Structural violence and schizophrenia.

Despite clear evidence of a substantial biological basis to schizophrenia, there is also evidence that social, economic and political factors have considerable relevance to the clinical features, treatment and outcome of the illness. Individuals from lower socio-economic groups have an earlier age at first presentation and longer durations of untreated illness, both of which are associated with poor outcome. Individuals with schizophrenia are over-represented in the homeless population. Migration is associated with increased rates of mental illness, including schizophrenia, and this relationsh

Exploring the relationship between homelessness and risk factors for heroin-related death--a qualitative study.

The aim of this study was to explore the relationship between housing status, associated social networks and risk factors for heroin-related death. We used semi-structured face-to-face qualitative interviews, recorded, transcribed and analysed thematically by framework techniques at three centres providing services to homeless people in a large cosmopolitan city. Different types of accommodation for homeless people have differing social cultures which have an impact upon the amount of heroin used, likelihood of injecting alone or likelihood of achieving abstinence. Hostel accommodation appeare

Exploring risk perception and behaviour of homeless injecting drug users diagnosed with hepatitis C.

Injecting drug users (IDUs) are at the greatest risk of hepatitis C infection by using any item of injecting equipment that has come into contact with contaminated blood. Alongside this, homeless IDUs have been identified as being at increased risk of harm in their illicit drug taking behaviour. This study interviewed 17 hepatitis C positive homeless IDUs about their injecting practices. In-depth interviews explored the impact of a positive hepatitis C diagnosis on their injecting and identified their risk behaviours and perceptions. The interviews were tape-recorded, transcribed and analysed

The sex ratio distortion in the human head louse is conserved over time.

BACKGROUND: At the turn of the 19th century the first observations of a female-biased sex ratio in broods and populations of the head louse, Pediculus humanus capitis, had been reported. A study by Buxton in 1940 on the sex ratio of lice on prisoners in Ceylon is still today the subject of reanalyses. This sex ratio distortion had been detected in ten different countries. In the last sixty years no new data have been collected, especially on scalp infestations under economically and socially more developed conditions. RESULTS: Here we report a female bias of head lice in a survey of 480 schoo

Persistence of tropical ataxic neuropathy in a Nigerian community.

OBJECTIVES: The term tropical ataxic neuropathy (TAN) is currently used to describe several neurological syndromes attributed to toxiconutritional causes. However, TAN was initially proposed to describe a specific neurological syndrome seen predominantly among the Ijebu speaking Yorubas in south western Nigeria. In this study, the prevalence of TAN was determined in Ososa, a semiurban community in south western Nigeria described as endemic for TAN in 1969, and its neurological features were compared with Strachan's syndrome, prisoners of war neuropathy, the epidemic neuropathy in Cuba, and kon

Hepatitis C in Lithuania: incidence, prevalence, risk factors and viral genotypes.

BACKGROUND: The epidemiology of hepatitis C virus (HCV) infection has been studied in many countries. However, little is known about HCV infection in Lithuania, a Baltic country, that was part of the former Soviet Union. OBJECTIVES: The aim of this study was to determine and evaluate the etiology of acute viral hepatitis, the risk factors for acquiring HCV in comparison to hepatitis B virus (HBV), seroprevalence of anti-HCV among blood donors and risk groups of the population in Lithuania. The distribution of HCV genotypes from Lithuanian first-time blood donors was also assessed. STUDY DESI

Media distortion of the public's perception of recidivism and psychiatric rehabilitation.

OBJECTIVE: The public's perception about the success or failure of psychiatric rehabilitation is frequently dependent upon information received through the news media. The primary objective of this report is to present an example of how the news media can distort public perceptions of treatment outcome. METHOD: Verbatim quotations were presented from a television news series that alleged criminal recidivism by nine patients purportedly treated for various paraphilias at a large, community-based sexual disorders clinic. Brief case vignettes about each of the nine were then reviewed. This allow

Proceedings: Persepectives on adolescent medicine: concepts and program design.

The concepts and goals of a program in adolescent medicine should include development of a capability to focus on current health needs of youth in a variety of settings; to plan clinical services to meet those needs with the flexibility necessary to respond to changing future requirements; and to deliver service within such a context while simulataneously creating a milieu conducive to education and investigation into the very process and definition of adolescence. The Division of Adolescent Medicine at Montefiore Hospital and Medical Center was designed 7 years ago to fulfill these goals and

Structures of survival: Syndemic pathways to care among people with HIV involved with the criminal legal system.

BACKGROUND: Criminal legal system (CLS) involvement intersects syndemically with HIV, mental illness, substance use, and exposure to violence. Mass incarceration remains both a cause and consequence of racialized health inequities in the U.S. The post-release period is particularly vulnerable for people living with HIV (PLWH) who experience CLS involvement, underscoring the need for care models attuned to their structural and psychosocial contexts. This study examines how syndemic pathways and cumulative life experiences shape HIV care trajectories among people with HIV involved with the CLS.

