Primary Invasive Squamous Cell Carcinoma of the Foot.

Malignant transformation of wounds is a rare complication that if missed can lead to loss of life or limb. This case report presents a rare invasive variant of squamous cell carcinoma presenting in the setting of a chronic wound complicated by osteomyelitis. This aggressive form of squamous cell carcinoma has a high growth rate and a high propensity for metastasis and recurrence. Early intervention greatly decreases the risk of metastasis and recurrence. We present the systematic evaluation and surgical management of an aggressive primary tumor occurring in the forefoot.

Third Molar Eruption and Hardware Fungal Infection Following Treatment of Mandibular Angle Fracture.

Appropriate management of lower 3rd molars is crucial to successful treatment of mandibular angle fractures. A unique case of a patient who presented for initial management of a left mandibular angle fracture with an unerupted lower 3rd molar was discussed in this study. After treatment with open reduction and internal fixation of the fracture, the lower 3rd molar erupted and the patient subsequently developed fungal osteomyelitis and hardware failure. This was successfully treated with hardware removal, maxillomandibular fixation, and combined use of an intravenous antibiotic and oral antifun…

Retronychia a little-known cause of paronychia: A report of two cases in adolescent patients.

Retronychia is a clinical condition resulting from embedding of the nail plate into the proximal nail fold. We report two adolescent girls, 14 and 16 years of age, with a history of chronic proximal paronychia of the great toe, one of them developing osteomyelitis. After failure of treatment with several systemic antibiotics, nail avulsion was performed, leading us to the diagnosis of retronychia and with rapid and complete resolution of symptoms in both cases. Delay in diagnosis of retronychia can lead to local complications and prolonged discomfort.

Multifocal tuberculosis of long bones in an immunocompetent child.

Although tuberculosis (TB) is prevalent in many parts of the world, multifocal TB involvement of bones is rare. It is believed that less than 5% of skeletal TB is multifocal. We report a case of multifocal TB of long bones in an immunocompetent child. We describe the radiological and diagnostic features of TB osteomyelitis and outline its therapy and prognosis. To the best of the authors' knowledge, such a case with multiple long bone involvement without articular sequel and visceral involvement/constitutional symptoms has not been reported previously in the literature.

Osteopetrosis: a Case of 'Hostile Chest' Associated with Symptomatic Aortic Valve Stenosis.

Osteopetrosis is a heterogeneous group of heritable conditions in which there is a defect in bone resorption by osteoclasts. It is associated with an increased skeletal mass due to abnormally dense, but brittle, bones. Osteopetrosis varies greatly in severity, and fracture treatment remains a matter of controversy due to altered responses to fixation and the risk of osteomyelitis. The fate of sternotomy in this condition is unknown, and osteopetrosis could represent a situation of 'hostile chest'. Here, the case is described of a patient with osteopetrosis and concomitant symptomatic aortic va…

Marble Bone Disease: A Rare Bone Disorder.

Osteopetrosis, or marble bone disease, is a rare skeletal disorder due to a defective function of the osteoclasts. This defect renders bones more susceptible to osteomyelitis due to decreased vascularity. This disorder is inherited as autosomal dominant and autosomal recessive. Healthcare professionals should urge these patients to maintain their oral health as well as general health, as this condition makes these patients more susceptible to frequent infections and fractures. This case report emphasizes the signs and symptoms of marble bone disease and presents clinical and radiographic findi…

[My Back is Still Hurting!].

We present the case of a 39-year-old man who reported to the primary care physician for low back pain. Pain persisted despite extensive assessment and therapy. During the course, bilateral femoral neck fractures occurred and due to multiple enrichments in scintigraphy chronic multifocal (sterile) osteomyelitis was suspected. In the further follow-up the appropriate diagnosis of osteomalacia was established in bone biopsy and adequate treatment with Vitamin D was initiated. During therapy, the patient was free of pain or discomfort.

Mandibular osteonecrosis following herpes zoster infection in the mandibular branch of the trigeminal nerve: a case report and literature review.

Herpes zoster virus (HZV) infections are caused by reactivation of the varicella zoster virus. Reactivation symptoms commonly affect the thoracolumbar trunk, and rarely affect the mandibular branches of the trigeminal nerve. When the mandibular branches are involved, lesions appear proximal to the innervation area. This condition may be associated with exfoliation of the teeth and osteonecrosis of the jawbone. We report a case of mandibular osteomyelitis after herpes zoster infection and we present a review of the literature on mandibular-branch involvement of HZV-related osteonecrosis.

Hereditary sensory and autonomic neuropathy type IV and orthopaedic complications.

Hereditary sensory and autonomic neuropathy type IV (HSAN-IV) is a very rare autosomal recessive disorder characterized by recurrent episodes of unexplained fever, extensive anhidrosis, total insensitivity to pain, hypotonia, and mental retardation. The most frequent complications of this disease are corneal scarring, multiple fractures, joint deformities, osteomyelitis, and disabling self-mutilations. We reported the case of a 12-year-old boy. The goal was to discuss our decision-making and compare this case with cases described in the literature.

[The factorial analysis in evaluation of cell immunity of patients with pyo-inflammatory diseases of long bones and large joints].

