Worsening abducens paresis and chronic orbital venous congestion after carotid-cavernous fistula embolization: long-term motility outcomes.

PURPOSE: Carotid-cavernous fistulas (CCFs) produce abnormal arteriovenous communication between the carotid circulation and the cavernous sinus, leading to orbital venous hypertension and neuro-ophthalmic manifestations. Although endovascular embolization is the treatment of choice and usually results in rapid hemodynamic normalization, delayed ocular motor sequelae after successful closure remain incompletely characterized. We report a case of persistent and worsening abducens nerve paresis and chronic orbital venous congestion following embolization of a direct CCF. METHODS: This case repor

Use of visual acuity to screen for significant refractive errors in preschool children.

Purpose: To determine the sensitivity and specificity of visual acuity (VA) for the detection of refractive errors in preschool children from the Sydney Pediatric Eye Disease Study. Methods: Data from both eyes were pooled for a total of 2332 observations. Children aged 3-6 years had a comprehensive eye examination including VA (monocularly tested using the Electronic Visual Acuity HOTV test) and cycloplegic (cyclopentolate 1%) refraction using the table-mounted autorefractor (RK-F1 Auto Ref-Keratometer; Canon, Tokyo, Japan). Clinically significant refractive error was defined as: Myopia ≤-1.0

Orbital dermoid cyst presenting with hypertropia and defective depression: a rare presentation.

INTRODUCTION: Intraconal dermoid cysts are a rare clinical entity. They are typically associated with symptoms such as proptosis, orbital pain, and diplopia. However, atypical presentations without these common features can occur, making diagnosis more challenging. METHODS: We describe a clinical case of a one-year-old child who presented with left hypertropia and restricted depression in the absence of proptosis or other clinical manifestations. High-resolution orbital and cranial magnetic resonance imaging (MRI) was performed. RESULTS: Orbital MRI revealed an intraconal lesion consistent w

Spectrum of non-ischemic oculomotor nerve palsies at a tertiary care centre - case series and literature review.

PURPOSE: Oculomotor nerve palsies (ONPs) have a varied range of clinical presentations. Though they are mainly divided into ischemic and non-ischemic variants, with the former being more common, a detailed clinic-radiological evaluation is needed in all cases to ascribe an etiology in all cases. METHODS: We conducted a retrospective review of records to identify all patients with non-ischemic ONPs presenting to a tertiary care teaching hospital over 5 years. RESULTS: Nineteen cases of non-ischemic ONP were found. The various causes of non-ischemic ONPs included congenital, malignancy-associa

Comparative evaluation of surgical outcomes in acute acquired comitant esotropia managed with and without pre-operative prism adaptation test.

Introduction: Acute acquired comitant esotropia (AACE) is a rare type of esotropia for which until now the main treatment modalities prescribed are prism glasses, botulinum toxin injection and surgery. The purpose of our study is to compare the surgical outcomes in patients with AACE managed with and without pre-operative prism adaptation test (PAT). Methods: We have done a prospective, interventional study for the duration of one year. Forty patients with AACE were randomly divided into two groups by computerized randomization - those undergoing surgery after pre- operative PAT and those unde

Undercorrection of symptomatic esophoria after unilateral medial recession.

PURPOSE: Symptomatic esophoria (SE) is a latent esodeviation that progresses into a manifest esotropia. Symptoms may include diplopia, headaches, and asthenopia. One muscle surgery is controversial due to concerns about undercorrections and/or the development of horizontal gaze incomitance. The aim of this retrospective observational study was to compare undercorrection rates in SE patients receiving standard orthoptic evaluation versus those supplemented with a prism adaptation test (PAT). METHODS: We reviewed records of patients with SE who underwent unilateral medial recession (UMRc) surge

Brown syndrome with Marcus-Gunn jaw-winking phenomenon and ptosis: distinct entities or a part of the congenital cranial dysinnervation spectrum?

INTRODUCTION: Brown syndrome involves restricted elevation in adduction due to superior oblique tendon anomalies. Marcus-Gunn jaw-winking Syndrome (MGJWS) is a congenital synkinesis between jaw movement and eyelid elevation. Their coexistence along with ptosis in the same eye is extremely rare. METHODS: We report a case of an 8-year-old girl presenting with right eye motility restriction consistent with findings of Brown Syndrome along with MGJWS and ptosis. RESULTS: The patient showed right hypotropia with limited elevation in adduction, consistent with Brown syndrome. Ptosis resolved with

Aberrant regeneration of third nerve combined with sixth nerve palsy in the setting of trauma: surgical results.

Introduction: Aberrant regeneration of the third nerve is a known entity after trauma. It is important to recognize signs of aberrant regeneration and keep lid aperture disparity in mind before choosing the surgical procedure in such cases. Surgical procedure in these cases is often customized on case-to-case basis. Methods: Two cases with combined third and sixth nerve palsy are described. Both the patients had synergistic adduction on elevation and were treated by a single muscle transposition. Results: Both patients were orthotropic in the primary position with no diplopia and a reduction i

Relationships between fusional convergence, suppression depth, and exotropia control in intermittent exotropia.

Purpose: To assess the correlation between the contribution rates of fusional convergence from the dominant and non-dominant eye and suppression depth and exotropia control. Study design: Cross-sectional prospective study. Methods: The fusional convergence of 25 participants with intermittent exotropia (mean age 10.8 ± 3.4; range 6-18 years) was measured with an eye-tracking system. The contribution rate was defined based on the amplitude of fusional convergence during refusion relative to the exo-deviation angle. The suppression depth was assessed, and exotropia control was evaluated using th

Medial rectus re-advancement: more bang for your buck?

Introduction: Although there are guidance tables in the literature on surgical dosage for primary medial rectus resection-lateral rectus recession surgery, there is a lack of consensus on the surgical gains in medial rectus re-advancement (MRR) for the management of consecutive exotropia. We compared the surgical outcomes of primary medial rectus resection-lateral recession (RR) surgery, to MRR in patients with consecutive exotropia. Methods: Retrospective, electronic note review of consecutive patients undergoing primary RR surgery for basic exotropia (RR group), convergence insufficiency-typ
