A case history, biopsy findings, and autopsy findings of an unusually long-lasting case of acute hemorrhagic leukoencephalitis in a young woman are presented. Diagnosis by stereotactic biopsy of brain lesions seen on computed tomography and magnetic resonance imaging scans had been attempted previously. On histologic examination the biopsy showed sheets of macrophages, which were found to contain unusual tubular inclusions on electron microscopy. The nature of these inclusions is discussed.

A 28-year-old white man, positive for HIV, who was admitted to hospital for pneumonia, had for 2 months had several fluctuating cutaneous purple nodules on his legs and abdomen. Cultures of two lesions were negative, but light and electron microscopy showed organisms characteristic of bacillary angiomatosis. The patient responded well to therapy with erythromycin. This is the first reported case of bacillary angiomatosis in South Africa.

Endothelin, a potent vasoconstrictor, was found to be ineffective in the treatment of experimentally induced corneal neovascularization. Endothelin was administered topically, subconjunctivally, and intraluminally in serial concentrations ranging from 0.0005 to 5.0 micrograms/mL in New Zealand white rabbits without effect. Electron microscopy of the neovascular cornea revealed the vessels consisted only of endothelin and pericytes. Hence, the vessels were not responsive to endothelin because they lacked contractile smooth muscle.

We describe the first case of renal tubular dysgenesis in a newborn female who survived 15 days. The immunohistochemical and lectin binding studies confirmed the lack of proximal tubule differentiation. Electron microscopy showed undifferentiated tubular epithelium. Widely patent cranial fontanels were present in the proposita. The father and sister showed acrocephalosyndactyly Saethre-Chotzen type. The parents were not related and there was no familial history of renal pathology.

Vascular endothelial cells of the basilar artery and secretory axons of the neurohypophysis from rabbits after experimental subarachnoid haemorrhage were investigated by postembedding peroxidase--anti-peroxidase technique for electron microscopy to detection of vasopressin (VP). The results indicate an lack of VP-positive endothelial cells in basilar artery, while VP-positive secretory granules were commonly present in the neurohypophysis. The results are discussed in terms of pathophysiological aspect of subarachnoid haemorrhage.

A 39-year-old woman presented with polyneuropathy, hepatomegaly, splenomegaly, endocrinopathy, monoclonal protein and skin changes, several of the many clinical features of the recently described POEMS syndrome. In addition, she had a Castleman's disease (angiofollicular lymph node hyperplasia). In this case ascites was a main presenting feature. Thus, the POEMS syndrome must be added to the list of rare causes of ascites. Electron microscopy of the liver showed perisinusoidal fibrosis.

Gold-coated plastic casts of the anterior segment ocular microcirculation of the rabbit eye were studied by scanning electron microscopy to define the interaction between ciliary sulcus fixated intraocular lenses and the anterior segment vasculature. A detailed description of the technique is given. Interactions such as loop deformation by the tissue and loop erosion into the vascular tissue are shown. Possible clinical applications of this experimental model are discussed.

The histopathology of the uterine and salpingeal epithelium of five cows affected with genital tuberculosis was studied. Both optical and scanning electron microscopy were used and the results were compared. Various forms and degrees of cellular lesions are described, including hypertrophy, flattening, detachment, nuclear protrusions and alterations of the cilia. In the different sites considered (uterus: intercaruncular and caruncular areas; salpinges: isthmus and ampulla) various characteristic lesions were observed.

The microvascular pattern of the feline renal medulla was studied by scanning electron microscopy using corrosion casts. At the corticomedullary junction, numerous efferent arterioles were observed giving rise to peritubular capillary plexuses and arteriole rectae. Considerable differences in vessel diameters were seen. Arteriolae rectae coursed around interlobar veins before returning to the corticomedullary area, as venulae rectae. Diffuse capillary papillary plexuses were seen protruding into the renal pelvis.

The abnormal cauliflower collagen fibrils present in skin of patients with Ehlers-Danlos type I were studied by electron microscopy and computerized image analysis. The size control of the few fibrils displaying a cauliflower shape is apparently lost, eventuating in an increased amount of collagen polymers constitutive of these fibrils. This situation is fundamentally different from that occurring in dermatosparaxis, an animal disease considered as a model for Ehlers-Danlos syndrome.