Risk Factors of Latent Tuberculosis Infection among Incarcerated Populations: A Case-Control Study in Shenzhen, China.

INTRODUCTION: Latent tuberculosis infection (LTBI) constitutes a significant reservoir for future tuberculosis (TB) cases, particularly among incarcerated populations, which experience a disproportionately elevated burden. Recognizing modifiable risk factors in correctional facilities is crucial for formulating effective public health strategies to mitigate TB spread. METHODS: This case-control research examined 1,252 incarcerated inmates in Shenzhen, China. Subjects (n = 215) were interferon-gamma release assay (IGRA)-positive, whereas controls (n = 1,037) were IGRA-negative. Data were obtai

Community-Partnered Training in Trauma-Informed Primary Care for Patients Experiencing Reentry From Incarceration: A Pilot Training Study.

INTRODUCTION/OBJECTIVES: Patients returning to the community from incarceration (ie, reentry) are at heightened risk of experiencing trauma when interacting with the healthcare system. Healthcare professionals may not recognize patients' trauma reactions or know how to effectively respond. This paper describes the development and pilot evaluation of a single-session training to prepare primary care teams to deliver trauma-informed care (TIC) to patients experiencing reentry. METHODS: A multidisciplinary team including community members with lived experience engaged in a multiphase human-cente

A Novel Reentry Hotline Led by Community Health Workers to Address the Health Needs of People Returning From Incarceration: Transitions Clinic Network, California, June 2020‒June 2023.

Community members returning from incarceration experience serious health disparities, which intensified during the COVID-19 pandemic. In response, community health workers with lived experience of incarceration in the Transitions Clinic Network established a California reentry health care hotline to support time-sensitive linkages to postrelease medical care. From June 2020 to June 2023, the hotline supported 1276 people transitioning from incarceration. Lessons from this peer-led public health intervention are relevant for states utilizing Medicaid waivers to improve health equity for justice

Parental Incarceration, Attachment to Caregivers, and Young Children's Physiological Stress.

Parental incarceration can be stressful for children, leading to less optimal development. Although parental incarceration typically takes place among other adversities, resilience processes occur in many families, for example, in instances of secure child-caregiver attachment relationships. Yet, it is not known how secure attachments are associated with stress processes in children with incarcerated parents, especially in the context of other risks. The current study analyzed data from 67 child-caregiver dyads, measuring cumulative stress hormones (cortisol and cortisone) through children's h

Physician Perceptions of the Evaluation and Management of Incarcerated Patients in the Emergency Department.

BACKGROUND: Incarcerated patients in the United States are frequently seen in the emergency department. Although other specialties have reported differential care for patients who are incarcerated, little is known about the emergency physicians' evaluation and management of patients who are incarcerated. OBJECTIVE: The aim of our study was to characterize emergency physician (EP) perceptions of the emergency department (ED) care of incarcerated patients. METHODS: We surveyed emergency medicine resident and attending physicians at three academic safety-net EDs that regularly treat incarcerate

HIV Pre-Exposure Prophylaxis Among People with Criminal Justice Involvement in the United States: A Systematic Review.

People with criminal justice involvement (PCJI) face a heightened risk of HIV yet remain underserved by prevention strategies such as pre-exposure prophylaxis (PrEP). This systematic review synthesizes current evidence on PrEP utilization among PCJI, identifying key barriers and facilitators that influence uptake and adherence. Following Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines, we conducted a comprehensive literature search and thematic analysis. Twenty-five studies met inclusion criteria, with most conducted in the Northeast and Southern United States, an

Personality assessment inventory (PAI) in forensic and correctional settings: A comprehensive review.

As Forensic Psychology continues to expand as an independent field, professionals regularly resort to psychological assessment tools to assess people involved within the justice system. The Personality Assessment Inventory (PAI) is a 344-item, self-report inventory that aims to provide meaningful information for diagnosis and clinical decision-making, specifically relating to psychopathology, personality, and psychosocial environment. Its applicability in forensic settings has been increasingly recognized on account of its benefits in comparison to other self-report inventories (e.g., MMPI-2,

Trauma History Questionnaire: validation with novel samples of incarcerated women and perinatal women.

Trauma exposure is associated with numerous negative outcomes, many of which are amplified within at-risk populations. Two under-researched and at-risk populations, incarcerated women and perinatal women, both report high rates of trauma, psychopathology, and PTSD compared to the general population. One common measure of trauma exposure in various populations, including incarcerated women and perinatal women, is the Trauma History Questionnaire (THQ). However, no known studies have validated the THQ within these two unique, understudied populations. Using data from two studies of incarcerated
