Fulminant development of pulmonary arteriovenous fistulas in an infant after total cavopulmonary shunt.

An infant with cyanotic congenital heart disease and polysplenia syndrome developed profound cyanosis within months of undergoing bilateral cavopulmonary anastomoses. Intrapulmonary shunting was diagnosed by contrast echocardiography with peripheral venous and selective pulmonary artery injection. Histopathology revealed abnormal, thin-walled vessels within the interstitium of the lung lobule. These vessels have not been reported previously and are likely to be the anatomic site of arteriovenous shunting. This case demonstrates that pulmonary arteriovenous fistulas (PAVFs) may develop rapidly

Immunohistochemical identification of syncytiotrophoblastic cells and megakaryocytes in pulmonary vessels in a fatal case of amniotic fluid embolism.

The histological diagnosis of amniotic fluid embolism (AFE) is based on finding amniotic fluid components in the pulmonary microvasculature. In addition to the distinctive constituents of AFE, placental and decidual tissue fragments as well as isolated trophoblastic cells and megakaryocytes are potentially detectable within pulmonary vessels. The identification of single syncytiotrophoblastic cells (STC), and their differentiation from circulating megakaryocytes (MK) within the lumen of small and medium-sized pulmonary vessels is difficult by classical morphological methods. In a fatal case of

Multiple sclerosis with hydromyelia demonstrated at autopsy.

Multiple sclerosis (MS) associated with intramedullary cysts has been recently documented radiographically in approximately a dozen cases, but histological confirmation in these cases has been lacking. These cysts have been noted to occur centrally in the spinal cord and have been previously diagnosed as syringomyelia. We report a patient with extensive hydromyelia, but no dissecting syringomyelic cavity, of the thoraco-lumbar spinal cord, which was co-existent with centrally located, non-necrotic, chronic demyelinative plaques. A few well demarcated non-necrotic plaques were seen in the cereb

The risk of over-reporting spinal manipulative therapy-induced injuries: a description of some cases that failed to burden the statistics.

OBJECTIVE: To describe some cases of severe or fatal symptoms that might be attributed to spinal manipulative therapy and one case that illustrates the importance of witnesses when a patient dies under unusual circumstances. CLINICAL FEATURES: Three of these cases were fatal (two strokes and one heart attack) and one was potentially lethal (a bleeding basilar aneurysm). Another patient developed paresis of one arm and leg (cervical disc prolapse). A case of lower limb paresis and incontinence is also described (cauda equina). INTERVENTION: Because of chance, none of these patients actually r

Creutzfeldt-Jakob disease in Oman: report of two cases.

Sporadic Creutzfeldt-Jakob disease (CJD) was diagnosed in two Oman Arab men, aged 50 and 75 years respectively, both with a history of rapidly developing dementia and myoclonic jerks. Illness developed over a period of 3 months in the first case and over six months in the second. Electroencephalography in both subjects showed periodic triphasic sharp waves characteristic of CJD. In neither case was it possible to obtain a brain biopsy or perform autopsy (autopsy is contrary to Islamic practice in the Middle East), however, electrophoresis of cerebrospinal fluid from the second patient revealed

Pulmonary cryptosporidiosis in the acquired immunodeficiency syndrome.

Since the beginning of the AIDS epidemic, Cryptosporidium has emerged as a pathogen responsible for diarrhea in humans. Cryptosporidiosis confined to the respiratory tract has been documented only rarely in humans. An HIV-infected patient is described here, who developed pulmonary and intestinal cryptosporidiosis. Lung involvement was proven by biopsy, which also revealed bronchiolitis but no other pathogens. The patient died of respiratory failure 2 months after the onset of respiratory symptoms.

[Off-label use of lincomycin hydrochloride in 2 horses. Dysbacteriosis and fatal complications due to inadequate symptomatic therapy].

A lawyer inquired about the possible harmful effects of 'off-label use' of lincomycin in two trotting horses. From information in the relevant dossier it could be concluded that there was no direct indication to use antibiotics. In addition, mistakes were made in the medicinal treatment of horses, namely, the off-label use of lincomycin without prior consultation with the manufacturer, fluid and electrolyte replacement therapy not continued for long enough, and incorrect use of antipyretic analgesics. The intravenous administration of gentamicin to dehydrated patients is permissible only in co

[Leiomyosarcoma metastatizing to the sphenoid bone presenting exophthalmos: a case report].

A rare subcutaneous leiomyosarcoma metastatizing to the sphenoid bone and presenting exophthalmos is reported. A 56-year-old female presented with protrusion of the right eye and a slowly growing lump on the right temporal region. Six years previously, she had undergone removal of a subcutaneous mass in the back, which was histologically diagnosed in another hospital as leiomyosarcoma. She had undergone four other operations, including removal of local recurrences and a right renal metastasis. On admission, physical examination showed no neurological deficits. Craniogram revealed an osteolytic

Cavernous angioma of brain stem mimicking multiple sclerosis.

A 14-year-old boy was admitted to our Department due to peripheral palsy of right VII and bilateral of the VI cranial nerves, spasticity, cerebellar symptoms as well as to dysphagia and dysarthria. In general, he was hospitalized 13 times because of the disease of a relapsing-remitting and next progressive course. He died 31 years after onset of the disease. Multiple sclerosis was diagnosed. Brain autopsy revealed tumor involving almost all brain stem structures and a part of right cerebellar hemisphere. Histologically, cavernous angioma was diagnosed.

Fatal pontine hemorrhage after podiatric surgery. A case report.

The authors present a case of massive fatal pontine hemorrhage as a complication of hypertension in a patient treated for an infected diabetic ulcer. The podiatric physician must be aware of the risks associated with concomitant medical problems such as hypertension and ensure that proper therapeutic measures are taken to avoid the potential for catastrophic complications.

[Multicentric Castleman's disease with lymphoid interstitial pneumonia died of aggressive course with adult respiratory distress syndrome].

A 49-year-old man was admitted to our hospital with anemia and hypergammaglobulinemia. Physical examination revealed superficial lymph node swelling and no hepatosplenomegaly. Laboratory findings showed WBC 5,300/microliters with normal hemogram, microcytic and hypochromic anemia. Total protein was 11.5 g/dl and immunoglobulinemia (IgG 10,100 mg/dl, IgA 295 mg/dl, IgM 160 mg/dl) was observed without M-component in serum and urine. The CD4/CD8 ratio of lymphocyte subsets was 0.58 and the tuberuculin skin test was negative. Urinary protein was positive and renal biopsy disclosed plasma cell infi

Spontaneous intracranial haemorrhage is one of the cerebrovascular complications in beta-thalassaemia major patients. This is a report of 2 cases of fatal intracranial haemorrhage. Their ages were 12 and 7 years respectively, and they had been receiving regular blood transfusion for the past 3 and 2 years respectively. They developed acute onset of headache, loss of consciousness and convulsions at 5 and 2 days respectively after their last blood transfusion. C-T scan of the brain showed massive intracranial haemorrhage with extension into the ventricles. The spontaneous intracranial haemorrha
