Impact of cognitive training on claims-based diagnosed dementia over 20 years: evidence from the ACTIVE study.

INTRODUCTION: The very long-term effect of cognitive training on the risk of Alzheimer's disease and related dementias (ADRD) is unknown. METHODS: This study links data from the Advanced Cognitive Training for Independent and Vital Elderly (ACTIVE) study (a four-arm randomized controlled trial of cognitive training in a large, diverse sample) to Medicare claims (1999 to 2019). Inclusion in the analyses required being enrolled in traditional Medicare at baseline (n = 2021). ADRD was measured with the Chronic Conditions Warehouse algorithm. RESULTS: Participants randomized to the speed-trainin

Lower limb motion intention recognition using multi-source able-bodied gait signals.

Objective. Accurate motion intention recognition is challenging due to the limited use of distributed muscle signals. In this study, we proposed a framework that integrates musculoskeletal modelling, advanced feature processing, and feature selection to enhance recognition performance.Approach. A muscle screening method was developed to identify the most informative muscles for joint motion representation. Nonlinear features were extracted from surface electromyography signals using variational mode decomposition and fused with linear features derived from joint angles and moments. To reduce f

The immunoregulatory effect of short-chain fatty acids in type 2 diabetes mellitus.

Short-chain fatty acids (SCFAs) are the primary metabolites of dietary fiber fermented by intestinal flora. They play a systemic role in the immune regulation of type 2 diabetes mellitus (T2DM) by integrating receptor-mediated signaling and epigenetic regulatory mechanisms. At the receptor pathway level, SCFAs activate G protein-coupled receptors such as GPR41/43/109 A, initiate downstream signaling cascades including MAPK, NF-κB, and mTOR/STAT3, and thereby achieve rapid modulation of immune cell function; at the epigenetic regulatory level, SCFAs induce chromatin remodeling and gene expressi

Sulforaphane improved cognitive behavior in APP/PS1 mice via promoting structural and functional synaptic plasticity in the hippocampus.

Cognitive dysfunction is the main clinical feature of Alzheimer's disease (AD), and oxidative stress is considered a critical contributor to AD pathogenesis. Sulforaphane (SFN), an aliphatic isothiocyanate predominantly derived from cruciferous vegetables, has been reported to exert antioxidant and neuroprotective effects; however, its impact on synaptic plasticity and the underlying electrophysiological mechanisms in AD remain unclear. In this study, ten-month-old male APPswe/PS1dE9 double transgenic (APP/PS1) mice and wild-type (WT) littermates were randomized to four groups (WT + saline, WT

Astrocyte MCT1 Expression Does Not Contribute to the Axonal Degenerative Phenotype Observed With Ubiquitous MCT1 Depletion.

We recently reported that the loss of oligodendrocyte metabolic support through the lactate and pyruvate transporter Monocarboxylate Transporter 1 (MCT1) is well tolerated into adulthood. Only with advanced aging did we observe axonal degeneration and hypomyelination due to the loss of MCT1 from oligodendroglia lineage cells. MCT1 is also expressed by other glial subtypes, such as astrocytes and endothelial cells where it has been suggested to be essential for learning and memory tasks. However, the importance of MCT1 in these cell types for long-term axonal metabolic support is still unknown.

An improved catch fish optimization based deep learning model for Parkinson disease classification using EEG signal.

Accurate and objective identification of Parkinson's Disease (PD) from Electroencephalogram (EEG) signals is important because EEG responses are complex, non-stationary, and difficult to evaluate manually. This work proposes an Improved Catch Fish Optimization (ICFO)-based encoder-decoder deep learning framework for PD classification using EEG signals. Initially, EEG signals are pre-processed using a bandpass filter to remove unwanted noise and retain the significant frequency range. Then, Tunable-Q Wavelet Transform (TQWT) is employed to decompose the EEG signals into informative sub-bands. T

Development of a Cognitive Screen Item Bank: Assessment of Individuals Across the Cognitive Spectrum, Including Advanced Dementia.

