Predicting hyperresponse among pacemaker-dependent nonischemic cardiomyopathy patients upgraded to cardiac resynchronization.

INTRODUCTION: Right ventricular (RV) pacing engenders left ventricular (LV) dyssynchrony and may diminish LV systolic function, promote adverse cardiac remodeling, and foster heart failure (HF). This process may be reversible in some pacemaker-dependent patients upgraded to cardiac resynchronization therapy (CRT). We examined the clinical characteristics of pacemaker-dependent patients who exhibit hyperresponse (i.e., normalization of LV function) with CRT upgrade. METHODS AND RESULTS: We identified 51 chronically RV-paced patients with no coronary artery disease, LV ejection fraction (EF) ≤

Improved cardiopulmonary exercise function after modified Nuss operation for pectus excavatum.

OBJECTIVES: Patients with pectus excavatum have compromised cardiac function during exercise. We hypothesized that the Nuss technique would improve cardiopulmonary function during exercise. METHODS: We investigated 75 teenagers (49 patients and 26 controls) at rest and during bicycle exercise prior to surgery and 1 year postoperative. RESULTS: Prior to surgery, patients had a lower cardiac index 6.6 ± 1.1 l/min/m(2) when compared with controls 8.1 ± 1.0 l/min/m(2) during submaximal exercise, P = 0.0001. There was no difference in heart rate or increase in heart rate between the two groups. O

Elastic properties of the reconstructed aorta in hypoplastic left heart syndrome.

BACKGROUND: Patients with repaired coarctation of the aorta retain abnormal elastic properties of the aorta. It is not known whether patients with hypoplastic left heart syndrome also manifest abnormal elastic properties after palliative surgery. The presence of such abnormalities may have important clinical implications as reduced aortic compliance might adversely impact single right ventricular function. METHODS: We prospectively evaluated the elastic properties of the aorta in a cohort of patients with hypoplastic left heart syndrome who had undergone the Norwood procedure with aortic arch

Partial Fontan: advantages of an adjustable interatrial communication.

Systemic venous hypertension after the Fontan procedure is a major cause of mortality and morbidity, accounting for 11 of 16 deaths in our series of 228 Fontan procedures. A partial Fontan with a residual atrial septal defect (ASD) would allow controlled right-to-left shunting to reduce venous pressure and improve cardiac output while maintaining a reduced but acceptable arterial oxygen saturation. This allows complete or graded closure of the ASD after the discontinuation of cardiopulmonary bypass in the operating room or at any time in the postoperative period by exposing the snare under loc

Duration-Dependent Cardiac Effects of Anabolic-Androgenic Steroid Use in Strength-Trained Athletes: A Systematic Review and Meta-Analysis.

PURPOSE: Nonmedical anabolic-androgenic steroid (AAS) use is increasing, and its duration-dependent cardiac effects remain unclear. We evaluated cardiac structure and function in AAS-using strength-trained athletes, overall and by exposure duration. METHODS: We searched PubMed, Embase, Cochrane Central, and SCOPUS through November 2025 for observational studies comparing AAS-using and nonusing strength-trained athletes of either sex. Random-effects models pooled mean differences (MD) overall and across four exposure strata (<6 months, 6 months-3 years, 3-6 years, >6 years); meta-regression as

Changes in Biventricular Cardiac Mechanics After Transcatheter Edge-to-Edge Repair for Severe Tricuspid Regurgitation.

BACKGROUND: Transcatheter edge-to-edge repair (TEER) is an established therapy for severe tricuspid regurgitation (TR). Invasive pressure-volume (PV) analysis is the gold standard for characterizing ventricular function and ventricular-vascular interactions. The effects of tricuspid TEER on biventricular PV relationships are unknown. OBJECTIVES: The authors aimed to assess postprocedural changes in right (RV) and left ventricular (LV: 1) end-systolic and end-diastolic pressures and volumes; 2) ventricular-arterial coupling, expressed as end-systolic elastance (Ees) to effective arterial elast

Right ventricular dilatation and systolic dysfunction and relationship to QRS duration in patients with left bundle branch block and cardiomyopathy.

BACKGROUND: Marked QRS widening in patients with left bundle branch block (LBBB) may reduce efficacy of cardiac resynchronization therapy (CRT). We hypothesized that extreme QRS prolongation may accompany right ventricular (RV) dilatation/systolic dysfunction (RVD/RVsD) as well as left ventricular dilatation/systolic dysfunction (LVD/LVsD). METHODS: We assessed rates of both ventricular dilatation and systolic dysfunction according to widening of QRS duration (QRSd) in 100 consecutive cardiomyopathy patients with true LBBB (QRSd ≥ 130 ms in female or ≥140 ms in male, QS or rS in leads V1/V2,

Novel Morphological Features on CMR for the Prediction of Pathogenic Sarcomere Gene Variants in Subjects Without Hypertrophic Cardiomyopathy.

