Etiological distribution and mortality factors in pericardial effusion: Insights from a large cohort study.

BACKGROUND: Pericardial effusion represents a potentially life-threatening clinical entity arising from diverse pathophysiological processes, with etiological prevalence demonstrating substantial geographic and ethnic variation. Despite this recognized heterogeneity, epidemiological data characterizing the distribution of pericardial effusion etiologies in Indian populations remain limited. METHODS: This observational cohort study enrolled 695 consecutive patients presenting with moderate-to-large pericardial effusion at a tertiary cardiac care center. Comprehensive demographic and etiologica

Tuberculosis and AIDS co-morbidity in Brazil: linkage of the tuberculosis and AIDS databases.

This study evaluated differences in AIDS patients with and without tuberculosis (TB) in Espírito Santo State, Brazil. Standard regional AIDS (SINAN, SISCEL, SICLOM and SIM) and tuberculosis (SINAN) databases were used. TB and AIDS databases were linked using Reclink software, version 3, with SPSS software support to identify co-infected cases. Data from July 2000 to June 2006 in Espírito Santo State were linked. The results showed 3,523 adult AIDS cases and 9,958 adult TB cases resulted in 430 co-infected patients, who were compared to 1,290 AIDS patients who never developed TB. Among 430 co-i

Mechanisms of mucosal and parenteral tuberculosis vaccinations: adenoviral-based mucosal immunization preferentially elicits sustained accumulation of immune protective CD4 and CD8 T cells within the airway lumen.

The mechanisms underlying better immune protection by mucosal vaccination have remained poorly understood. In our current study we have investigated the mechanisms by which respiratory virus-mediated mucosal vaccination provides remarkably better immune protection against pulmonary tuberculosis than parenteral vaccination. A recombinant adenovirus-based tuberculosis (TB) vaccine expressing Mycobacterium tuberculosis Ag85A (AdAg85A) was administered either intranasally (i.n.) or i.m. to mice, and Ag-specific CD4 and CD8 T cell responses, including frequency, IFN-gamma production, and CTL, were

Some Manifestations of Tuberculosis in Otorhinolaryngology - Case Series and a Short Review of Related Data from South-Eastern Europe.

Tuberculosis is a disease of global outreach that may affect the entire human body but is most commonly located in the lungs. Otorhinolaryngological manifestations of tuberculosis are rare, mostly occurring secondary to pulmonary disease but nevertheless represent significant diagnostic challenges. Nasopharyngeal tuberculosis is rare, representing around 1% of all upper air-way localizations and the most common presentation is in the form of adenoids. Tuberculous glossitis (oral tuberculosis) is even scarcer and may present in various clinical forms, usually mimicking a malignant neoplasm, or,

The role of interferon-gamma release assays in the diagnosis of active tuberculosis.

BACKGROUND: Interferon-gamma release tests are appealing alternatives to the tuberculin skin test (TST) for latent tuberculosis infection. OBJECTIVES: To determine the yield of the Quantiferon TB Gold test (QFT-G) in the diagnosis of active tuberculosis disease, with a focus on elderly patients, human immunodeficiency virus (HIV) co-infection, and extra-pulmonary tuberculosis (EPTB). METHODS: The QFT-G test was performed in 98 patients suspected of having active tuberculosis. The results were evaluated for each subgroup of patients and compared to the results of the TST. RESULTS: Active tub

Protective efficacy of intranasal vaccination with Mycobacterium bovis BCG against airway Mycobacterium tuberculosis challenge in mice.

The effect of route of immunization on the protective efficacy of BCG against tuberculosis has been investigated. Immunoprotection was monitored by evaluating the bacterial burden in the lungs and spleen of mice challenged with Mycobacterium tuberculosis H(37)Rv after BCG immunization by intranasal (i.n.) and subcutaneous (s.c.) routes. Our results showed that as compared to s.c. BCG immunization, intranasal BCG vaccination induces significantly higher immune responses at local level (mediastinal lymph nodes, cervical lymph nodes and lung). Further, i.n. BCG vaccination induced significantly h

Inhaled aerosol viral-vectored vaccines against tuberculosis.

Bacille Calmette-Guérin (BCG) remains the sole licensed vaccine against tuberculosis (TB), despite its variable efficacy in protecting against pulmonary TB. The development of effective TB vaccines faces significant challenges, marked by the absence of validated correlates of protection and predictive animal models. Strategic approaches to enhance TB vaccines and augment BCG efficacy include utilising prime-boost strategies with viral-vectored vaccines and exploring innovative delivery techniques, such as mucosal vaccine administration. Viral vectors offer numerous advantages, including the ca

Identification of Novel Potential Vaccine Candidates against Tuberculosis Based on Reverse Vaccinology.

Tuberculosis (TB) is a chronic infectious disease, considered as the second leading cause of death worldwide, caused by Mycobacterium tuberculosis. The limited efficacy of the bacillus Calmette-Guérin (BCG) vaccine against pulmonary TB and the emergence of multidrug-resistant TB warrants the need for more efficacious vaccines. Reverse vaccinology uses the entire proteome of a pathogen to select the best vaccine antigens by in silico approaches. M. tuberculosis H37Rv proteome was analyzed with NERVE (New Enhanced Reverse Vaccinology Environment) prediction software to identify potential vaccine

[152 cases of tuberculosis in African Negro immigrants in a Parisian service of internal medicine (author's transl)].

