Chronic pancreatitis with a pseudocyst may cause acute massive intestinal bleeding due to simultaneous erosion of the splenic artery and transverse colon. This is a potentially lethal complication, which requires instant diagnosis and treatment. The pathogenesis, diagnosis and treatment of this condition are illustrated by two patients. Exact localisation of the source of bleeding is achieved by selective angiography of the visceral arteries. In shocked and inoperable patients, selective embolisation of the bleeding artery is the treatment of choice.

Aiming at depicting the advantages of chick embryo as a model of fetal surgery we report on our experience of 402 operations carried out in an attempt to induce fetal intestinal obstruction. We have used fertile domestic hen (gallus gallus) eggs incubated at 37.5 degrees with 80% humidity and turned every hour. At the 12th day of incubation we coagulated, after exposing the umbilical cord, a small bowel loop in the physiologic umbilical hernia through a 1 cm-wide opening in the shell which was then sealed with plastic sterile adhesive. The article presents original materials characterizing morbidity in acute enteric infections, such as typhoid fever and other Salmonella infections, dysentery, in the USSR. Morbidity registered in recent years is shown to be the consequence of the unsatisfactory solution of problems connected with supplying the population with epidemiologically safe water and foodstuffs. The decisive role played by sanitary and hygienic measures in the prevention and control of acute enteric infections is emphasized.

Intestinal obstruction in ovarian cancer patients is a major complication which frequently affects survival and quality of life. After a reasonable trial of conservative management fails, surgery is the only hope for relief of obstruction. In an effort to evaluate the success of such surgery we have reviewed the outcome of 54 operations (52 patients) for relief of intestinal obstruction performed over the 3-year period 1983-1985. A previously undescribed type of congenital abdominal wall hernia is described. Presentation was with the symptoms and signs of subacute large bowel obstruction. The diagnosis was suggested by barium enema and confirmed at laparotomy.

The binding of [3H]aldosterone in the chick intestine cytosol was analyzed in terms of affinity and specificity. The recognition of potentially noxious physiologic substances in the intestinal milieu prompted the use of an "elemental" semihydrolyzed formula diet in the prophylaxis of experimental acute ischemic enteropathy. Elemental diets have been used in the management of a variety of digestive diseases. An elemental diet protects the intestinal mucosa of rodents from radiation injury and facilitates mucosal healing. This study examined jejunal sugar absorption in piebald mice with congenital megacolon and in normal littermates. Transmural potential difference, short-circuit current, and tissue conductance of flat sheets of jejunum set up in flux chambers were significantly greater in the diseased mice compared to normal siblings. A case of intestinal pseudo-obstruction in a 4 1/2 year-old boy is reported. All etiologic investigations remained negative. Management successively required continuous enteral feeding, ileostomy then total parental nutrition. The proof of a chronic barbiturate intoxication, induced by the mother, was made only after 2 1/2 years of follow-up, when the patient was in a critical condition. Separation of the child from his family led to complete disappearance of symptoms within a few days.

Since spring 1985 81 persons were examined because of suspected intestinal drug-smuggling (body-packing). 46 patients (57%) had drug-packages in the gastro-intestinal tract, in 35 persons (43%) the suspicion was not confirmed. 26 patients had swallowed the drugs (mostly cocaine) whereas in 20 cases drugs were found in the rectum (mostly heroine). Most important for diagnosis was abdominal x-ray whereas in our hands abdominal sonography was unreliable.
