Systemic treatment of children and adolescents with atopic dermatitis aged ≥2 years: a Delphi consensus project mapping expert opinion in Northern Europe.

BACKGROUND: Paediatric atopic dermatitis (AD) can be burdensome, affecting mental health and impairing quality of life for children and caregivers. Comprehensive guidelines exist for managing paediatric AD, but practical guidance on using systemic therapy is limited, particularly for new therapies including biologics and Janus kinase (JAK) inhibitors, recently approved for various ages in this indication. OBJECTIVES: This expert consensus aimed to provide practical recommendations within this advancing field to enhance clinical decision-making on the use of these and other systemics for child

Interventions to Improve Medication Adherence: A Review.

IMPORTANCE: Among adults with chronic illness, 30% to 50% of medications are not taken as prescribed. In the United States, it is estimated that medication nonadherence is associated with 125 000 deaths, 10% of hospitalizations, and $100 billion in health care services annually. OBSERVATIONS: PubMed was searched from January 1, 2000, to September 6, 2018, for English-language randomized clinical trials of interventions to improve medication adherence. Trials of patients younger than 18 years, trials that used self-report as the primary adherence outcome, and trials with follow-up periods less

CircRNAs as New Therapeutic Entities and Tools for Target Identification in Acute Myeloid Leukemia.

Giant cell tumor (GCT) of bone is one type of giant cell-rich lesion of bone. This benign mesenchymal tumor has characteristic multinuclear giant cells. Mononuclear stromal cells are the physiologically active and diagnostic cell type. Most GCTs are located in the epiphyseal regions of long bones. The axial skeleton-primarily the sacrum-is a secondary site of involvement. Most patients present with pain, swelling, joint effusion, and disability in the third and fourth decades of life. Imaging studies are important for tumor staging and radiographic grading. Typically, these clinically active b

Gastroesophageal Reflux Disease After Roux-en-Y Gastric Bypass: Pathophysiology and Management.

Introduction: Patients with obesity are often affected by gastroesophageal reflux disease (GERD). Roux-en-Y gastric bypass (RYGB) is considered the ideal operation for patients with severe obesity and GERD. Although the majority of patients improve their reflux with the operation, some might persist symptomatic and others can even develop de novo GERD. The aim of this study was to determine pathophysiologic factors involved in the development of GERD after RYGB surgery and define potential treatments for this condition. Materials and Methods: Studies including patients with GERD before and aft

Advanced heart failure: a contemporary approach.

Advanced heart failure (HF) is defined as the persistence of severe symptoms despite the use of optimized medical, surgical, and device therapies. These patients require timely advanced treatments, such as heart transplantation or long-term mechanical circulatory support (MCS). Inotropic agents are often used to reduce congestion and increase cardiac output, while renal replacement therapy may be beneficial if necessary. Cardiac resynchronization therapy has clear benefits in patients with HF with reduced ejection fraction, particularly with left bundle branch block (QRS duration > 130 ms). Th

Acute myocardial infarction in a patient with HAM/TSP evaluated for circulatory dynamics during mobilization before and after antihypertensive drug modification: a case report.

BACKGROUND: Antihypertensive therapy plays a crucial role in managing cardiovascular disease but may sometimes cause significant hypotension during mobilization, hindering the recovery of activities of daily living. In patients struggling with orthostatic hypotension, evaluating circulatory dynamics using a tilt bed provides rich clinical insights for identifying the underlying pathophysiology. We encountered a rare case in which the patient also had HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), prompting an assessment of circulatory dynamics in the passive upright postu

2026 Taiwan guidelines for the acute pharmacologic treatment of migraine in adults.

Since the publication of the Taiwan Headache Society's 2022 Guidelines for the Acute Treatment of Migraine Attacks, advances in headache medicine and the approval of new drugs in Taiwan have necessitated an update. To address clinical needs, the Guideline Committee of the Taiwan Headache Society reviewed evidence for pharmacological and non-pharmacological acute treatments using evidence-based principles. Drawing from the latest European, American, and international guidelines, and through expert consensus, this revision updates treatment roles, grades of recommendation, efficacy, and adverse-

Comparative effectiveness of epilepsy surgery versus additional anti-seizure medications for Lennox-Gastaut syndrome: study protocol for a multicenter, mixed-methods study.