This study of 152 cases of tuberculosis in African Negro immigrants seen between 1972 and 1976 showed the predominance of especially pulmonary respiratory lesions and/or hilar ganglio-mediastinal lesions. The other lesions are frequent, i.e. the lesions of lymph nodes, bones, peritoneum, liver, pericardium ; the multiple lesions are likewise frequent. Smears and culture of sputum and/or biopsy of organs or tissues such as pleura, lymph node, liver and peritoneum are the diagnostic procedures. In spite of frequent resistance, treatment with isoniazid, rifampicin and ethambutol gave good results

Spina Ventosa of the Thumb With Valgus Deformity in a Female With No Pulmonary Involvement: A First Report of Its Type in an Immunocompetent Adult From India.

Small bone involvement caused by Mycobacterium tuberculosis infection is an uncommon clinical entity. Usually, the condition is linked to the spread of bacteria from the lungs. It is rather uncommon for people without pulmonary seeding, trauma, or a history of tuberculosis to experience isolated episodes of spina ventosa. We describe a rare case of an immunocompetent Indian woman, aged 25, who complained of swelling in her left thumb. The diagnosis of primary tubercular dactylitis of the thumb was made after a thorough clinical and laboratory workup, and she received conservative treatment.

Prostatic lithiasis complicating granulomatous prostatis of tuberculous origin: About a case report.

Urogenital tuberculosis remains a frequent disease in our country. It is the most common extra-pulmonary location. Prostatic involvement is extremely rare. We report the observation of a prostatic lithiasis complicating a granulomatous prostatitis of tuberculous origin, revealed essentially by obstructive and storage lower urinary tract symptoms. The diagnosis was suspected on imaging and clinical findings and confirmed by histology. Treatment consisted of endoscopic resection of the prostate associated with endoscopic ballistic lithotripsy. Prostatic lithiasis is a rare condition with a poorl

First things first: Importance of eosinophil count in diagnosing occult parasites.

Tropical pulmonary eosinophilia (TPE) is a rare allergic manifestation to the filarial nematode. A 38-year old male and a 15-year old female presented with cough and breathlessness. Their complete blood count showed eosinophilia. This finding was overshadowed by the radiological findings suggestive of tuberculosis. The diagnosis of TPE was confirmed by filarial antigen detection test and both the patients were successfully treated with diethylcarbamazine. TPE presents with cough and breathlessness and can be often confused with tuberculosis, especially in endemic settings. An important clue in

Tuberculosis of Glans Penis- A Rare Presentation.

Tuberculosis (TB) of penis is a very rare entity, even in developing countries. It may present as primary or secondary to Pulmonary TB (PTB). Penile TB mimics carcinoma penis, granulomatous penile ulcer, genital herpes simplex, granuloma inguinale and HIV infection. We, hereby, present the case of a 57-year-old male patient who presented to us with ulcerative growth over glans penis and was clinically diagnosed as carcinoma penis, however biopsy of the lesion showed evidence of tuberculosis which was supported by chest X-ray.

[Levofloxacin (Tavanic) in complex therapy of tuberculosis].

Preclinical and clinical investigations of levofloxacin in complex with antituberculosis drugs of the main and reserve groups in 152 patients with newly diagnosticated drug resistant pulmonary tuberculosis demonstrated its efficacy and safety. The adverse events due to levofloxacin (8.6% of the cases) disappeared after discontinuation of the drug use without any affection of the patient's organs. Levofloxacin is applicable in antituberculosis therapy of patients with extended, acute progressing or polycavernous tuberculosis of the lungs. It provides significant clinical improvement and if nece

Treatment of tuberculosis in patients with advanced human immunodeficiency virus infection.

BACKGROUND AND METHODS: Infection with the human immunodeficiency virus (HIV) increases the risk of tuberculosis and may interfere with the effectiveness of antituberculosis chemotherapy. To examine the outcomes in patients with both diagnoses, we conducted a retrospective study of all 132 patients listed in both the acquired immunodeficiency syndrome (AIDS) and tuberculosis case registries in San Francisco from 1981 through 1988. RESULTS: At the time of the diagnosis of tuberculosis, 78 patients (59 percent) did not yet have a diagnosis of AIDS, 18 patients (14 percent) were given a concomit

Sensitivity and specificity of Xpert MTB/RIF for extrapulmonary non-spinal osteoarticular tuberculosis.

BACKGROUND: Conventional diagnostics for Osteoarticular Tuberculosis (OATB) are slow and prone to inaccuracy due to low bacterial load, delaying treatment. Xpert MTB/RIF is WHO-recommended for TB cases, but there is a dearth of literature for non-spinal OATB. METHODS: In a 4-year study (Sept 2016-Feb 2020), non-spinal osteoarticular infections were retrospectively analyzed. Xpert MTB/RIF sensitivity and specificity were evaluated against tuberculosis defined by histology and culture against patients having culture-isolated pyogenic infections as a reference standard for negative tuberculosis.
