Bell's palsy is an acute unilateral weakness or paralysis of the face resulting from peripheral facial nerve dysfunction. While there is no readily identifiable cause, there is some recovery of function within 6 months. This article offers practical guidelines for diagnosing and treating Bell's palsy. These guidelines are based on the author's experience with 63 patients over the last 8 years. Physical examination should reveal diffuse nerve involvement, normal otoscopic findings, and no skin blebs or blisters and parotid masses. Other cranial nerve palsies may be present. Diagnostic testing s

Twenty-six fetuses with dilated urinary tracts were studied with serial sonograms. Eight fetuses with unilateral hydronephrosis were followed without intervention; all are well after postnatal surgical correction. Three cases of bilateral hydronephrosis resolved spontaneously before birth. Eight fetuses with bilateral hydronephrosis had evidence of poor function: Three were not treated and died shortly after birth with small lungs and dysplastic kidneys; three others had diagnostic intervention that demonstrated irreversible disease and allowed termination of the pregnancy; two had obstruction

BACKGROUND CONTEXT: Proper implant selection and placement in oblique lumbar intervertebral fusion (OLIF) are essential to achieve the best possible results for the patient. Key factors such as interbody cage length, height, angle, and material must all be carefully considered to achieve the intended results and minimize complications. Significant challenges remain in selecting the appropriate cage parameters to control spinal alignment while minimizing subsidence risk. Ongoing debates include how long a cage should be to optimize load distribution, as well as how variations in cage angle and

Necrotizing enterocolitis (NEC) is a deadly intestinal disease that most commonly occurs in premature infants. Current treatments are reactive rather than preventative, and include feeding cessation, antibiotics, gastric decompression, and supportive care. In severe NEC, surgical intervention is required for intestinal perforation or failure of medical management. Promising treatment modalities are being explored to prevent NEC and reduce NEC severity, including probiotics, probiotic delivery systems, growth factors, stem cell and small molecule therapeutics, human milk oligosaccharides, and h

We report the case of a 38-year-old female who developed persistent right upper extremity paresthesia, neuropathic pain, and ulnar-sided sensory disturbance following surgical decompression for thoracic outlet syndrome (TOS). Despite clinically significant cubital tunnel tenderness and ultrasound evidence of focal ulnar nerve impingement, electrodiagnostic studies, including nerve conduction studies (NCS) and electromyography (EMG), were normal. The patient underwent ultrasound-guided ulnar nerve hydrodissection with corticosteroid and dextrose solution as part of conservative management. This

A man in his 70s from a nursing facility with multiple comorbidities presented with acute onset nausea, vomiting and septic shock. CT abdomen revealed gastric pneumatosis consistent with emphysematous gastritis (EG). He was treated in the intensive care unit with broad-spectrum antibiotics, vasopressors and supportive care; oesophagogastroduodenoscopy showed extensive fundal ulceration with necrotic mucosa. Echocardiography demonstrated severe, reversible left ventricular systolic dysfunction consistent with septic cardiomyopathy. With conservative management (antibiotics, gastric decompressio

Patients with fall and head strike are commonly assessed in the emergency department (ED), and discharged into care of their primary care physicians (PCPs) if spinal cord injury is excluded clinically or radiologically. We present the case of 84-year-old man, who sustained a fall with head strike, and was cleared of spinal cord injury. He later presented to his PCP with progressive upper limb weakness, requiring urgent surgical decompression. PCPs are well-placed to catch such patients and prevent morbidity and mortality. We advocate clear communication between ED and the PCP to ensure continu

This video demonstrates our surgical approach for an entirely tubular trans-spinous process bilateral lumbar discectomy. A 47-year-old man presented with 12 months of severe predominantly right L5 radiculopathy. Magnetic resonance imaging revealed a large diffuse disc bulge at L4/5 resulting in bilateral lateral recess and central stenosis. He underwent bilateral discectomies via a trans-spinous process tubular approach. Postoperatively, his pain resolved and was discharged home the following day. Clinical resolution was sustained at 6 weeks. An entirely tubular trans-spinous process technique

Ureterocele is a rare congenital anomaly characterized by the dilation of the distal ureter and its protrusion into the bladder, often related with duplex collecting renal system, leading to urinary obstruction. While it commonly occurs in females, this case report presents the rare presentation of a massive ureterocele in a 2-day-old male neonate, causing urinary retention. A diagnostic workup revealed bilateral hydronephrosis and a massive ureterocele. Surgical intervention with endoscopic decompression of the ureterocele successfully alleviated the obstruction. At one year follow-up, patien

Calcium pyrophosphate dihydrate deposition (CPPD), or pseudogout, typically affects joints like the knee and shoulder but can also deposit in spinal structures, sometimes leading to myeloradiculopathy with severe neck pain and upper limb weakness. Mild cases are managed with anti-inflammatory drugs, while severe cases require surgical decompression. We report a rare case of pseudogout causing cervical spine myelopathy at the C1-2 level, discovered during spine surgery and confirmed by pathology. After removing the deposits, the patient showed significant improvement, emphasizing the need to co

