Osteoporosis after spinal cord injury.

Immobilisation secondary to spinal cord injury (SCI) is associated with marked and rapid atrophy of trabecular bone. The purpose of this study was to evaluate bone mineral density (BMD) in both the upper and lower extremities following SCI sustained for various lengths of time and to correlate the BMD to the level of the lesion, time from injury, spasticity and serum calcium, phosphorus and alkaline phosphatase (ALP) levels. A study was undertaken in 41 SCI patients with a mean age of 35.8 +/- 12.7 years. A significant difference in BMD between upper and lower extremities of the paraplegics we

Plasticity and function--the fate of a free, neurovascular muscle graft ten years post-reconstruction.

A 16-year-old female sustained a subtotal amputation of the left thigh. Debridement resulted in a bone and soft-tissue defect of 20 cm in length. The whole quadriceps muscle was lost, and the knee joint was open. The femur was stabilized by transfer of corticocancellous bone grafts. A latissimus dorsi muscle was harvested and transferred to reconstruct the lost quadriceps muscle. The thoracodorsal nerve was coaptated to the motor branch of the femoral nerve. The years after trauma, the muscle provides excellent motor function. EMG evaluation reveals no sign of denervation; macro-electromyograp

Effect of maximal arm exercise on skin blood flux in the paralyzed lower limbs in persons with spinal cord injury.

The purpose of the present study was to examine the effect of maximal arm exercise on the skin blood circulation of the paralyzed lower limbs in persons with spinal cord injury (PSCI). Eight male PSCI with complete lesions located between T3 and L1 performed graded maximal arm-cranking exercise (MACE) to exhaustion. The skin blood flux at the thigh (SBFT) and that at the calf (SBFC) were monitored using laser-Doppler flowmeter at rest and for 15 s immediately after the MACE. The subject's mean peak oxygen uptake and peak heart rate was 1.41 +/- 0.22 l.min-1 and 171.6 +/- 19.2 beats.min-1, resp

Multiple relaxing skin incisions in orthopaedic lower extremity trauma.

The purpose of this study was to evaluate the efficacy of using multiple relaxing skin incisions (MRSIs) to facilitate the closure of difficult lower extremity wounds. Such wounds are caused by direct trauma or by surgical intervention for management of bone and soft tissue injury that result in wound closure under tension. Common alternatives include closure by secondary intention, delayed primary closure, split thickness skin grafting, or flap coverage. The technique of MRSI involves numerous small (5-10 mm) incisions through the dermis arranged in 1-cm-wide rows parallel to the primary woun

The benefits of plaster casting for lower-extremity burns after grafting in children.

Plaster casts are frequently used for immobilization of extremities after grafting in our burn center. A retrospective study was undertaken to evaluate the efficacy of this technique versus posterior splinting in children with burns below the knee. Thirty-six of 165 patients who required skin grafts from October 1986 to October 1989 received skin grafts to the lower extremity below the knee. Twenty patients (56%) were casted and 16 (44%) were not. Various parameters were analyzed in order to compare the casted versus noncasted groups. Several statistical techniques were used to compare the two

Risk factors for the development of stress fractures during fluoride therapy for osteoporosis.

We attempted to identify risk factors for the development of lower limb stress fractures during fluoride therapy for osteoporosis (OP). We compared 18 patients who developed 41 such fractures (26 periarticular, 6 femoral neck, 5 long bone shaft, 1 greater trochanter and 3 pubic rami fractures) during fluoride therapy, with 24 similarly treated patients who did not develop stress fractures. Treatment consisted of sodium fluoride 0.99 mg/kg per day, elemental calcium 1 g/day, and vitamin D. We obtained a previous fracture history, annual radiographs of the spine (fractures), hands (metacarpal co

Pulmonary emboli in burned patients.

The incidence of and prophylaxis against clinically important pulmonary emboli (PE) in burned patients is an often discussed problem. To study its magnitude, all patients admitted with acute burns were followed for clinical evidence of thromboembolism: 2,106 patients were evaluated (1,439 adults). No children had evidence of PE. Six adults (0.4%) sustained a PE: none died. Mean burn size was 26.8%; mean age, 35.1 years; and mean weight, 90.8 kg. Mean PBD of the PE was 16.5 days. None with PE were ICU patients and none had lower-extremity IV lines. Two patients had embolic events after discharg

[Limb-saving surgery of the lower extremities in children].

In 21 children with severe injuries of the lower extremities observed within 10 years, limb saving surgery was attempted. The evaluation of an injury is determined by the severity of the soft tissue damage rather than by the type of a fracture. A new classification is suggested for this purpose. Therapeutic clues are an open treatment of the wound with repeated irrigations and debridements. Bone fractures are stabilised and vascular separations repaired. Definitive care of skin, soft tissues and bones is undertaken only within weeks following the injury and in good circulatory conditions. All

Bony and soft tissue reconstruction and rehabilitation following traumatic hemipelvectomy, exsanguination and cardiac arrest.

