We report two cases of severe cranio-orbito-facial trauma associated with a dislocation of the globe into the maxillary sinus. Simultaneous surgical repositioning the globe in the orbit and reconstruction of the orbital floor defect with an autogenous bone was performed in the first case. Postoperatively, the globe was in a normal position with a moderately reduced motility. The patient's visual acuity was reduced to light perception while indirect ophthalmoscopy was normal. In the second case, no surgery could be attempted because the patient never regained consciousness and died 7 days after

PURPOSE: To report two cases of Hallermann-Streiff syndrome with exudative retinal detachment after cataract surgery.
METHODS: Case report.
RESULTS: Four eyes of two patients with Hallermann-Streiff syndrome developed exudative retinal detachments after lensectomy and anterior vitrectomy at 2 and 4 months of age. Both patients had extreme microphthalmia. The exudative retinal detachment regressed spontaneously in three of the four eyes; however, one eye required subscleral sclerectomy. In one patient, the best-corrected visual acuity was 20/200 at 3 years of age; the other patient had good f

Ultraviolet (UV) light is the most common cause of radiation injury to the eye which in acute exposure causes photokeratitis and photoconjunctivitis. After a whole day exposure to the sun on the Mediteranian coast patient presented with mixed conjunctival injection, chemosis, edematous corneal epithelium with superficially present fluoresceine positive small pinpoint defects. Epithelial bullous changes, circular stromal infiltration in the middle stromal perifery and reduction of corneal transparency were more pronounced on the left eye. After treatment moderate conjunctival injection remained

A 27-year-old man with no history of intraocular surgery presented with progressive blurred vision in both eyes. Slitlamp evaluation revealed a white, thin membrane in the pupillary area that resembled posterior capsule opacification. A naturally formed lens was absent. Ultrasound biomicroscopy demonstrated a rarefaction of lens fibers in the equator region with a loss of the normal anatomic lens structure on the left side. Visual acuity was 20/32 with +10.00 -0.75 x 170 in the right eye and 20/100 with +11.50 -0.50 x 25 in the left eye. After capsulotomy and excision of the membranous capsule

BACKGROUND: Interferon associated retinopathy has been described mainly by Japanese working groups as a complication of systemic interferon therapy for various diseases. In Europe, only few reports exist, especially regarding interferon therapy for dermal malignant melanoma.
PATIENT: We report a 58-year-old female patient with symptomatic, bilateral microangiopathy caused by interferon for metastatic dermal malignant melanoma. Multiple bilateral cotton wool spots caused visual field defects and unilateral decreased visual acuity. The retinal changes were reversible when stopping interferon th

PURPOSE: To describe the results of the removal of ectopic lenses from patients with Marfan's syndrome using the scleral fixation method.
METHODS: Intraocular lenses (IOLs) were implanted in six eyes affected by lens dislocation (ectopia lentis) using either the scleral fixation method (five eyes) or a silicone IOL in the capsular bag (one eye). Mean patient age ranged from 8-11 years and follow-up ranged from 7-20 months.
RESULTS: Functional success was obtained in all eyes. Postoperative visual acuity was 20/20 to 20/40. One patient showed a dislocation of the IOL in the anterior chamber.

BACKGROUND: Malignant peripheral nerve sheath tumors are extremely rare orbital tumors that carry a poor prognosis despite wide excision with disfiguring surgery, chemotherapy, or radiotherapy. The authors present the youngest reported case, a 15-month-old boy who underwent an orbitotomy to excise a bilobed tumor from the right orbit.
FINDINGS: Histologic examination revealed a malignant peripheral nerve sheath tumor; standard treatment options, including orbital exenteration, chemotherapy, and radiotherapy, were denied and instead the patient was followed with serial computed tomography scan

At the age of 12, a prematurely born boy with an otherwise unremarkable past medical history developed bilateral optic neuritis associated with transverse myelopathy. Over the ensuing 3 years, recurrent bouts of optic neuritis OU, with dyschromatopsia, and acuity and field loss (arcuate, central, and paracentral scotomas) were controlled with increasing doses of corticosteroids. However, the patient became steroid-dependent and experienced recurrent optic neuritis during multiple attempts at tapering the steroids. He developed optic atrophy and steroid complications, including cushingnoid feat

