Socioeconomic and Clinical Determinants Driving Access to BRCA Genetic Testing in Cancer : A Case-Control Study Using Observational Electronic Health Records Across Multiple Sites.

IMPORTANCE: BRCA genetic testing is critical for cancer risk assessment, treatment and personalization, yet substantial underutilization persists. Socioeconomic and clinical factors may strongly influence testing uptake; therefore, identifying the potential drivers to BRCA testing and treatment is essential for addressing gaps in access, increasing retention into care, and improving cancer outcomes. OBJECTIVE: To quantify the putatively causal effects of SDoH on BRCA genetic testing among individuals with breast, ovarian, pancreatic, and prostate cancers and to develop a predictive model to i

Applying the Vertically Integrated Project Model to Youth-Inclusive Social Media Coding of Zyn-Related YouTube Shorts: Methodological Case Study.

BACKGROUND: The rapid evolution of the nicotine and tobacco product (NTP) marketplace continues to outpace existing research. Although adolescents are active participants on social media who both consume and generate content, this age group has rarely been included in analyses of NTP content, raising a methodological challenge for accurate interpretation and coding of youth-oriented NTP content. Most research examining oral nicotine product content online has focused on TikTok, despite YouTube's widespread adolescent use. One promising framework is the vertically integrated project (VIP) model

Environmental enrichment during social isolation prevents behavioral impairments and morphological alterations of the midcingulate cortex in aggressive socially isolated mice.

Social isolation (SI) in rodents affects neuronal integrity and behavioral regulation. On the other hand, enriched environment (EE) has been reported to yield beneficial behavioral and neural effects in group-housed rodents. The midcingulate cortex (MCC) is a brain region that is increasingly implicated in socioemotional regulation; however, whether SI induces structural alterations in this region, and whether EE can mitigate such effects, remains unclear. In the present study, we examined the behavioral consequences of SI and the associated dendritic alterations in MCC pyramidal neurons. Furt

Psychosocial and health-related determinants of self-reported unmet healthcare needs in Türkiye: evidence from a nation-wide survey.

BACKGROUND: Equitable access to healthcare services remains a major public health concern, particularly for vulnerable populations. Despite significant improvements following comprehensive healthcare reforms in Türkiye, perceived unmet healthcare needs persist, disproportionately affecting individuals with chronic illnesses, depressive symptoms, and limited social support. This study, therefore, examines gender-stratified associations between these factors and different types of perceived unmet healthcare needs. METHODS: We analyzed data from the 2019 Türkiye Health Survey (n = 17,084), a nati

Quality of life and patient-reported outcomes after non-surgical management of early-stage endometrial cancer and endometrial hyperplasia with atypia: a long-term follow-up of the feMMe phase II randomized clinical trial.

OBJECTIVE: Non-surgical, fertility-sparing management using a levonorgestrel intra-uterine device is a de-escalation strategy for selected patients with early-stage endometrial adenocarcinoma or endometrial hyperplasia with atypia. Although the oncologic outcomes of the feMMe trial have been reported, long-term patient-reported outcomes remain insufficiently characterized. This study evaluated long-term quality of life, self-efficacy, social support, and patient-reported experiences following conservative management. METHODS: The feMMe trial (open-label, randomized phase II study) includes th

Understanding long-term HIV survivorship among African American/Black and Latinx persons living with HIV in the United States: a qualitative exploration through the lens of symbolic violence.

BACKGROUND: Persons living with HIV (PLWH) are living longer, although racial/ethnic and socioeconomic status (SES) disparities persist. Yet, little is known about the experience of living with and managing HIV over decades. The present study took a qualitative approach and used the lens of symbolic violence, a type of internalized, non-physical violence manifested in the power differential between social groups. We focused on adult African American/Black and Latinx (AABL) PLWH from low-SES backgrounds. METHODS: Data were drawn from two studies with AABL PLWH in New York City (N = 59). After

Understanding Resilience in Psychosis: An Emerging Field.