Survival following traumatic hemipelvectomy in the child with subsequent local flap coverage of wounds has been previously described. The use of axial skin flaps and thigh and rectus abdominis musculocutaneous flaps for coverage of hemipelvectomy defects following tumor extirpation has also been noted in the literature. The use of muscle flaps to salvage otherwise nonvital bone and to eradicate osteomyelitis with subsequent wound closure has also been thoroughly demonstrated both clinically and experimentally. The use of musculocutaneous flaps to revascularize otherwise nonviable bone of the p

[Effectiveness of various therapeutic schemes for patients with radiation edema of the extremities].

The efficacy of various therapeutic schemes: medicinal (basic therapy--BT), acupuncture (AP) and laser therapy (LT) against a background of basic therapy--was assessed and compared in 36 patients with radiation limb edema. It was established that a degree of a decrease in edemas, the improvement of indices of rheovasography grew in the following order: BT----AP----LT. The recovery of the lymph flow and immunological indices were the same in all therapeutic schemes.

Spasms in spinal cord injury: a study of 72 subjects.

Seventy-two patients with spinal cord injury of at least two years duration were reviewed retrospectively to determine incidence and effects of spasms. The average time between the injury and the examination was ten years. Seventy patients experienced spasms. In 50 patients (72%) the spasms appeared within six months after the injury. Historically, the spasms reached their maximal severity within 12 months. No significant difference was found in intensity and frequency of spasms between patients with clinically complete or incomplete spinal cord injury. Extensor spasms were seen more frequentl

MRI of extracranial hematomas: preliminary observations.

Thirteen patients (16 examinations) with extracranial hematomas (four mediastinum, three pelvis, two calf, two psoas muscle, one liver, one abdominal wall) were examined with magnetic resonance imaging (MRI). With the exception of an acute hematoma (less than 48 hr), which did not have a distinctive MRI appearance, subacute and chronic hematomas (up to 10 months' duration) had areas of high signal intensity on both T1- (TR 500/TE 30) and T2- (TR 1500/TE 90) weighted pulse sequences. The hematomas in 10 of the patients were also evaluated by computed tomography (CT). The MRI findings complement

[Vascular and capsule-band reconstruction following knee joint dislocation].

This is a report about bilateral dislocations of the knee joints in one patient with arterial and venous stop because of endothelial lesions and thrombosis of the popliteal vessels on both sides. After reconstruction of circulation by means of vena saphena, the disrupted knee ligaments were also sutured. Large ventral exposure of the knee joint is recommended for both reconstruction of vessels and ligaments.

The vastus lateralis muscle flap: technique and applications.

The vastus lateralis muscle can provide a broad sheet of muscle for repair of defects of the lower abdomen, groin, perineum, hip, and ischium. The authors limit the use of the vastus lateralis muscle to situations in which more traditional techniques are not feasible. This muscle is especially suitable for defects involving irradiated tissue, infected bone, or infected prosthetic material. Although the vastus lateralis muscle has no cutaneous component, it may be used to augment an overlying tensor fascia lata flap.

Marlex mesh in gas gangrene.

Clostridial gas gangrene is a well recognized complication of traumatic and surgical wounds, and is associated with an overall mortality rate of 25% (5, 22). Gas gangrene of a limb results in a mortality rate approximately half that of gas gangrene of the trunk (4, 7, 8, 9, 11, 12, 13, 15, 16, 19, 24). Radical debridement and antibiotic therapy or high amputation of involved limbs are accepted traditional approaches to the problem. The role and value of hyperbaric oxygenation (OHP) remains controversial despite intense study over the past few decades. Patients with gas gangrene involving all l

Pulmonary aspects of treatment of long bone fractures in the polytrauma patient.

During the past decade, there have been significant advances in the treatment of long bone fractures in the polytraumatized patient. The major controversy in this area is whether definitive long bone stabilization needs to be done emergently. In general, definitive fixation should be done early in the patient's hospital course after it is determined that the patient is hemodynamically stable and is resuscitated adequately. The major benefits of fixation are: (1). improve mobilization to enhance pulmonary function; (2). decreased narcotic requirements with improved pain control after fixation;

Psychosocial and physical factors associated with lower extremity injury.

With the increasing availability of modern occupant restraints, more drivers and passengers are surviving high-energy crashes. However, a large number, especially those involved in frontal and offset frontal crashes, incur disabling lower extremity injuries. In the past, not much attention was paid to these injuries, as they were usually not life threatening. Despite the low AIS scores associated with injuries to the lower extremities, they pose a major physical and psychological burden on patients' and their ability to return to pre-crash functioning. Associated injuries, such as mild brain i

Vascular Steal Phenomenon in Lower Extremity Reconstruction: A Review of Literature and Case Report.

INTRODUCTION: Free tissue transfer is a well-described tool for complex lower extremity reconstruction in areas of local tissue paucity. Successful coverage of orthopedic hardware or exposed vital structures create opportunities for limb salvage. METHODS: A retrospective chart review was performed of a patient sustaining a gunshot wound to the lower extremity with bony and soft tissue loss who underwent orthopedic fixation and free tissue transfer at the authors' institution with resultant distal extremity necrosis consistent with vascular steal phenomenon.