In one of the seminal works on visual agnosia, Adler (1944, 1950) presented the case of a 22-yr-old woman who sustained carbon monoxide cerebral toxicity in the Cocoanut Grove nightclub diaster of 1942. We located this patient 40 yrs after injury and performed a detailed reevaluation. The patient demonstrated persistent deficits in visual recognition, characterized most prominently by defective recognition of elemental shape and form, associated with alexia, prosopagnosia, visuospatial disorientation and impaired visual imagery. Visual acuity, colour recognition, writing ability and verbal int

Seventeen patients with Behçet's disease were treated with intermittent intravenous bolus injections of cyclophosphamide. Twelve patients had severe eye lesions, 7 had central neurological disorders and 2 had both ocular and central nervous system involvement. Cyclophosphamide was given during 4 weeks on average, in mean doses of 922 +/- 127 mg. Each patient received an average of 10 +/- 5 injections. The drug was generally well tolerated; only one patient developed moderate and transient hepatocytolysis. Visual acuity was improved in 12 of the 22 eyes with active lesions, and signs of inflamm

A patient with immune deficiency, recurrent pyogenic infections and active chorioretinitis is described; in addition to agammaglobulinemia, both quantitative and qualitative T-cell deficiencies were documented. Furthermore, the patient's granulocytes (polymorphonuclear leukocytes), although normal in their bactericidal capacity for Staphylococcus, responded poorly to both leukocyte migration inhibition factor and neutrophil immobilizing factor obtained from normal cells. The immunologic features of this patient appear to comprise a new syndrome. Remarkable diminution of the ocular lesions and

OBJECTIVE: Complicated urinary tract infections (c-UTI) are expensive and frequently require hospitalization. This study estimated the impact of patient comorbidities, readmissions and treatment delay on the costs of complicated urinary tract infections in both inpatient and outpatient settings. A related aim is to provide the first estimates of the potential savings of treating patients currently treated as inpatients that have clinical characteristics similar to, though less complicated, to patients treated as outpatients.
DESIGN: Using one of the largest national claims data sets (30 milli

BACKGROUND: Anti-VEGF's have changed the prognosis of exudative age-related macular degeneration (AMD). Ranibizumab and aflibercept have proven their functional efficacy, but their use has revealed in real life a wide variety of patient profiles with varied responses to treatment. This work focused on patients receiving "intensive" IVT treatment with a sustained injection rhythm, also referred to as having a high treatment burden.
OBJECTIVE: The main objective of this work was to determine, in real-life conditions, the proportion of patients receiving "intensive" treatment among those being f

Background: Diabetic macular oedema (DME) is a major cause of vision impairment in individuals with diabetes mellitus, characterised by fluid accumulation in the macula due to increased vascular permeability. The growing prevalence of diabetes worldwide has led to an increasing burden of DME on healthcare systems. While current treatment options such as anti-vascular endothelial growth factor (anti-VEGF) injections, corticosteroids, and laser therapy exist, the variability in patient responses highlights the need for reliable prognostic tools. Imaging biomarkers, particularly those identified

IMPORTANCE: Central retinal artery occlusion (CRAO) is an ophthalmologic emergency that can result in blindness. At present, no proven therapy for CRAO exists. Treatment with fibrinolytic agents has shown promise but remains of unproven benefit.
OBJECTIVES: To assess the efficacy of systemic fibrinolytic therapy for patients with CRAO and to define a time window of efficacy.
DATA SOURCES: We systematically queried PubMed, Web of Science, and Scopus using the following index terms: "retinal artery occlusion" OR "retinal ischemia" AND "thrombolysis" OR "fibrinolysis" OR "tissue plasminogen act

INTRODUCTION: Hyperhomocysteinemia is known to be a risk factor in both retinal artery and retinal vein occlusions. We report the case of a young patient with combined occlusion of the cilioretinal artery and the central retinal vein due to hyperhomocysteinemia.
PATIENTS AND METHODS: A 23-year-old patient without significant medical history, presented for sudden, painless visual loss in the right eye. Ophthalmologic examination revealed best-corrected visual acuity of the right eye 8/10 P2, and 10/10 P2 on the left. Anterior segment exam was normal in both eyes, while the right fundus reveale

This case report describes a near-fatal airway obstruction in a woman attempting to conceal drug contraband in her mouth. A patient was brought to the emergency department by emergency medical services accompanied by police after she had refused to open her mouth after being detained. The patient was initially seen at triage, being awake and responsive, but still refusing to open her mouth. While in the emergency department, she rapidly deteriorated exhibiting signs of upper airway obstruction. The patient had aspirated a plastic bag presumably containing "crack" cocaine which completely occlu
