Metagenomic Next-Generation Sequencing Diagnosis of Streptococcus agalactiae Meningitis in a Diabetic Patient.

Streptococcus agalactiae meningitis is primarily observed among neonates and is uncommon in adults. We present a rare case of Streptococcus … thickening and enhancement of the anterior and posterior longitudinal ligaments and meninges at the skull base, suggestive of Streptococcus agalactiae infection. After 10 days of anti-infective treatment with ceftriaxone, the patient's condition improved, and he was transferred to a local hospital for continued management.

Acalvaria: First Surviving Case From Egypt-Case Report and Comprehensive Review of the Literature.

BACKGROUND AND OBJECTIVES: Acalvaria is an extremely rare congenital malformation characterized by the absence of calvarial bones, with preservation of the skull base, facial bones, and usually normal brain tissue. Most reported cases are fatal in the neonatal period. The aim of this report was to present the first surviving case from Egypt and provide a comprehensive review of the literature. METHODS: A detailed clinical, radiological, and imaging evaluation of a full-term male infant diagnosed with acalvaria was conducted. A literature search was performed to identify and summarize previous…

Delayed Post-Traumatic Temporal Bone Encephalocele With Lateral Ventricle Herniation.

The main objective of this report was to describe a very unique clinical case of a post-traumatic temporal bone encephalocele with lateral v … l hematoma (SDH) managed with successful bur hole drainage. Long-term follow-up especially in large skull base defects is critical to monitor delayed occurrence of encephalocele and/or cerebrospinal fluid (CSF) leak. Surgical repair with the removal of devitalized brain tissue and multilayer closure may result in the development of subacute SDH after success …

Study of Arachnoid Granulations and Their Pits in Head Injuries.

Although arachnoid granulations (AGs) and their pits have been known for over 300 years, they have not been extensively studied regarding the injuries inflicted upon them. This study aims to identify the injuries sustained by the AGs and their pits in cases involving head injuries. The study involved autopsies of 130 bodies with head injuries and 40 without head injuries. The AGs and their respective pits were examined for morphology, types, injuries, and histology. Injuries in AGs and their pits were documented in 61 cases. Intradiploic, irregular AGs, and their pits are more susceptible to i

Two case reports on central nervous system metastasis from nasopharyngeal carcinoma and a literature review.

Nasopharyngeal carcinoma (NPC) is a common malignant tumor in Southern China. The clinical manifestations, including extensive local invasion and distant metastasis, exhibit certain patterns. As we know, the most common sites of distant failure are bone, liver and lung. Although intracranial invasion by direct extension from the nasopharynx through the skull base in patients with locally advanced disease is not uncommon, metastasis to the central nervous system (CNS) is very scarce. We analyzed and summarized the literature about central nervous system metastasis of NPC by reviewing the Englis…

A Durable Response in Sinonasal Undifferentiated Carcinoma Treated With Subtotal Resection and Ultra-Early Postoperative Chemoradiotherapy: A Case Report.

Sinonasal undifferentiated carcinoma (SNUC) is a rare and aggressive malignancy that arises in the nasal cavity and paranasal sinuses. Although gross total resection (GTR) is associated with improved local control and survival, the anatomical complexity and frequent skull base invasion often render complete resection infeasible. In such cases, the timing of postoperative therapy may be critical for disease control. We report a case of a 68-year-old man with stage IV SNUC who underwent subtotal endoscopic resection, followed by ultra-early postoperative concurrent chemoradiotherapy (CCRT) initi…

A Report of Two Cases of Planum Sphenoidale Injuries With Cerebrospinal Rhinorrhea Following Office Balloon Dilation of the Sphenoid Ostium: Lessons Learned.

Balloon-assisted dilation (BAD) of paranasal sinus ostia is a Food and Drug Administration (FDA)-approved minimally-invasive procedure used … demonstrates that serious complications can occur. These case reports demonstrate the potential for skull base injury, particularly in hyper-pneumatized sphenoid sinuses with dehiscent bone. Preoperative anatomic evaluation is crucial to identify at-risk patients. During BAD, trauma to critical structures may cause CSF leak and result in pneumocephalus. This …

Atraumatic Tension Pneumocephalus in a Shunted Patient: A Case of Rapid Neurological Decline.

Pneumocephalus is a rare but potentially life-threatening condition caused by the presence of air within the intracranial cavity. It commonly arises from trauma, neurosurgical procedures, infections, malignancies, or spontaneous causes. In patients with ventriculoperitoneal (VP) shunts, pneumocephalus may result from complications such as altered cerebrospinal fluid dynamics or skull base defects leading to a vacuum-like effect. We report a case of an 83-year-old male with a history of VP shunt placement for normal pressure hydrocephalus who presented with acute neurological deterioration, inc…

Dural arteriovenous fistulas of the anterior condylar confluence involving the anterior condylar vein within the hypoglossal canal: Two case reports.

BACKGROUND: Dural arteriovenous fistulas (DAVFs) of the anterior condylar confluence (ACC) are rare vascular lesions at the skull base, often characterized by complex venous anatomy and variable clinical presentations. Their symptoms may overlap with those of cavernous sinus (CS) DAVFs, leading to potential misdiagnosis. Advanced imaging techniques and individualized treatment approaches are essential for accurate diagnosis and effective management. CASE DESCRIPTION: We present two cases of ACC DAVFs successfully treated with transvenous embolization (TVE). The first case involved a patient w…

Surgical management of a temporal meningoencephalocele with a patient-specific combined craniofossa prosthesis: illustrative case.

