Ultrasonography of Hodgkin's disease in the liver and spleen.

Ultrasound findings of 39 spleens and 68 livers in which histology was obtained at laparotomy or autopsy within a month of the examination and four livers in which abnormalities were found clinically by other methods during long-term follow-up were reviewed to evaluate the various sonographic appearances and the accuracy in detecting involvement of the spleen and liver. Only one out of 17 positive spleens had definite ultrasonic focal lesions. Not only small splenic Hodgkin's foci but also larger ones were overlooked. Of 10 positive livers, six showed diffuse inhomogeneity throughout the liver…

[Changes Of Liver And Diaphragm In Experimental Paragonimiasis].

Changes of abdominal organs in early paragonimiasis are described in dogs and cats. Experimental infection was induced by feeding 40~50 and 20~30 metacercariae of Paragonimus westermani to 15 dogs and 15 cats respectively. These animals were killed sequentially starting from 8 days to 120 days after infection. Complete autopsies were carried out in all animals and the tissues were examined microscopically with the main emphasis on changes of the liver and the diaphragm. A definite bout of acute diffuse fibrinoexudative peritonitis with ascites ranging from 5 to 20 ml was present in both dogs a…

Anatomical features of liver in situs inversus.

The hepatic veins and the intrahepatic branches of both the portal vein and the hepatic artery have been dissected and studied in three specimens of liver obtained from bodies in situs inversus. The position of the hepatic veins was found generally to correspond to the intersegmental scissure planes of the liver. The intrahepatic branches of the portal vein were found to delineate the subdivision of the liver into lobes, segments and subsegments which correspond to the 'mirror image' of the normal liver. The arterial irrigation derives from the mirror image constituting peculiar patterns in ea…

Spontaneous rupture of the liver during pregnancy.

Spontaneous rupture of the liver, a rare but grave complication of pregnancy merits attention of an obstetrician. It is important to stress that proper antenatal care, prompt hospitalization with effective treatment at first alarming sign of preeclampsia and early induction of labour when medically indicated may prevent such a grave complication. The aim of this paper is to promote earlier diagnosis with massive fluid and blood replacement in order to decrease the mortality. It also forms the research bases for liver function tests in all those patients with essential hypertension or persisten…

[Partial resection of the liver in the treatment of echinococcosis].

On the basis of reports' reviewed and authors' experience, in 118 patients with hydatid disease of the liver, 15 of whom (12%) have been treated by partial resection of the liver, it can be concluded that: - Partial resections of the liver represents a radical surgical procedure in the treatment of liver hydatidosis, - A surgical treatment of the liver hydatid disease requires a total extirpations of echinococcus cysts, as well as irreversibly damaged liver tissue itself, - These resections' methods should be carried out in definite indications, - In comparison with other, especially more cons…

Hydatid cyst of the liver with intrathoracic rupture.

In the last 10 years we have operated on 33 cases of hydatid cyst of the liver with intrathoracic rupture. Twenty-one out of 33 cases ruptured into bronchi, in seven the rupture affected the pleural cavity, and in six simultaneous rupture into the bronchus and pleural cavity occurred. Laboratory tests were not carried out in all cases. The Casoni intradermal test was carried out in 23 cases and was positive in 18. A liver scan was performed in 10 and was positive in all. Conservative operations were carried out in 22 patients. In these the hepatic cavity was evacuated and separately drained. T…

[Acute hepatic insufficiency and the artificial liver].

The authors reviews some aspects related to hepatic encephalopathia and liver tissue regeneration, as well as criteria for prognosis of evolution and lethality of hepatic failure of the acute type. Methods of hepatic assistance include peritoneal dialysis, hemodialysis, exsanguinotransfusion, plasmapheresis, perfusion of isolated liver, crossed circulation, total washing of the organism, liver transplantation and artificial liver. The principles, the techniques and the materials used for the construction of an artificial liver are presented. Such materials include active charcoal, resins, enzy…

Liver lesions in reactional states in leprosy.

Liver biopsy was done in 21 adult patients of leprosy in acute reaction. They included 10 of lepromatous leprosy, seven of tuberculoid leprosy and four of dimorphic leprosy. Except for a single case of tuberculoid leprosy all showed granulomatous lesions of varying sizes and frequency, in the liver. The lesions appeared to be unique for reaction and immunologically-based. The sequence of events appeared to be: (1) Immunologically-mediated alteration of Kupffer cells with adhesion of lymphocytes and polymorphounuclear leucocytes (2) Small granulomas of proliferating Kupffer cells, lymphocytes a…

Simultaneous amebic liver abscess and hepatitis A.

A child with amebic colitis, liver abscess and hepatitis A is reported. Speculation as to why these two infectious agents have rarely been associated is presented. Diagnosis of hepatitis was not suspected in this case until physicians caring for the patient developed clinical hepatitis. The importance of suspecting hepatitis A in all patients with unexplained liver enzyme elevations is stressed.

Leiomyosarcoma metastatic to the liver: CT appearance.

Leiomyosarcoma is an uncommon malignancy that metastasizes to the liver and lungs. Each of the three cases of leiomyosarcoma metastatic to the liver examined by computed tomography demonstrated similar morphologic and dynamic characteristics. Common features included hepatomegaly and large, circumscribed "cannonball" lesions that distorted normal hempatic architecture. These larger metastases were sharply defined with central necrosis and were accentuated by contrast enhancement of normal surrounding liver. Smaller perfused metastases often became isodense following contrast medium administrat…

Pancreatic-polypeptide-producing apudoma of the liver.

A large hepatic excised from a 62-year-old man resembled a carcinoid tumor. No endocrine syndrome was present and an alternative primary source of tumor was found, despite an intensive search. The tumor cells were classified according to the type of electron-dense granules present: ACTH, gastrin (G), bombesin (P), and pancreatic polypeptide (PP) type granules were identified. Immunofluorescent staining with antibodies to several polypeptide hormones detected PP-containing tumor cells only. This rare tumor is therefore a PP-apudoma; possibly of intrahepatic bile duct origin.

The pathology of the liver in porphyria cutanea tarda.

We report the findings in 53 biopsies from 45 patients with porphyria cutanea tarda (PCT). Red autofluorescence and birefringent acicular cytoplasmic inclusions were constant findings in all untreated cases. Autofluorescence occurs in other hepatic porphyrias, but acicular inclusions appear to be specific for PCT; we have seen them in subclinical porphyria and before development of cutaneous symptoms. They are probably uroporphyrins and they trend to disappear during rinsing by water during most staining procedures. We recommend unstained paraffin sections for their demonstration. Liver damage…

Itch in liver disease: facts and speculations.

Pruritus in hepatobiliary disease is commonly believed to be caused by retention of bile acids with their sequestration in the skin. HOwever, we have recently demonstrated that skin levels of bile acids in patients with cholestasis correlate poorly with pruritus. In this report, we present additional data concerning the relationship of pruritus to bile acid retention: (1) the urinary excretion of sulfated and nonsulfated bile acids was not significantly different in patients with cholestasis who itched compared to those who did not; (2) one patient with itch associated with a liver abscess had…
