Two cases of complex obstetric fistulae are reported. Poor assessment and mismanagement of labour were revealed. The resulting fistulae were vesico-urethro-rectovaginal and vesico-cervico-vaginal. Plastic repair using a modified Martius graft was used in case 1. Considering the young age of the patient, transplantation of the ureters into the rectum or colon was deemed undesirable. We present a case of a post-traumatic finger-pulp reconstruction in which we used a free finger-pulp flap elevated from the excised extra digit in a patient with coincidental polydactyly of the thumb that had not been previously corrected. It resulted in satisfactory reconstruction of the defect with a well-padded, sensitive fingertip (static two-point discrimination of 3 mm) and excellent cosmetic appearance with the simultaneous correction of the polydactyly.

Knowing that the injured MCL and PCL can heal and that the injured ACL and lateral side, predictably, will not heal are the bases for the treatment approach to these injuries. (1) Allow MCL healing nonoperatively. (2) Allow PCL healing to occur as long as PCL laxity is 2+ or less (reconstruct the PCL acutely if posterior drawer is > 2+ initially). PURPOSE: In patients who undergo the Monfort abdominoplasty the umbilicus is placed high on the abdominal wall. We describe a technique for abdominal wall reconstruction in patients with the prune-belly syndrome with the modification of neoumbilicus creation at an anatomically normal position.

Hallermann-Streiff syndrome is rare, with approximately 150 case reports in the world literature. Between 1982 and 1997, 29 patients with congenital absence of the vagina underwent modified McIndoe vaginoplasty at the Division of Plastic and Reconstructive Surgery, Ege University Medical School, Izmir, Turkey. Ultrasonography-assisted laparoscopic creation of a neavagina was performed on 6 patients suffering from vaginal aplasia. As the needle progress through the vesicorectal space, ultrasonography provided an accurate check of the bladder and rectal integrity, and improved the ease of the surgical procedure. Ultrasound can be considered an effective and reliable advance for laparoscopic management of vaginal aplasia.

To compare the operative and sexual efficiencies of transabdominal-vulvar and transvulvar techniques for peritoneal vaginoplasty in patients with congenital absence of the vagina, we treated 36 patients by peritoneal vaginoplasty, originally described by Rothman, 31 with the transabdominal-vulvar technique and five with a new transvulvar technique. One-hundred-and-forty patients with hypospadias and chordee were reviewed from the points of view of surgical results and hospital costs. Ninety cases constituted a historical series, and 50 patients a current personal series. All were operated on by Byars' two-staged protocol with release of the chordee followed by urethral reconstruction. In order to resolve the shortcomings of traditional pedicled abdominal skin flap, the pedicled abdominal subcorium vascular-net flap was reformed and applied clinically. Twenty-eight cases with scar on hand or wrist were treated, including 20 males and 8 females. The age was ranged from 18 to 35 years old. The key point in the design was rotating 45 degrees of the flap from the primary site toward the pedicle.