The resilience paradigm offers a complementary perspective to risk-focused models of psychosis by asking how favorable outcomes can be maintained or regained despite substantial vulnerability, adversity, or illness-related burden. Schizophrenia resilience research encompasses both processes that buffer antecedent risk and reduce the likelihood that vulnerability leads to psychotic illness, and those that support adaptation and recovery after illness onset. Drawing on clinical, psychological, social, and biological research, we consider the relationship between resilience and recovery, resilien

From Retraumatization to Recovery: Making Acute Psychiatric Care Work for Transgender and Gender-Diverse People.

Transgender and gender-diverse people frequently experience acute psychiatric care as traumatizing because of structural stigma, including misgendering, discontinuation of gender-affirming medications, and restrictions on gender expression. The authors leveraged lived-experience evidence and population-based data to outline changes that reduce traumatization and improve continuity after discharge: standardized documentation and display of chosen names and pronouns, consistent access to gender-affirming medications and gender expression items, trauma-informed de-escalation, involvement of chose

Bodies, Boundaries, and Belonging: A Nurse's Guide to Providing Affirming Sexual Health Care for Transgender and Gender-Diverse Individuals.

Transgender and gender-diverse (TGD) individuals experience profound inequities in sexual health due to intersecting social, structural, and clinical barriers, including limited provider competency. Using a nursing lens, we examine clinical practices for delivering sexual health care to TGD populations. Specifically, we address sexual identity, sexual function, gender-affirming care considerations, and clinical guidance for comprehensive sexual health assessment. We also discuss screening, prevention, and treatment of sexually transmitted infections and HIV. Emphasizing person-centered and tra

Behind the High Scores: Adaptive Compliance and the Evaluation of Care Among Dependent Older Adults in Chinese Long-Term Care.

This convergent mixed-methods study examines how institutional dependency shapes older adults' evaluations of long-term care in China. A SERVQUAL-based survey of 154 residents across three Chongqing facilities was integrated with 35 audio-recorded cases of score-narrative inconsistency. Despite high survey reliability, qualitative findings revealed lowered expectations, strategic face-work, and mismatches between abstract quality concepts and lived care experiences. High satisfaction scores may thus reflect adaptive compliance shaped by structural dependency and cultural norms. The findings hi

Emotional Abuse in Women and Girls Mediated by Patriarchal Upbringing and Its Impact on Sexism and Mental Health: A Narrative Review.

Sexism is a type of structural discrimination that can manifest as the subjugation of woman in familial and social roles. Gender-based violence often occurs in societies where patriarchal norms exist. Violence against women and girls (VAWG) includes physical, sexual and psychological/emotional abuse directed towards females. Emotional violence against women and girls is usually underestimated, if not overlooked. Patriarchal upbringing can predominantly result in the emotional abuse of female children. This narrative review will discuss the impact emotional abuse in women and girls mediated by

Balancing motherhood and drug addiction: the transition to parenthood of addicted mothers.

This is a study about balancing motherhood and drug addiction, during the transition to parenthood. Few studies have dealt with the parental experience of drug-addicted mothers. The participants included 24 drug-addicted mothers, on methadone, with ages 25-42 and with children 1-32 months of age. Semi-structured interviews were conducted and analyzed according to Grounded Theory. The mothers' main concern was the ambivalence they felt towards pregnancy/motherhood and drug addiction, which was associated with strong feelings of guilt. Confronted with this ambivalence their maternal role becomes

Vaccine innovation, translational research and the management of knowledge accumulation.

What does it take to translate research into socially beneficial technologies like vaccines? Current policy that focuses on expanding research or strengthening incentives overlooks how the supply and demand of innovation is mediated by problem-solving processes that generate knowledge which is often fragmented and only locally valid. This paper details some of the conditions that allow fragmented, local knowledge to accumulate through a series of structured steps from the artificial simplicity of the laboratory to the complexity of real world application. Poliomyelitis is used as an illustrati
