Transcranial Sonography in Neurodegenerative Diseases with Cognitive Decline.

Transcranial sonography (TCS) of the brain parenchyma detects alterations in the substantia nigra (SN), raphe nuclei and basal ganglia; this technique has been established as a tool for the early diagnosis of Parkinson's disease and differential diagnosis from atypical parkinsonian syndromes. Here, we aimed to review the main applications of TCS in neurodegenerative diseases presenting with dementia syndrome, focusing on Alzheimer's disease (AD), dementia with Lewy bodies (DLB), frontotemporal lobar degeneration, idiopathic normal pressure hydrocephalus, and atypical and secondary parkinsonism

Cognitive Disabilities and Long-term Outcomes in Children with Epilepsy: A Tangled Tail.

Cognitive problems ranging from mild specific learning problems to profound intellectual disability (ID) are very common in children with epilepsy. For most affected patients there is good evidence that the cognitive problems are present at the onset of seizures and do not deteriorate over time. There is no evidence that a few seizures lead to cognitive deterioration. An exception may occur in children with epileptic encephalopathies, although this contention is not always easy to prove. ID is a strong predictor of intractable epilepsy, and the greater the degree of the ID the greater the risk

Assessment of Mental Status.

Assessing the mental status of patients with a neurobehavioral disorder is a critical element in the diagnosis and treatment of these patients. This assessment should always be performed after the patient's history it taken and a general physical as well as a neurologic examination is completed. The mental status examination commences with observing the patient's appearance and level of consciousness. The examiner should also pay attention to patient's social behavior, emotional state and mood. There are 3 major means of assessing a patient's mental status. One type attempts to determine if th

A Systematic Review and Meta-Analysis of Brief Versus Ultrabrief Right Unilateral Electroconvulsive Therapy for Depression.

OBJECTIVE: Electroconvulsive therapy (ECT) is an effective depression treatment, but it has potential cognitive side effects. Ultrabrief pulse (UBP) right unilateral (RUL) ECT is an increasingly used treatment option that can potentially combine efficacy with lesser cognitive side effects. However, current trials are underpowered or have conflicting results. A systematic review and meta-analysis was conducted to evaluate the relative efficacy and cognitive effects of brief pulse (BP) and UBP RUL ECT. DATA SOURCES: MEDLINE, EMBASE, PsycINFO, CENTRAL, DARE, and the International Clinical Trials

Postoperative cognitive dysfunction: current developments in mechanism and prevention.

Postoperative cognitive dysfunction (POCD) is a subtle disorder of thought processes, which may influence isolated domains of cognition and has a significant impact on patient health. The reported incidence of POCD varies enormously due to lack of formal criteria for the assessment and diagnosis of POCD. The significant risk factors of developing POCD mainly include larger and more invasive operations, duration of anesthesia, advanced age, history of alcohol abuse, use of anticholinergic medications, and other factors. The release of cytokines due to the systemic stress response caused by anes

Vascular dementia.

The epidemic growth of dementia causes great concern for the society. It is customary to consider Alzheimer's disease (AD) as the most common cause of dementia, followed by vascular dementia (VaD). This dichotomous view of a neurodegenerative disease as opposed to brain damage caused by extrinsic factors led to separate lines of research in these two entities. Indeed, accumulated data suggest that the two disorders have additive effects and probably interact; however it is still unknown to what degree. Furthermore, epidemiological studies have shown "vascular" risk factors to be associated wit

[Depression and dementia: perspectives from clinical studies].

In this review, we present an overview of clinical studies that addressed the relationship between depression and dementia or cognitive decline. Cross-sectional studies and meta-analyses have repeatedly shown an association between late-life depression (LLD) and dementia, particularly Alzheimer's disease (AD) and vascular dementia; however, the findings of cohort studies have been inconsistent. Furthermore, studies on the association between depression with a younger age of onset and dementia have yielded inconsistent results. Regarding cognitive decline associated with LLD, several studies ha

Update on typical and atypical antipsychotic drugs.