An item bank of cognitive items targeting the spectrum of cognitive function, with a focus on advanced dementia was developed with data from residents in long term services and supports (LTSS) settings. Latent variable models, including item response theory and factor analyses were applied to fifty items from multiple cognitive screening measures assessing memory, orientation, naming, attention, calculation, and following directions. Because the intent was to use the item bank to measure overall cognition, subdomains were not modeled. Factor analyses tested for essential unidimensionality, fol

Neonatal sevoflurane exposure disrupts oligodendroglial development and TGF-β-associated signaling in the prefrontal cortex and hypothalamus, with persistent behavioral deficits.

BACKGROUND: Repeated exposure to general anesthetics during early brain development may cause persistent neurobehavioral abnormalities, but the contribution of oligodendroglial dysfunction remains unclear. We examined whether neonatal sevoflurane exposure alters oligodendroglial development, myelin basic protein (MBP), and transforming growth factor-β (TGF-β)-associated signaling in the medial prefrontal cortex (mPFC) and hypothalamus. METHODS: C57BL/6 mouse pups received 3% sevoflurane in 40% O2 for 2 h daily on postnatal days 6-8. Motor and social behaviors were assessed at P30-P32. RNA seq

AI-Based Identification of Head Impact Locations, Speeds, and Force Based on Head Kinematics Simulations.

OBJECTIVE: With the development of wearable sensors, head kinematics data have become widely available. However, key impact information-such as impact direction, speed, and force-which is crucial for helmet development, is still not being directly measured. This study presents a deep learning model designed to accurately predict these head impact parameters from head kinematics during helmeted impacts. METHODS: Leveraging a dataset of 16,000 simulated helmeted head impacts using the Riddell helmet finite element model, we implemented a Long Short-Term Memory (LSTM) network to process the head

From brain waves to heartbeats: Exploring the combined role of electroencephalography and heart-rate variability in Alzheimer's disease diagnosis and management.

Alzheimer's disease (AD) affects not only memory and cognition but also the body's automatic functions, such as heart rate and blood pressure. These changes reflect the early disruption of the central autonomic network, the system that links the brain and the heart to maintain physiological balance. Electroencephalography (EEG) measures the brain's electrical activity and reveals patterns of cortical slowing and desynchronization, while heart-rate variability (HRV) reflects how flexibly the heart responds to internal and external demands. This narrative review brings together evidence publishe

Enhancing seizure prediction using a DC-SA-EBiLSTM framework with self-attention mechanism.

BACKGROUND: Accurately predicting seizures remains challenging. With advances in smart medical technology, EEG-based monitoring has become essential. This study aims to improve prediction accuracy using a hybrid framework that models multiscale EEG characteristics. METHODS: EEG signals are decomposed into multiple sub-bands using the Discrete Wavelet Transform, and representative time-frequency and nonlinear features are extracted. These features are fed into a channel-centric model integrating depthwise separable convolution, self-attention, and an enhanced bidirectional long short-term memo

An algorithm-enhanced stool DNA system improves the differential diagnosis of colorectal cancer versus Crohn's disease in high-risk symptomatic patients.

BACKGROUND AND OBJECTIVE: Crohn's disease (CD) and colorectal cancer (CRC) share many clinical symptoms, making non-invasive differential diagnosis difficult. FIT-sDNA is sensitive for CRC screening in average-risk populations but often gives false positives in CD patients due to inflammation-induced mucosal turnover. This study aimed to develop and validate an algorithm-enhanced system (FIT-sDNA-CA) to improve the specificity of CRC triage using current DNA tests. METHODS: The study enrolled 312 subjects, comprising a training cohort of 234 confirmed patients and a prospective validation coh

Cognitive and neurodevelopmental outcomes in spinal muscular atrophy: a scoping review.