INTRODUCTION: Lennox-Gastaut Syndrome (LGS) is a severe developmental epileptic encephalopathy without a known cure. Management of symptoms requires substantial care. Treatment options include anti-seizure medications, dietary therapy, and epilepsy surgery. Two main treatment pathways for patients with LGS with drug resistant epilepsy are additional anti-seizure medications or epilepsy surgery, which have been reported to be effective in reduction of seizure burden and improving quality of life. No studies have directly compared the outcomes of using epilepsy surgery versus using additional an

Concomitant medication use and clinical outcome in hepatocellular carcinoma treated with immune-based therapy: a multicenter analysis.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have been increasingly used in hepatocellular carcinoma (HCC). Despite emerging evidence indicating that concomitant medications might impact clinical outcomes, their association with ICI efficacy is undefined in HCC. METHODS: The multicenter cohort included 851 HCC patients receiving ICIs between January 2018 and December 2022 in 13 institutions. Concomitant medications given within 30 days before or after the initiation of ICIs were evaluated in association with survival, tumor response, and treatment-related adverse event (TRAE) occurrence.

Factors associated with adherence to antiretroviral therapy among HIV-positive adolescents and young adult patients attending HIV care and treatment clinic at Bombo Hospital in Tanga region-Tanzania.

BACKGROUND: Adherence to HIV treatment regimens involves the consistent and correct intake of all prescribed medications. The implementation of antiretroviral therapy (ART) program has significantly reduced mortality among adolescents living with HIV. However, adherence to ART is lower among adolescents compared to other sub-populations and even lower in sub-Saharan Africa. The factors influencing ART adherence are context-specific and vary across countries and regions. In the Tanzanian context, there is a paucity of data regarding these factors. METHODOLOGY: This cross-sectional study involv

The Effect of a Group-Based Mindfulness and Acceptance Training on Psychological Flexibility and Adherence to Antiretroviral Therapy Among Adolescents in Uganda: An Open-Label Randomized Trial.

Adherence to antiretroviral therapy (ART) is lower in adolescents with HIV (AWH) than in any other age group, partly due to self-regulatory challenges during development. Mindfulness and acceptance training have been shown to support psychological flexibility, a self-regulatory skill that potentially improves adolescent adherence to medication. We assessed the effect of weekly group-based mindfulness and acceptance training sessions on ART adherence among older adolescents (15-19 years) in Kampala, Uganda. One hundred and twenty-two AWH (median age 17, range 15-19 years, 57% female) receiving

The secondary prevention of coronary heart disease in US adults 75 Years and older in daily practice: Results from the National Health and Nutrition Examination Survey 1999-2018 survey.

BACKGROUND: Pharmacologic therapies, risk factor control, and lifestyle alterations were independently proven to reduce long-term cardiovascular events. However, comprehensive research examining the extent to which individuals aged 75 and above in the United States adhere to national guidelines for the secondary prevention of coronary heart disease is limited. Therefore, the primary objective of this study was to examine the current state of secondary prevention of coronary heart disease in persons 75 years of age and older in the United States and to examine the factors that contribute to ina

Paediatric sepsis-associated encephalopathy (SAE): a comprehensive review.

Sepsis-associated encephalopathy (SAE) is one of the most common types of organ dysfunction without overt central nervous system (CNS) infection. It is associated with higher mortality, low quality of life, and long-term neurological sequelae, its mortality in patients diagnosed with sepsis, progressing to SAE, is 9% to 76%. The pathophysiology of SAE is still unknown, but its mechanisms are well elaborated, including oxidative stress, increased cytokines and proinflammatory factors levels, disturbances in the cerebral circulation, changes in blood-brain barrier permeability, injury to the bra