Background: Carriers of pathogenic DNA variants (G+) causing hypertrophic cardiomyopathy (HCM) can be identified by genetic testing. Several abnormalities have been brought forth as pre-clinical expressions of HCM, some of which can be identified by cardiovascular magnetic resonance (CMR). In this study, we assessed morphological differences between G+/left ventricular hypertrophy-negative (LVH-) subjects and healthy controls and examined whether CMR-derived variables are useful for the prediction of sarcomere gene variants. Methods: We studied 57 G+ subjects with a maximal wall thickness (MWT

Short-term follow up of the Ross operation in children.

BACKGROUND AND AIMS OF THE STUDY: Aortic valve disease in the pediatric population poses special problems to surgeons and cardiologists. The pulmonary autograft has proven to be a good alternative for aortic valve replacement and left ventricular outflow tract (LVOT) reconstruction in this special group. METHODS: Forty-one children (mean age 10.0 +/- 4.8 (SD) years; range: 35 days to 18.8 years) underwent aortic root replacement with a pulmonary autograft between February 1994 and April 1998. Twenty-one patients (51%) had previous cardiac surgery; seven (17%) had balloon valvulotomy. Aortic r

Systemic endothelial dysfunction in children with idiopathic pulmonary arterial hypertension correlates with disease severity.

BACKGROUND: Idiopathic pulmonary arterial hypertension (IPAH) is a life-threatening disease manifested by progressive pulmonary vascular remodeling, compromised pulmonary blood flow and right heart failure. Most studies have explored how pulmonary endothelial function modulates disease pathogenesis. We hypothesize that IPAH is a progressive panvasculopathy, affecting both pulmonary and systemic vascular beds, and that systemic endothelial dysfunction correlates with disease severity. Recent studies have demonstrated systemic endothelial dysfunction in adults with pulmonary hypertension; howeve

Continuous-Flow Left Ventricular Assist Devices and Valvular Heart Disease: A Comprehensive Review.

Mechanical circulatory support with implantable durable continuous-flow left ventricular assist devices (CF-LVADs) represents an established surgical treatment option for patients with advanced heart failure refractory to guideline-directed medical therapy. CF-LVAD therapy has been demonstrated to offer significant survival, functional, and quality-of-life benefits. However, nearly one-half of patients with advanced heart failure undergoing implantation of a CF-LVAD have important valvular heart disease (VHD) present at the time of device implantation or develop VHD during support that can lea

Resuscitative effect of hyperoxia fluid on high-altitude hemorrhagic shock in rats and antishock mechanisms.

Pathophysiological characteristics of hemorrhagic shock at high altitude are different from that at plain which involve severe injury, high mortality, difficult treatment and compromised liquid tolerance. High-altitude pulmonary/cerebral edema and multiple-organ dysfunction render the conventional treatment ineffective. Herein, we evaluated the resuscitation effects of hyperoxia solution on high-altitude hemorrhagic shock in rats. For this purpose, a rat model of high-altitude (3,658 m) hemorrhagic shock was established on the plateau and hyperoxia solution (4 ml/kg) was infused through extern

A Neural Network Approach to Identify Left-Right Orientation of Anatomical Brain MRI.

PURPOSE: This study presents a novel application of deep learning to enhance the accuracy of left-right orientation identification in anatomical brain MRI scans. Left-right orientation misidentification in brain MRIs presents significant challenges due to several factors, including metadata loss or ambiguity, which often occurs during the de-identification of medical images for research, conversion between image formats, software operations that strip or overwrite metadata, and the use of older imaging systems that stored orientation differently. METHOD: A three-dimensional convolutional neur

Pre- and post-hemodialysis differences in heart failure diagnosis by current heart failure guidelines in patients with end-stage renal disease.

BACKGROUND: Patients with end-stage renal disease (ESRD) who are on hemodialysis (HD) have reduced vascular compliance and are likely to develop heart failure (HF). In this study, we estimated the prevalence of HF pre- and post-HD in ESRD using the current guidelines. METHODS: We prospectively investigated HF in ESRD patients on HD using echocardiography pre- and post-HD. We used the structural and functional abnormality criteria of the 2021 European Society of Cardiology guidelines. RESULTS: A total of 54 patients were enrolled. The mean age was 62.6 years, and 40.1% were male. Forty-five p

Reversible osimertinib-associated heart failure mimicking cancer recurrence in EGFR-mutated non-small cell lung cancer after pneumonectomy: A case report.

Osimertinib is a third-generation epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor widely used in patients with EGFR mutation-positive non-small cell lung cancer (NSCLC). Although generally well tolerated, clinically notable cardiovascular toxicities, including heart failure, have been recognized. The present study describes the case of a 77-year-old man who developed symptomatic heart failure 5 months after initiating osimertinib treatment for postoperative recurrence of NSCLC. The patient presented with dyspnea and right-sided pleural effusion, which was initially suspected

Indication and timing in tricuspid interventions.

Tricuspid regurgitation (TR), previously considered a secondary valvular disorder with limited clinical implications, is now recognized as a progressive and prognostically significant disease. The increasing prevalence due to aging populations and common comorbidities, such as atrial fibrillation and heart failure, has underscored the clinical urgency of addressing TR effectively. Transcatheter tricuspid valve interventions (TTVI) have emerged as valuable therapeutic alternatives, especially for patients at high surgical risk. This review addresses critical clinical questions regarding optimal
