From antiquity through the Renaissance, congenital blindness was generally regarded as incurable, as noted in both medical and lay publications. For those who manage strabismus in adults, the patient with diplopia following retinal surgery presents a challenge. Mechanical and sensory factors may combine to preclude single binocular vision, and neutralizing the patient's strabismus may not be sufficient to permit resolution of their diplopia. OBJECTIVES: To determine the role of transnasal endoscopic approach to orbital lesions.
METHODS: Retrospective case series.
RESULTS: Three patients underwent endoscopic diagnostic surgery for orbital lesions. In all three patients was successfully finded out the diagnosis, the orbita was decompressed. Intratarsal keratinous cysts are uncommonly reported entities that originate within the tarsal plate. Previously reported cysts have always been solitary, whether primary or recurrent. The authors present an unusual case of a 50-year-old man with 11 intratarsal keratinous cysts on a single eyelid. Complete excision of all the cysts was curative for at least 2 months.

A case of grade III lipemia retinalis is reported in a 14-year-old girl presenting to the emergency department with fatigue and somnolence. Diabetic ketoacidosis diagnostic of type 1 diabetes mellitus and severe hypertriglyceridemia (23,508 mg/dL) were found on routine blood tests. Strabismus is common in craniosynostosis, with rates from 39% to 90.9% in Crouzon, Apert, Pfeiffer, and Saethre-Chotzen syndromes. This article reviews the epidemiology of strabismus in these disorders and discusses competing theories of the mechanism, including absent muscles, excyclorotation of muscles, and instability of muscle pulleys. The authors then review options for surgical treatment of the often complex ocular misalignment in these disorders.