A case of chronic, corticosteroid-responsive arthritis affecting particularly one ankle in a patient with type IV hyperlipoproteinaemia is reported. After gout or inflammatory rheumatism of another nature had been excluded, and following synovial fluid examination and synovial membrane biopsy, a diagnosis of type IV hyperlipoproteinaemia rheumatism was made. At electron microscopy, the synovial membrane showed numerous large spumous cells and a peculiar appearance of the capillary vessels.

A patient with infiltration of the skin resulting in eyelid swelling and facial nodules was recognized as a case of sea-blue histiocyte syndrome with cutaneous involvement. Typical sea-blue histiocytes were found in the skin and confirmation was provided by electron microscopy. Hepatosplenomegaly, lung infiltrates and bone marrow involvement were the other symptoms. The relationship between sea-blue histiocyte syndrome and adult Niemann-Pick disease is also discussed.

A 34-year-old African patient with AIDS developed a new form of oral leucoplakia closely resembling the "hairy" leucoplakia described by Greenspan in male homosexuals in the San Francisco area. A herpes-like virus was seen on ultrastructural analysis with electron microscopy. This case supplies further evidence suggesting that the syndrome in patients originating in Central Africa is similar to the acquired immunodeficiency syndrome (AIDS) reported in American patients.

A case of pagetoid reticulosis is presented. Histopathology showed infiltration of the epidermis by mononuclear cells. Twenty percent of the mononuclear cells showed the presence of lysozyme indicating a histiocytic origin. Electron microscopy confirmed the presence of lymphocytes and histiocytes but these cells were outnumbered by Sézary cells. The presence of large numbers of Sézary cells indicates that pagetoid reticulosis is a cutaneous T-cell lymphoma closely related to mycosis fungoides.

Neurohistological and electron microscopy studies of the foci of periventricular leukomalacia allowed defining 3 stages in their development: necrosis, resorption and formation of the glial cicatrix or a cyst. Comparative study of the lesions in two groups of neonates (with and without sepsis) showed that sepsis promoted the process persistence and development of new pathological foci. One of the possible pathogenetic mechanisms might be the thrombosis of brain white substance vessels.

Two different tissues have been observed in specimens of tympanosclerotic material studied under light and electron microscopy. The first, devoid of cells and blood vessels, is a dense fibrous tissue similar to that already described in fibrous tympanosclerosis. The second resembles a bone tissue, containing osteocytes and vascular channels; it is laid down by metaplasic fibroblasts following the invasion of the first tissue by osteoclasts and blood vessels after its calcification.

A primary malignant fibrous histiocytoma of the spleen was studied by light and electron microscopy and is believed to be the first reported. The neoplasm was predominantly fibroblastic, with a characteristic storiform pattern, and included histiocyte-like cells, giant and foam cells confirmed by ultrastructural studies. Additionally, undifferentiated cells, intermediate cells and myofibroblasts were seen. The differential diagnosis from other sarcomas and the histogenesis of this tumour are discussed.

Two independent outbreaks of ectromelia in mice occurred in The Netherlands. In both cases, the causative virus was isolated and identified as ectromelia virus on the basis of serology, demonstration of antigen by indirect immunofluorescence, negative contrast electron microscopy, morphology of lesions on chorioallantoic membranes of embryonated chicken eggs, and cytopathogenicity for mouse cells. Inoculation of the virus into the dermis of rabbits demonstrated a low virulence for this species.

Greyish-white spots, varying in size, caused diagnostic problems in 2 patients with pre-invasive corneal carcinoma. Keratectomy specimens permitting light and electron microscopy indicated that the smaller spots predominating in one patient correlated with epithelial microcysts and vacuoles, while areas of parakeratosis accounted for the macroscopic white patches found in the other. A variety of ultrastructural abnormalities, including excessive basement membrane fibrillogenesis, were present in both cases.

Four cases of choroid plexus papilloma (CPP) obtained at the time of surgical excision were examined by electron microscopy and compared with normal choroid plexus (CP) of mouse and chick. In apical tight junctions fusion of the two outer leaflets of the adjacent cytoplasmic membrane was verified as in CP. This fact suggests that there is a blood-CSF barrier not only in CP but also in CPP.
