atrial septostomy for complete transposition of great arteries.

Mortality before surgery must be taken into account when comparing the surgical mortality of atrial redirection procedures (Mustard's or Senning's operation) and the arterial switch operation for patients with complete transposition. This is because the switch operation is usually performed within the neonatal period or early infancy but Mustard's or Senning's operation usually after 4 months of age. The outcome of balloon atrial septostomy was therefore assessed in all 102 infants with transposition of the great arteries (plus or minus associated anomalies) who underwent the procedure at our

Senning repair for transposition of the great arteries in the first week of life.

Infants with D-loop transposition of the great arteries (D-TGA) may have unacceptable results after the balloon septostomy while awaiting surgery. It has been our policy to repair defects in infants with D-TGA and intact ventricular septum or small ventricular septal defect at the time of diagnosis. We report our experience with the Senning operation in 18 newborns less than 1 week of age. The mean age at operation was 3 days (range 12 hr to 7 days) and the mean weight was 3.5 kg (range 2.8 to 4.8). There were two early postoperative deaths (11%) and one late death (5%). Early mortality was as

Early congenital syphilis: clinico-radiologic features in 202 patients.

Venereal syphilis is highly prevalent among women of child-bearing age in Zambia. It is estimated to contribute 25-30% of the perinatal mortality rate of 50 per 1,000 births at the University Teaching Hospital in Lusaka. Because of multisystem involvement, early congenital syphilis, the offshoot of maternal syphilis, has varied criteria for its diagnosis. Therefore, in an attempt to draw guidelines for an easier and more reliable diagnosis, the authors analyzed the clinico-radiologic features of congenital syphilis in 202 patients. Although all infants were under the age of six months, there w

Coarctation of the aorta in infants under 1 year of age.

Untreated coarctation of the aorta in infants is associated with a high mortality, reaching up to 90%. During a period of 7 years 26 infants underwent surgical repair of this condition, the youngest being 6 days old and the oldest 11 months. Two underwent emergency surgery after catheterization, while the rest had an elective or semi-elective operation. Associated anomalies were present in 85% of the whole group; of the 22 patients with anomalies, a ventricular septal defect (VSD) and patent ductus arteriosus (PDA) were present in 11 patients, a PDA in 7, a VSD in 6, transposition of the great

Pregnancy outcome in 211 patients with mild chronic hypertension.

The purpose of the present clinical investigation was to determine the risks associated with mild chronic hypertension in pregnancy. Two hundred eleven consecutive pregnancies complicated by mild chronic hypertension (diastolic blood pressure, 90 to 110 mmHg) were analyzed. All patients were followed closely throughout pregnancy with frequent prenatal visits and serial assessment of fetal status. Antihypertensive drugs were discontinued at the time of the first prenatal visit. Only 13% of these patients required antihypertensive medications later in pregnancy. There were 2 stillbirths and 4 ne

Coarctation of the thoracic aorta: an 18-year experience.

From 1960 to 1978, 80 patients from 2 weeks to 49 years of age underwent operations for coarctation of the aorta. Twelve patients were under 3 months old, and 68 were older. All of the infants presented with congestive heart failure and multiple cardiac defects. In the older patients, hypertension was the most common presenting symptom; 14 were asymptomatic. All patients under 3 months old received primary correction. Seven (58%) died of complications associated with other cardiac anomalies. In the older group, there was 59 primary reconstructions, six interposition grafts, and three other pro

Nasal Continuous Positive Airway Pressure vs Nasal Intermittent Positive Pressure Ventilation in Preterm Infants With Respiratory Distress Syndrome: A Randomized Clinical Trial.

IMPORTANCE: Respiratory distress syndrome (RDS) remains a leading cause of morbidity and mortality in preterm infants. Evidence regarding the optimal initial noninvasive ventilation (NIV) mode for extremely preterm infants (<30 weeks' gestation) with RDS is inconsistent. OBJECTIVE: To determine whether nasal continuous positive airway pressure (NCPAP) is noninferior to nasal intermittent positive pressure ventilation (NIPPV) as primary respiratory support before minimally invasive surfactant administration (MISA) for reducing intubation within 72 hours in preterm infants with RDS. Ring chromosome 6: A case report and literature review.

