Effect of Microfracture on Meniscal Tear Healing in a Goat (Capra hircus) Model.

Meniscal injuries are an extremely common cause of knee pain. Meniscal repairs performed with concomitant anterior cruciate ligament reconstruction appear to heal at a higher rate than meniscal repairs performed in isolation. This may be due in part to the release of marrow elements into the knee and the time of meniscal repair. In cases of isolated meniscal repair, some orthopedic surgeons use microfracture to release marrow elements into the joint as an adjunct to enhance meniscal healing. This study evaluated rates of meniscal tear healing with or without the performance of microfracture in

Opening wedge high tibial osteotomy: an operative technique and rehabilitation program to decrease complications and promote early union and function.

BACKGROUND: High tibial osteotomy has been associated with significant complications, including delayed union or nonunion, loss of correction, arthrofibrosis, and patella infera. HYPOTHESES: A technique for opening wedge osteotomy that incorporates an autogenous iliac crest bone graft will prevent delayed union or nonunion, allow early rehabilitation and weightbearing, and prevent knee arthrofibrosis and patella infera. Secondly, the authors' methods for calculating the desired correction of valgus alignment prevent undesired alterations in tibial slope. STUDY DESIGN: Case series; Level of e

Muscular coactivation. The role of the antagonist musculature in maintaining knee stability.

The objective of this study was to quantify the coactivation patterns of the knee flexor and extensor muscles as part of continued efforts t … erties of the antagonist muscle in response to exercise. It was also concluded that reduced risk of knee injuries in high performance athletes with muscular imbalance could result from complementary resistive exercise of the antagonist muscle.

Diagnosis of acute fractures of the extremities: comparison of low-field MRI and conventional radiography.

The aim of this study was to compare low-field MRI (0.2 T) and conventional radiography for the detection of acute fractures of the distal part of the extremities. X-ray and MRI examinations of 78 (41 fractures, 37 without fracture) patients with the clinical suspicion of an acute fracture in the distal part of the extremities were compared. Four experienced radiologists, two for each of the two modalities, independently analyzed the images. Interobserver variability and receiver operating characteristic (ROC) analysis for both methods were established. The MRI and conventional radiography rev

Overgrowth After Pediatric Tibial Spine Repair with Symptomatic Leg Length Discrepancy: A Case Report.

CASE: A 12-year-old girl sustained a right-sided tibial spine fracture while jumping on a trampoline. Postoperative course was complicated initially by arthrofibrosis requiring manipulation under anesthesia and subsequent leg length discrepancy attributed to posttraumatic overgrowth necessitating femoral epiphysiodesis. Ten years after initial injury, she reported her knee to be 63% of normal and an International Knee Documentation Committee score of 63.2. Symptomatic overgrowth requiring epiphysiodesis after tibial spine repair has not been previously reported to our knowledge. CONCLUSION: T

Patellar tendon reconstruction with semitendinosus-gracilis autograft.

We present a case of a 24-year-old, otherwise healthy, man who sustained a right knee injury after a fall. A small, comminuted inferior pole patella fracture with medial and lateral retinacular tears was encountered that required a small, nonarticular partial patellectomy and patellar tendon repair. An uneventful postoperative course was complicated by a fall onto a flexed knee and rerupture of the patellar tendon at 3 months following surgery. Intraoperatively, a significant tissue void was seen in the area of the prior repair. The patellar tendon was reconstructed with semitendinosus and gra

Development of an accurate three-dimensional finite element knee model.

This paper presents the development of a detailed articulating three-dimensional finite-element model of the human knee, derived from MRI scan images. The model utilises precise material models and many contact interfaces in order to produce a realistic kinematic response. The behaviour of the model was examined within two fields of biomechanical simulations: general life and car-crash. These simulations were performed with the non-linear explicit dynamic code PAM-SAFE trade mark. The knee model produced results that compared favourably with existing literature. Such a model (together with oth

Incidence of injury in elite Gaelic footballers.

The purpose of this study was to undertake a comprehensive prospective epidemiological study of injuries sustained by elite Gaelic Football players over one season. The pattern of injury is strikingly similar across all teams with 47% of all injuries occurring in the final quarter of games and training. Injuries to the lower limb, particularly the hamstrings muscles accounted for the majority of injuries. 65% of players were unable to participate fully in Gaelic Football activity for between one and three weeks as a result of injury. The high incidence of injury especially hamstrings injuries

Survivorship, complications, and outcomes following distal femoral replacement for neoplastic indications.

BACKGROUND: Distal femoral replacements (DFRs) are excellent treatment options for limb salvage procedures in patients who have bone loss secondary to neoplasm. Multiple studies report adequate survivorship and complication rates following DFR implantation, primarily for non-neoplastic indications. However, current literature regarding neoplasm-specific reports is often limited by sample size, survivorship, and patient reported outcome measurements. Therefore, we sought to examine patients who received a DFR for a neoplastic indication at multiple tertiary academic centers. Specific outcomes a

Factors Affecting Burn Contracture Outcome in Developing Countries: A Review of 2506 Patients.

INTRODUCTION: Burn contractures hinder joint mobility, resulting in functional impairment and reduced quality of life. This is of greater significance in developing countries where there are fewer resources for assistance with such impairments. Contracture release reduces deformity, but multiple factors affect the extent of postsurgical improvements and outcomes. Elucidating these factors may enable surgeons to better care for burn patients. This study assesses factors that impact burn contracture resolution in developing nations. METHODS: A retrospective review of 2506 burn contractures was

Internal Brace Augmentation Versus Standard Anterior Cruciate Ligament Reconstruction: A Comparative Clinical Study in Young Active Patients.

BACKGROUND AND AIMS: Anterior cruciate ligament (ACL) rupture is a common injury in young, active individuals, and arthroscopic ACL reconstruction (ACLR) is the standard of care. Internal brace augmentation has been proposed to enhance graft protection and potentially facilitate earlier return to sport, but comparative clinical data remain limited. This study aimed to compare early functional outcomes, knee stability, return to sports, and complications between internal brace augmentation ACLR and standard ACLR in patients with ACL rupture. METHODS: This prospective comparative study was cond