Spinal muscular atrophy (SMA) has traditionally been described as a motor neuron disorder; however, increasing evidence suggests a broader neurodevelopmental involvement, particularly in the era of disease-modifying therapies. As survival and motor outcomes improve, cognitive and behavioral trajectories have become clinically relevant but remain inconsistently characterized. To systematically synthesize current evidence on cognitive and neurodevelopmental outcomes in children and adults with SMA, with particular focus on differences across phenotypes and therapeutic eras. A scoping review was

Pharmacological Role of 3-O-Methyldopa in the Central Nervous System-Mediated Behavioral Effects of L-DOPA in Rats.

Long-term l-3,4-dihydroxyphenylalanine (l-DOPA) therapy for Parkinson's disease increases plasma concentrations of 3-O-methyldopa (3-OMD), a major metabolite of l-DOPA. This study aimed to investigate the role of 3-OMD in the central nervous system (CNS)-mediated behavioral effects associated with chronic l-DOPA treatment in rats. The effects of 3-OMD on CNS function were examined using two rat models: repeated oral co-administration of l-DOPA/benserazide and single or repeated intraperitoneal (i.p.) administration of 3-OMD. Behavioral assessments included spontaneous locomotor activity, spont

The Rectal Mucosal Myeloid Niche in HIV-1 Persistence: Reservoir Support, Viral Sequestration, and Therapeutic Opportunities.

Persistent HIV-1 reservoirs remain a major barrier to a durable functional cure despite long-term suppressive antiretroviral therapy. Although resting memory CD4+ T cells constitute the best-characterized cellular reservoir, tissue microenvironments shape viral persistence and immune clearance. The rectal mucosa represents a specialized tissue niche containing HIV-susceptible target cells, antigen-presenting cells, microbial products, inflammatory cues, and local metabolic signals. Within this setting, myeloid-lineage cells, particularly tissue-resident macrophages and dendritic cells, may con

Activity-Dependent Changes in Axonal Action Potential Latency Coordinated with Synaptic Potentiation in Individual Hippocampal Neurons.

Neural plasticity enables the nervous system to adapt its structure and function in response to experience. Although synaptic plasticity is a central cellular mechanism underlying learning and memory, action potential propagation along axons is also subject to plastic regulation and critically shapes neural computation. However, how these distinct forms of plasticity are coordinated within individual neurons remains poorly understood. Here, we simultaneously monitored synaptic responses and antidromically evoked action potentials in hippocampal CA1 pyramidal neurons from male rats using whole-

Early Detection of Postharvest Potato Tuber Dry Rot Based on Hyperspectral Imaging and a Dual-Branch ResNet12-SE Spatial-Spectral Fusion Network.

Potato tuber dry rot is a major postharvest decay caused predominantly by Fusarium spp. During early infection, external symptoms may be absent even when faint internal browning, localized dehydration, and tissue structure changes have begun. Manual inspection, destructive cutting, and culture- or molecular-based assays are therefore poorly suited to rapid, nondestructive, high-throughput screening. We developed a near-infrared hyperspectral imaging method that combines spatial and spectral representations in a dual-branch ResNet12-SE network. The working dataset contained 1725 labeled records

In situ DC-primed vaccine combined with CD137 agonist elicits long-lasting antitumor immunity through cDC1-mediated tumor antigen-specific CD8+ T cell responses.

BACKGROUND: Intratumoral vaccines offer a promising avenue in cancer immunotherapy by harnessing the tumor microenvironment to stimulate immune responses. However, challenges persist in maximizing their effectiveness and addressing immune suppression within tumors. METHODS: Conventional in situ vaccine (CisVac) and α-CD137 antibody were administered in implanted mouse models of lung and colon cancer to evaluate therapeutic efficacy. The initiation mechanism of CD8+ T cells was determined using antibody blockade and transgenic mouse models. Single-cell RNA sequencing was used to further charac