Ring chromosome 6 is a rare structural chromosomal anomaly with a variable phenotype that includes growth restriction, congenital malformations, and neurological impairment. We present a term newborn female with a prenatal history of fetal growth restriction (FGR) and central nervous system malformations. At birth, she exhibited dysmorphic facies, bilateral corneal opacity, hypotonia, and a congenital heart defect. Cytogenetic analysis revealed a karyotype of 46,XX,r(6)(p25q27)[27]. The infant died on day 18 days of life due to a respiratory complication. Conclusions: Ring chromosome 6 is asso

Diagnosis by metagenomic next-generation sequencing of invasive pulmonary aspergillosis in an infant with chronic granulomatous disease.

Invasive pulmonary aspergillosis (IPA) is a serious fungal infection, with a high degree of mortality in immunocompromised individuals. Diagnosis of IPA is challenging in that clinical manifestations are not specific, with sensitivity of traditional detection procedures low. We report a case of IPA in a chronic granulomatous disease (CGD) infant who was initially suspected to have a lung tumor. Aspergillus fumigatus was identified as the pathogen in bronchoalveolar lavage fluid (BALF) by next-generation sequencing (mNGS). The patient recovered rapidly following a change of appropriate antifung

A Case of Pulmonary Artery Sling Anomaly with Tracheal Stenosis and Management of Difficult Airway.

Pulmonary artery sling is a rare congenital vascular abnormality, where the left pulmonary artery originates from the right pulmonary artery, passes between trachea, and esophagus and reaching the left hilum. Cough, wheezing, and difficulty in feeding are three major symptoms. Untreated pulmonary sling carries high morbidity and mortality, most of which is due to the airway and other associated anomalies. Herein, we reported a 40-day-old male infant who admitted to the pediatric intensive care unit with progressive respiratory distress and diagnosed with left pulmonary sling with tracheal sten

Renal-Hepatic-Pancreatic Dysplasia: An Ultra-Rare Ciliopathy with a Novel NPHP3 Genotype.

Renal-hepatic-pancreatic dysplasia-1 (RHPD1) is an ultra-rare genetic disorder with a high mortality. It is caused by biallelic pathogenic variants in NPHP3 , which encode nephrocytin, an important component of the ciliary protein complex. The NPHP3 -related disease phenotype is diverse with RHPD1, nephronophthisis-3, and Meckel syndrome-7. In this case report, we present a female infant with hepatomegaly, cholestasis, and elevated transaminases who was found to carry a homozygous c.2975C > T variant of NPHP3. This is the first description of this genotype and RHPD1 phenotype in the literature

Advances in therapeutic development for spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a leading genetic cause of infant mortality. The disease originates from low levels of SMN protein due to deletion and/or mutations of SMN1 coupled with the inability of SMN2 to compensate for the loss of SMN1. While SMN1 and SMN2 are nearly identical, SMN2 predominantly generates a truncated protein (SMNΔ7) due to skipping of exon 7, the last coding exon. Several avenues for SMA therapy are being explored, including means to enhance SMN2 transcription, correct SMN2 exon 7 splicing, stabilize SMN/SMNΔ7 protein, manipulate SMN-regulated pathways and SMN1 gene de…

[Anthropological analysis of interactions between health care providers and children in some paediatrics services in Western Africa (Abidjan, Bamako, Conakry)].

Africa represents a major part of infant global mortality. But if the medical causes of these deaths are well-known, the quality of the medical care provided to the children and the way it affects them are not really studied. Our study conducted in three countries of western Africa describes how much the care given to children involves a part of violence and emphasizes how the social status and the children educational popular modes influence the quality of care. The article underlines how the improvement of the quality of care in pediatrics services cannot be limited to a simple improvement o

Infants of diabetic mothers.

Advances in the management of the mother with diabetes have reduced the rate of morbidity and mortality for her infant. Aggressive control of maternal glycemic status is warranted, because most morbidities are epidemiologically and pathophysiologically closely linked to fetal hyperglycemia and hyperinsulinemia. The burgeoning public health problem of overweight and obesity in children will likely result in an increased incidence of metabolic syndrome X, characterized by insulin resistance and type II diabetes in adulthood. An early manifestation of this may be glucose intolerance during pregna

[Advancements of the studies on the relationships between air temperature and solar activity with diseases].

Varieties of air temperature and solar activity have a close correlation with human health. The megatemperature mainly results in the increased incidence of cerebrovascular disorder (CVD). It is related to the regulatory mechanism of homeothermism. In the hypothermy environment, obvious increment of human's blood viscosity was observed, which may induce the cerebral infarction, myocardial infarct or coronary heart disease. Not only have solar activities a close relationship with the incidences of plague and epidemic cerebrospinal meningitis, but also high correlation with onco-genesis and infa