The factorial analysis was applied to calculate the values of main components determining condition of cell immunity in patients with pyo-inflammatory diseases of long bones and large joints. It is demonstrated that in conditions of chronic purulent infection the main are the factors related with regulative T-lymphocytes involved in suppression of T-cell immunity. In case of development of fistula form of osteomyelitis the main role is attached to mechanisms related to markers of activation of T-lymphocytes and to derangement of expression of HLA-DR on monocytes in case of infection of prosthe…

Mycobacterium chelonae infection of the parotid gland.

Mycobacterium chelonae can cause numerous infections, including lung disease, local cutaneous disease, osteomyelitis, joint infections and ocular disease. With the exception of lung disease, these syndromes commonly develop after direct inoculation. The most common clinical presentation in immunocompetent individuals is skin and soft tissue infection. We present a case of M. chelonae infection of the parotid gland that was successfully treated with clarithromycin monotherapy. To our knowledge, this is the first case report of M. chelonae parotitis in an adult.

A facial swelling with multiple discharging sinuses: a diagnostic conundrum.

BACKGROUND: Many disease processes can present with facial swelling and multiple discharging sinuses. A differential diagnosis would include actinomycosis, tuberculosis and osteomyelitis, but tinea barbae is a relatively uncommon presentation. CASE REPORT: The following case report describes a 48-year-old jockey who was diagnosed with tinea barbae. It chronicles the investigations that were performed to determine the offending pathogen and the difficulty in achieving a diagnosis. DISCUSSION: The investigations and the pathophysiology of dermatophytic infections and their management in a maxi…

Skull base infection presenting with multiple lower cranial nerve palsies.

Diseases of the temporal bone causing lower cranial nerve palsies are uncommon. In the presence of bony erosion, they are highly suggestive of a malignant process. However, when there is a clear history of otitis externa in an immunocompromised or diabetic patient, a diagnosis of osteomyelitis and secondary inflammatory mass should be considered. We report 4 separate cases of infective skull base lesions causing multiple lower cranial nerve palsies in elderly patients who were not immunocompromised or diabetic, highlighting that this condition is not exclusive to this population.

Management of complications of open reduction and internal fixation of ankle fractures.

The management of complications resulting from the open reduction and internal fixation of ankle fractures is discussed in detail. The initial radiographic findings of the most common postsurgical complications of ankle fracture reduction are briefly discussed, namely lateral, medial, and posterior malleolar malunion or nonunion, syndesmotic widening, degenerative changes, and septic arthritis with or without concomitant osteomyelitis. Emphasis is placed on the management of these complications, with a review of the treatment options proposed in the literature, a detailed discussion of the aut…

Phaeohyphomycosis of subcutaneous tissue caused by Phaeoacremonium parasiticum.

Though Phaeoacremonium parasiticum is an unusual cause of human disease, subcutaneous infection, eumycetoma, osteomyelitis, arthritis and even disseminated diseases, such as fungemia and endocarditis have been reported. Here, we report a case of subcutaneous abscess on the forearm due to P. parasiticum in a 26-year-old woman. There were no obvious predisposing factors. The patient was treated with surgical debridement followed by intravenous amphotericin B and itraconazole to which she responded well. We report this case, being rare and the first from India.

[Elastic materials in surgery for inflammatory diseases of the vertebral column].

When 17 patients were surgically treated for spinal tuberculosis and osteomyelitis, anterior spine fusion was performed by A. Ye. Garbuz's procedure that was based on the replacement of a spinal defect by a carbon-carbonic implant that had rifampicin containers in the frontal sections, as well as by autologous bone grafts. The carbon-carbonic implants reliably fix an operated spinal portion, prevent the progression of kyphotic deformity, and create good conditions for autologous bone grafts to knit. An osteocarbonic block forms in the late postoperative period.

MR imaging of the diabetic foot: diagnostic challenges.

Pedal complications of diabetes have long presented a challenge for the clinician and radiologist predominately related to the difficulty in distinguishing infection from neuroarthropathy. The spectrum of diabetic foot infections is broad, ranging from callous and ulcer formation, to septic arthritis, abscess formation, and osteomyelitis. This article summarizes the MR imaging findings in the diabetic foot and the optimal pulse sequences. Focus is placed on aids in differentiating diabetic infection from other entities and increasing the specificity of diagnosing diabetic foot complications.

Recurrent sinus of the cheek due to a retained foreign body: report of an unusual case.

Pathological sinuses in the maxillofacial region are frequently encountered in clinical practice. These sinuses may be a sequelae of periapical and periodontal pathologies or infections like osteomyelitis, actinomycosis etc. Classical clinical symptoms and radiographic features accompany all these infections. Rarely, sinuses in the oro-facial region can be sequelae of retained occult foreign bodies like wood in the soft tissues. We report a case of recurrent sinus of the cheek caused by an occult wooden splinter and discuss its diagnosis and clinical management.

"Lumpy jaw" in exotic hoof stock: a histopathologic interpretation with a treatment proposal.

"Lumpy jaw" of artiodactyls and macropods, although often considered a manifestation of actinomycosis, is actually an osteomyelitis, perhaps commencing with a dental root abscess or trauma to the jaw. Anaerobes may be cultured from the lesions, and vegetable matter may be impacted in them. It is a chronic disease and difficult to treat. Successful treatment may include draining the abscess, cleaning the cavity with saline and antibiotics, flushing with a combination of hydrogen peroxide-sodium hypochlorite and Betadine, and apicoectomy and endodontic filling when active inflammation has subsid…
