Magnetic resonance imaging of the pediatric knee.

Magnetic resonance imaging (MRI) is an excellent modality for imaging the pediatric knee because of its superb soft-tissue contrast, multiplanar capability, and lack of ionizing radiation. The knee is the most common joint imaged by MRI in the pediatric population. The majority of studies are performed on older children or adolescents with pain and/or known trauma. There are variations of injury and patterns of injury that are unique to children. The knee of a child also may be studied by MRI for other indications, including tumor, inflammatory disease, and developmental abnormalities. It is i

[The hair-thread tourniquet syndrome of the toes and penis].

We present eight cases of the hair-thread tourniquet syndrome which affected the toes in 6 cases and presented strangulation of the penis in two. All patients were infants and the mechanism of injury was the strangulation by hair or thread wrapped around an appendage. Neither lesions were promptly recognized by their physicians and the patients were seen in the emergency room with swelling, erythema and a circumferential scar with the typical distal edema. The lesions had ischemic signs, but without tissue necrosis. All patients were treated by immediate removal of the constricting fibers and

Insertional Achilles Tendinopathy: A Minimally Invasive Approach.

Insertional Achilles tendinopathy is a common overuse injury of the lower extremity classically treated with open debridement and tendon repair. However, wound complications and prolonged non-weightbearing are limiting factors for this treatment. Recently, a minimally invasive endoscopic debridement with double-row suture repair has emerged as an alternative, soft-tissue friendly option for operative management. The purpose of this study is to examine the benefits of and provide a technique for this procedure using the Arthrex MIS Achilles Speedbridge System (Naples, FL).

Intra-articular Proximal Biceps Tenotomy and Transfer: A Technique to Repair Specific Types of Rotator Cuff Tears.

The stability and function of the glenohumeral joint predominantly rely on the integrity of the rotator cuff tendons and their attachment at the footprint region on the greater tuberosity of the humerus. Current therapeutic strategies for rotator cuff tears are progressively evolving toward individualized and refined approaches, with vigorous debates ongoing regarding the optimal surgical techniques for specific injury patterns. In this Technical Note, we describe an arthroscopic intra-articular proximal biceps tenotomy and transfer procedure that increases footprint coverage and establishes a

The volar approach to proximal interphalangeal joint arthroplasty.

Proximal interphalangeal joint arthroplasty has resulted in good outcomes in patients treated for osteoarthritis, posttraumatic arthritis, and rheumatoid arthritis. Most hand surgeons complete arthroplasties of the proximal interphalangeal joint through a dorsal approach. However, for the past 7 years, we have had positive results with a volar approach. We describe this technique, which avoids injury to the extensor tendon and allows for a more simplified approach to postoperative therapy compared with the therapy regimen required after the dorsal approach.

Subcoracoid dislocation of the distal end of the clavicle. A case report.

Subcoracoid dislocation of the outer end of the clavicle in association with an acromial fracture is a rare injury usually produced by high energy trauma that moves the clavicle posterior to the conjoined tendon and inferior to the coracoid process. To restore the acromioclavicular joint, open reduction is needed. After the reduction is accomplished, the clavicle has to be stabilized with a coracoclavicular screw. The acromial fracture can be reduced and held with nonabsorbable sutures placed through the bone. To avoid osteolysis of the distal end of the clavicle and to be able to repair the t

Dorsally displaced epiphyseal fracture of the phalangeal base.

A dorsally displaced epiphyseal fracture of the middle phalanx (Salter-Harris Type I) is described. The epiphyseal fragments were attached to the central slip of the extensor tendon and collateral ligaments. The articular surface of the PIP joint was intact and smooth. The epiphysis was reduced and fixed without cutting the central slip or the collateral ligaments 8 months after injury. This kind of fracture can occur in the PIP and DIP joints, and presents special diagnostic difficulties. Open reduction is evidently necessary to correct the displacement.

[Traumatic chondrosis dissecans of the ulnar head (case report)].

A thirty-five-year old woman suffered from a sprain of her wrist joint. Painful blocking and snapping during supination appeared. No pathology was seen in the X-rays and the arthrogram. At surgery the expected dislocation of the extensor carpi ulnaris tendon was not discovered but rather a handle-like dissection of the cartilage of the ulnar head was squeezed into the inferior radioulnar joint by supination. The patient recovered soon after resection of the dissected cartilage. In retrospect the injury could have been diagnosed by arthrography of the inferior radioulnar joint.

Combined repair technique for patellar tendon rupture using two suture anchors and three transosseous sutures: a retrospective cohort study.

BACKGROUND: Patellar tendon rupture is an uncommon injury that usually requires surgical repair, but no consensus exists regarding the ideal repair technique. Suture anchor repair and the transosseous suture method have be widely used, but the suture anchor technique may cause anchor pull-out, while the transosseous suture method may result in insufficient suture strength. Therefore, we propose and evaluate the efficacy of combined repair technique with two suture anchors and three transosseous sutures for patellar tendon repairs. METHODS: We conducted a retrospective cohort study of patients

The 2-Window Posterolateral vs Single-Window Approach for Ankle Fracture Fixation.

BACKGROUND: The posterolateral approach to the ankle allows for reduction and fixation of the posterior and lateral malleoli through the same surgical incision. This can be accomplished via 1 or 2 surgical "windows." The purpose of this study is to compare outcomes including wound complications following direct fixation of unstable rotational ankle fracture through the posterolateral approach using either 1 or 2 surgical windows. METHODS: One hundred sixty-four patients with bi- or trimalleolar ankle fractures treated using the single-window posterolateral approach (between the peroneal tendo

Ultrasonographic Assessment of Median Nerve and Carpal Tunnel Variations: Implications for the Interventional Management of Carpal Tunnel Syndrome.

OBJECTIVES: Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy. Anatomic variations of the median nerve (MN) and carpal tunnel (CT) may predispose patients to CTS or increase procedural complexity and risk during carpal tunnel release (CTR). This study aimed to systematically evaluate the prevalence of 25 MN and CT anatomic variations and measurements using ultrasound (US) in patients referred for electrodiagnostic testing (EDX). METHODS: In this cross-sectional observational study, patients underwent diagnostic US exams of the MN (mid-brachium to distal CT), foc

Individual Region- and Muscle-specific Hamstring Activity at Different Running Speeds.

INTRODUCTION: Hamstring strain injuries typically occur in the proximal biceps femoris long head (BFlh) at high running speeds. Strain magnitude seems to be the primary determinant of strain injury, and may be regulated by muscle activation. In running, BFlh strain is largest in the proximal region, especially at high speeds. However, region-specific activity has not been examined. This study examined the proximal-distal and intermuscular activity of BFlh and semitendinosus (ST) as a function of increasing running speed. METHODS: Thirteen participants ran at steady speeds of 4.1 (slow), 5.4 (

Comparison of 3 Suture Constructs on Upper One-third Subscapularis Tendon Tear Healing: An Animal Study Comparing Biomechanical, Histological, and Imaging Characteristics of Different Techniques.

BACKGROUND: Although arthroscopic single-row repair is a common approach for treating subscapularis tendon tears, its tendon-to-bone healing efficacy remains to be fully validated, particularly in terms of collagen remodeling and local inflammatory response. The H-loop knotless technique aimed to simplify the procedure and enhance repair stability. However, its healing outcomes compared with either the single-row (SR) or modified Mason-Allen (MMA) technique require experimental evaluation. PURPOSE: The aim of the study was to compare single-row, MMA, and H-loop techniques for repairing upper