BACKGROUND: Meningoencephaloceles can originate through any defect in the cranial bones, including the skull base. They can be completely asymptomatic or present with typical symptoms such as headaches, seizures, or meningitis. OBSERVATIONS: The authors present the case of a 54-year-old female who presented with right-sided ear and temporomandibular joint (TMJ) pain. Computed tomography showed a large lytic lesion of the squamous part of the temporal bone with cortical bone destruction, extending to the mandibular fossa. Magnetic resonance imaging demonstrated a temporal bone meningoencephalo…

[CEREBROSPINAL FLUID (CSF) LEAK FOLLOWING COVID-19 NASAL SWAB: A CASE REPORT].

Coronavirus 19 (Covid-19) emerged as a global pandemic with over 12,900 deaths in Israel alone. On average, more than 40,000 tests to detect the virus daily were performed in Israel. According to the US Centers for Disease Control and Prevention (CDC), the recommended methods for testing are nasopharyngeal swab, mid-turbinate, or anterior nasal swab specimen. Although these methods are generally safe, they are not without risks. Particularly, traumatic or improperly angled test may injure the skull base and lead to a cerebrospinal fluid leak. This rare condition presents with headache, unilate…

Olfactory Carcinoma, Similar yet Distinct to Olfactory Neuroblastoma: A Case Report and Literature Review.

INTRODUCTION: Olfactory carcinoma (OC) is a rare tumor that is often misdiagnosed as olfactory neuroblastoma (ONB). Due to its rarity, there … athological analyses confirmed high-grade OC. Treatment comprised induction chemotherapy, extensive skull base tumor resection, adjuvant chemoradiotherapy, and reconstructive surgery in both cases. The 54-year-old remains disease-free at 7 years, while the 48-year-old demonstrates no evidence of disease at 8 months. CONCLUSION: Accurate diagnostic different …

Primary CNS Burkitt Lymphoma Presenting as Sudden Bilateral Blindness in a Patient With Underlying Kabuki Syndrome: A Case Report.

Burkitt lymphoma is an aggressive B-cell non-Hodgkin lymphoma (NHL). Primary CNS lymphoma (PCNSL) is a rare disease, and the subtype of Burkitt lymphoma presenting as a sole CNS lesion is an even rarer diagnosis. Acute sudden blindness is a rare presenting symptom of PCNSL or NHL in general. We present an interesting case of a four-year-old boy with dysmorphic features whose visual examination showed a sudden bilateral loss of vision. There was bilateral eye proptosis and complete ptosis. Extraocular muscles were fixed straight. The pupils were fixed and mid dilated bilaterally and there was g

An unusual finding of the Porus trigeminus: Case report with histological findings.

Many anatomical variants can potentially contribute to trigeminal neuralgia. These include compression by the superior cerebellar artery and less commonly, bony compression near the trigeminal cave. Herein, we report the gross and histological findings of a cadaveric specimen found to have a bony roof of the trigeminal cave. During the routine dissection of a male cadaver, an unusual finding of the skull base was observed. Palpation over the porus trigeminus identified a completely ossified roof. The bony spicule was 1.22 cm long and 0.76 mm wide. The trigeminal nerve was noted to have an inde…

Percutaneous transorbital transvenous embolization of a cavernous sinus dural arteriovenous fistula via the superior orbital fissure-technical video.

Carotid-cavernous dural arteriovenous fistulas causing debilitating ocular symptoms and/or retrograde cortical venous drainage necessitate curative treatment, which is achieved by disrupting the proximal draining vein. Transvenous embolization of carotid-cavernous dural arteriovenous fistulas can be achieved through the superior or inferior petrosal sinuses, facial veins, or superior ophthalmic veins.1, 2 However, if these approaches are not feasible, various percutaneous approaches have been described that use the skull base foramina to provide direct access to the cavernous sinus.3, 4 Here w…

The Evolution of Pituitary Gland Surgery from the Ancients to the Millennials.

Located at the base of the skull, the pituitary gland has a long and controversial history, not only in terms of its anatomy, but especially in the functions it performs and in the attempt to approach it surgically. Considered by Galen of Pergamon to have a role in releasing waste products of the brain, a theory accepted until the 17th century, the pituitary gland became a separate entity once with the anatomical descriptions of the famous Andreas Vesalius. At the beginning of the 18th century, researches of the time began to be more and more interested in this gland, trying to identify its fu

[History of frontal sinus surgery and current view of the problem. Part 1].

Surgical treatment of inflammatory diseases of the frontal sinus is one of the biggest challenges of modern otorhinolaryngology. Close proximity of the frontal sinus and frontal sinus drainage pathways to the skull base, the orbit and the anterior ethmoid artery, great limitations with its visualization and instrumentation, and high risk of the frontal recess scarring cause difficulties in either endoscopic or external approaches to the frontal sinus. At the same time endoscopic approach to the frontal sinus is considered as preferred method of frontal sinusitis surgical treatment by majority …