Acute gastric volvulus presents with the classic triad of severe epigastric pain, vomiting, and difficulty or inability to pass a nasogastric tube. It can be complicated by strangulation and perforation which are associated with high mortality rates. Quick diagnosis and attempt at decompression is paramount. Computer-aided tomography is an important diagnostic step, and findings such as gastric wall thickening, extraluminal gas, and mediastinal fluid should prompt urgent surgical intervention. Approaches can include minimally invasive or open operations and in general should include reduction

Direct compression of the oculomotor nerve (OcN) is usually accompanied by OcN palsy, with an abnormal pupillary function. We herein present the case of a 76-year-old man with pupil-sparing paresis of the right inferior rectus muscle. A radiological examination revealed an epidermoid cyst at the right cerebellopontine angle, which came in contact with the right OcN. However, surgical intervention revealed compression of the inferomedial aspect of the OcN by the right posterior cerebral artery. Decompression of the nerve gradually improved the ocular movement. This unique case offers insights i

Pituitary apoplexy is a rare condition that occurs due to acute haemorrhage in the pituitary gland and usually presents with features of acute hypopituitarism, visual defects and ophthalmoplegia. Pregnancy is one of the risk factors for pituitary apoplexy. Medical management is the first line of management in pregnancy irrespective of the gestational age. However, acute neuro-ophthalmological deterioration might warrant an urgent surgical decompression. Preoperative corticosteroids have dual benefits of providing hormonal replacement in the mother and also promoting fetal lung maturity in the

A 75-year-old female patient was admitted following head trauma responsible for an acute-onset neurological worsening with a Glasgow Coma Scalescore of 6. Computed tomography scan revealed a large bifrontal meningioma with extralesional bleeding causing cranio-caudal transtentorial brain herniation. Despite emergency surgical excision of the tumor using craniotomy, the patient remained comatose. The brain magnetic resonance imaging revealed a Duret brainstem hemorrhage of the upper and middle pons, associated with supratentorial decompression-related brain injuries. One month later, the patien

Among all intramedullary spinal cord lesions, intramedullary spinal cord lipomas account less than 1%. Non-dysraphic intramedullary lipoma is very rare. It is most commonly seen in cervicodorsal region followed by cervico bulbar, lumbar and sometimes multiple. Here we present a 17-year-old female who underwent MRI due to upper dorsal pain followed by progressive bilateral lower limb weakness which showed intramedullary lesion extending from T1-T9, involving eight vertebral segments with distal syrinx and features suggestive of lipoma. Patient underwent surgical decompression of lipoma. Patient

Spinal gunshot injuries are one of the most serious injuries that can cause morbidity and mortality. We report a case of a nine-year-old boy, referred to our emergency department from another hospital, with lower limb weakness after being shot by an air gun in his back. After extensive investigation, it appeared that a foreign body representing the bullet was found in his spinal canal; the patient was taken to the operating room for surgical decompression and removal of foreign bodies. After that, the patient started to show partial improvement in his neurological deficits and is to be continu

Intramuscular choristomatous cystic lesions within the orbit are rare but may require removal due to their volume effects. The authors describe 2 such cases of intramuscular cysts in adult patients. Both patients presented with symptoms due to local mass effects with swelling, proptosis and diplopia. Imaging confirmed cystic intramuscular masses. Surgical removal involved intraoperative decompression of the cyst to aid dissection with excision of some muscle fibers. Histology confirmed a keratinized stratified squamous lined dermoid cyst within lateral rectus for case 1 and stratified squamous

Ductus arteriosus aneurysm (DAA) is rarely encountered in adults. There have been several hypotheses regarding its origin and potential indications for intervention in asymptomatic cases. If left untreated, rupture, compression of surrounding organs, and serious complications due to thromboembolism may occur, and aggressive surgical intervention appears desirable for patients who can tolerate surgery. We report a case involving a 30-mm, saccular, patent DAA that was incidentally discovered in a 49-year-old man on computed tomography. Open repair was performed by femorofemoral bypass assistance

Venous thoracic outlet syndrome (TOS) is uncommon but occurs in young, healthy patients, typically presenting as subclavian vein (SCV) effort thrombosis. Venous TOS arises through chronic repetitive compression injury of the SCV in the costoclavicular space with progressive venous scarring, focal stenosis, and eventual thrombosis. Diagnosis is evident on clinical presentation with sudden spontaneous upper extremity swelling and cyanotic discoloration. Initial treatment includes anticoagulation, venography, and pharmacomechanical thrombolysis. Surgical management using paraclavicular decompress

Spontaneous spinal epidural abscess is rare entity in neonates. These are surgical emergency in which early diagnosis and prompt decompression is necessary to avoid permanent cord damage. The diagnosis is based on clinical findings of paraplegia supported by radiological findings on an MRI. We found a large extra spinal abscess in an infant that on further evaluation showed a communicating epidural component, yet the baby was neurologically intact. The abscess was drained in emergency with clearance of epidural component and appropriate antibiotics instituted for Streptococcus pyogenes as per
