We report herein the case of a 23-year-old man with Crohn's disease who was found to have a perforated small bowel following blunt abdominal trauma sustained in a traffic accident. The general findings of diffuse peritonitis were identified by physical examination, and a plain X-ray film showed free air in the abdominal cavity. An emergency laparotomy was performed which revealed three perforated ulcers in the affected intestine. An abrupt increase in intraluminal pressure due to the striking force of the steering wheel to the abdomen was assumed to have been the cause of these perforations.

A rare but dramatic case of haemoperitoneum of non-gynaecological origin which occurred during pregnancy is reported. Obstetrical causes of shock having been excluded, laparotomy made it possible to discover an aneurysm of the splenic artery and treat it by resection-ligation and splenectomy. In such cases, the uterus is spared before the 6th month of pregnancy, and beyond that data caesarean section is mandatory. The pathogenesis of such arterial ruptures during pregnancy is unknown. Gastric lesions in the progression of syphilis are very rare and they have several forms of presentation resembling lesions of different nature. We describe a case of secondary syphilis in an heterosexual patient, with persistent gastric ulcers and hepatic affection, in which the evolution and the observations with laparotomy suggested neoplasia. This study was undertaken to analyze the factors associated with fertility outcome following tubal pregnancy. Eighty-six patients were analyzed in this regard. The surgical treatment of complete rectal prolapse has been the object of great controversy. Currently, it is believed that best results are obtained by transabdominal rectopexy. Formation of a fistula from a Meckel's diverticulum to the bladder is extremely rare and may not be recognized, especially in patients with other gastrointestinal disease. We describe a patient with Crohn's ileitis who was assumed to have two enterovesical fistulae from his diseased ileum. Laparotomy revealed a Meckel's diverticulum with fistulization to the bladder. The diverticulum contained inflamed and ulcerated ectopic gastric mucosa but was not affected by Crohn's disease. The authors have performed an analysis of results of 242 emergent laparoscopies fulfilled in patients aged from 15 to 89 years. No acute surgical diseases were found in 103 cases. In 56 patients there were chronic diseases of organs of the abdominal cavity, 47 patients had no such pathology. A 4-year-old well boy was seen because of an asymptomatic left testicle undescended since birth; the testis was not palpable and the right side was normal. After an episode of left lower quadrant and left hip area pain, with some bladder symptoms and left leg limping, a work-up including a CAT scan showed a calcified retroperitoneal pelvic tumor on the left side. At laparotomy, an infarcted mass was found in the pelvis just above the internal ring. It was a torsion of an undescended intraabdominal testis with a benign testicular teratoma. The tumor was removed and his recovery was uneventful.

A 28 year old man presented with haematuria and renal failure. Renal arteriography revealed bleeding into the pelvis of a solitary right kidney from a hilar artery. The use of silastic perianal rings to control rectal prolapse in the elderly has been reviewed. Forty-one patients were treated over a 6 year period by a total of 52 operations. Their mean age was 80 years and there was no operative mortality. Prolapse was adequately controlled in 71 per cent of patients, and a further three patients had no recurrence despite failure and removal of the ring. The results of this procedure are inferior to those obtained by abdominal operations for rectal prolapse, but deserve serious consideration in those elderly patients who may be unfit for laparotomy.

Nine cases of acute pancreatitis due to parasites in the common bile duct (Ascaris lumbricoides 4, Clonorchis sinensis 5) are reported. Eight cases were discovered at laparotomy and one at postmortem. The indications for operation were worsening pancreatitis, sepsis or cholangitis. Decompression of the biliary system and removal of parasites resulted in the recovery of the eight cases operated upon. A coliky and depressed four year old Arabian mare was examined three days after replacement of a rectal prolapse that had occurred during parturition. Fresh blood was present in the rectum and a septic peritonitis was diagnosed based upon the clinical pathological examination of the peritoneal fluid.