Antipsychotic drugs (APDs) are best classified as typical or atypical. The distinction is based solely on their ability to cause extrapyramidal side effects (EPS), including tardive dyskinesia (TD). The two classes differ in mechanism of action, with atypical APDs providing important modulation of serotonergic neurotransmission. TD increases the death rate and can be minimized by limiting use of typical APDs. Clozapine is unique among the atypical APDs in its efficacy for ameliorating psychosis in patients with treatment-resistant schizophrenia (TRS), for reduction of suicide, and for improvin

Profile of cognitive impairment in Parkinson's disease.

Cognitive impairment (CI) is a common nonmotor complication of Parkinson's disease (PD), and is associated with significant disability for patients and burden for caregivers. Similar to motor symptoms, the characteristics of CI in PD can be quite variable, both in terms of what cognitive domains are impaired, and the timing of onset and rate of progression. This review will examine the profile of cognitive domain impairments observed in PD, with a focus on early CI (without dementia). We will also discuss possible relationships between specific cognitive domain impairments in PD and pathologic

The treatment of hyperhomocysteinemia.

The unique biochemical profile of homocysteine is characterized by chemical reactivity supporting a wide range of molecular effects and by a tendency to promote oxidant stress-induced cellular toxicity. Numerous epidemiological reports have established hyperhomocysteinemia as an independent risk factor for cardiovascular disease, cerebrovascular disease, dementia-type disorders, and osteoporosis-associated fractures. Although combined folic acid and B-vitamin therapy substantially reduces homocysteine levels, results from randomized placebo-controlled clinical trials testing the effect of vita

Functional imaging in mild cognitive impairment and early Alzheimer's disease: is it pertinent?

Neuroimaging techniques, namely positron emission tomography (PET), single photon emission computed tomography (SPECT), and magnetic resonance imaging (MRI) are increasingly used to study mild cognitive impairment (MCI) and its conversion to dementia, as well as early Alzheimer's disease (AD). Despite an important overlap of the various imaging parameter values between MCI,early AD and controls, some markers may help clinical diagnosis in individual patients. For example, the combination of significantly reduced hippocampal volume and brain hypometabolism in a MCI patient establishes the anatom

Defining dyslexia.

In 2007, the New Zealand Ministry of Education formally recognized the condition of dyslexia for the first time and has subsequently developed a working definition of the condition. The aim of this article is to draw on contemporary theory and research on reading development, reading difficulties, and reading intervention to describe what the authors believe are four key components of a definition of dyslexia/reading disability. They begin by discussing some preliminary factors that need to be considered in developing a definition of dyslexia. The authors then present the four components of the

[Cognitive remediation therapy in schizophrenia: a case report].

BACKGROUND: Despite progress in chemo-therapeutics, schizophrenia remains a chronic disease with occurrence of residual symptoms and drug resistance in 60% of the cases. Besides, cognitive impairment is frequent and highly correlated to social dysfunction seen in patients with schizophrenia. Several cognitive remediation programs have been elaborated. REHACOM is one of such programs. Aim of the study is to evaluate through a case control the efficiency of REHACOM towards cognitive functions. METHODS: This program has been administered to a patient suffering from undifferentiated schizophrenia

Neurobiological correlates of the cognitive side effects of electroconvulsive therapy.

Although electroconvulsive therapy (ECT) is a highly effective form of treatment, its use is limited by the emergence of cognitive side effects, notably anterograde and retrograde amnesia. Despite a large literature on the neurobiology of therapeutic mechanisms of ECT, very little is known about the neurobiological underpinnings of its cognitive effects. On theoretical grounds, structures within the medial temporal lobes, especially the hippocampus, are predicted to be critical regions mediating anterograde and, possibly, retrograde amnesia. However, functional neuroimaging studies in normal v

[Hypertension as a risk factor of dementia and cognitive decline in the elderly].

Management of dementia and cognitive decline is a major issue in geriatrics. Since the average age of society is advancing and patients of dementia are increasing, it is important to remove risk factors of dementia and cognitive decline in order to maintain quality of life in the elderly and to save cost of medicine and care. While hypertension has been known to be a risk factor of cerebrovascular events and vascular dementia, recent studies show that midlife hypertension is also a risk factor of cognitive decline and Alzheimer's disease in late life. Clinical trials and retrospective observat
