Medication adherence, utilization of healthcare services, and mortality of patients with epilepsy on opiate replacement therapy: A retrospective cohort study.

INTRODUCTION: Substance misuse is not uncommonly recognized in people with epilepsy (PWE). Mortality is significantly greater in those with comorbid substance misuse, but it remains unclear whether epilepsy care and management contribute to this. This cohort study aimed to compare the rates of mortality in PWE receiving opiate replacement therapy (ORT) and PWE alone, as well as evaluate their medication adherence, levels of engagement with epilepsy services as currently delivered, and utilization of unscheduled hospital care. MATERIAL AND METHODS: A 5-year historical cohort for PWE was identi

The side effect profile of Clozapine in real world data of three large mental health hospitals.

OBJECTIVE: Mining the data contained within Electronic Health Records (EHRs) can potentially generate a greater understanding of medication effects in the real world, complementing what we know from Randomised control trials (RCTs). We Propose a text mining approach to detect adverse events and medication episodes from the clinical text to enhance our understanding of adverse effects related to Clozapine, the most effective antipsychotic drug for the management of treatment-resistant schizophrenia, but underutilised due to concerns over its side effects. MATERIAL AND METHODS: We used data fro

Efficacy and safety of direct switch to indacaterol/glycopyrronium in patients with moderate COPD: the CRYSTAL open-label randomised trial.

BACKGROUND: Dual bronchodilation combining a long-acting β2-agonist (LABA) and a long-acting muscarinic antagonist (LAMA) is the preferred choice of treatment recommended by the Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 guidelines for the management of patients with moderate-to-severe chronic obstructive pulmonary disease (COPD). The once-daily (q.d.) fixed-dose combination (FDC) of LABA, indacaterol 110 μg and LAMA, glycopyrronium 50 μg (IND/GLY 110/50 μg q.d.) demonstrated superior improvements in lung function, dyspnoea and overall health status and better tolerabil

Outcomes of aortic stenosis in patients with cardiac amyloidosis: A systematic review and meta-analysis.

BACKGROUND: Cardiac amyloidosis (CA) results from the deposition of abnormally folded protein fibrils, leading to restrictive cardiomyopathy, valvular heart disease, and arrhythmias. Up to 15 % of patients with severe aortic stenosis (AS) have concomitant CA (AS-CA). We conducted this systematic review and meta-analysis to compare medical management, transcatheter aortic valve replacement (TAVR), and surgical AVR (SAVR) in AS-CA. METHODS: A comprehensive literature search was conducted for relevant studies from inception through January 20, 2024. Studies exploring outcomes in adult AS patient

Continuation of Buprenorphine During Hospitalization and Subsequent Retention in Therapy: an Observational Study in Veterans Administration Hospitals.

BACKGROUND: Inpatient hospitalization has the potential to disrupt buprenorphine therapy. OBJECTIVE: Among patients receiving outpatient buprenorphine prior to admission, we determined the rate of discontinuation during medical and surgical admissions to VA hospitals and its association with subsequent post-discharge continuation of buprenorphine therapy. DESIGN AND MAIN MEASURES: We conducted an observational study using Veterans Administration data from 10/1/2018 to 3/31/2020 for all medical and surgical admissions where Veterans had active buprenorphine prescriptions at the time of admiss

Management of Venous Malformations.

Venous malformations (VMs) are slow-flow vascular anomalies arising from hyperactivation of the TIE2-PI3K-AKT-mTOR pathway. This comprehensive review delineates the diverse clinical landscape of VMs, ranging from sporadic lesions to complex hereditary and syndromic phenotypes. We compile clinical characteristics of all VM subtypes. We detail diagnostic protocols and assessment for localized intravascular coagulopathy. Management requires a multidisciplinary approach, integrating conservative measures, sclerotherapy, surgery, and targeted medical therapies to optimize symptomatic relief and pat

The value of adrenal venous sampling in unveiling the conundrum of double unilateral adrenal adenomas in a case of primary hyperaldosteronism.

Primary hyperaldosteronism (PHA) is a cause of secondary hypertension, often due to adrenal adenoma. The diagnosis and management of multiple unilateral adrenal adenomas in PHA represent a unique challenge. Adrenal venous sampling (AVS) has become a pivotal diagnostic tool in these cases, providing critical information for accurate localization of aldosterone production. Here, we describe a rare case of a 52-year-old female with a history of resistant essential hypertension who presented with multiple unilateral adrenal adenomas concomitant with adrenal hyperplasia in a case of primary hyperal

Nonsurgical Management of a Post-Traumatic Left Ventricular Septal Aneurysm in a 13-Year-Old Male.

CASE SUMMARY: A 13-year-old previously healthy boy developed a left ventricular septal aneurysm following blunt chest trauma, presenting with electrocardiographic changes and elevated troponin. Echocardiography demonstrated an apical septal aneurysm without ventricular septal defect and with preserved systolic function. Cardiac magnetic resonance imaging revealed near-transmural late gadolinium enhancement. He was managed conservatively with enalapril, propranolol, and aspirin and remained clinically stable with preserved function. TAKE-HOME MESSAGE: Traumatic left ventricular aneurysms witho

Retinal and choroidal detachment following corneal wasp sting: a case report and literature review.

PURPOSE: Ocular injuries caused by wasp stings are rare but potentially devastating. This report describes a case of severe retinal and choroidal detachment secondary to corneal wasp envenomation, with a focus on pathogenesis and management challenges. CASE PRESENTATION: A 63-year-old female sustained a left corneal wasp sting, progressing to corneal edema, hypopyon, retinal-choroidal detachment, and eventual light perception loss. Despite medical therapy (topical corticosteroids, antibiotics), the patient declined surgical intervention, ultimately resulting in evisceration. CONCLUSION: Wasp

Cutaneous T-cell lymphoma: Consensus on diagnosis and management in Taiwan.

Cutaneous T cell lymphomas (CTCLs), with mycosis fungoides (MF) and Sézary syndrome (SS) as the classic types, are the commonest group of primary cutaneous lymphomas. The diverse clinical manifestation and non-specific histologic findings of early lesions in CTCLs render diagnosis challenging. Treatment modalities also vary and include topical and oral medications, chemotherapy, phototherapy, and radiation therapies. Local dermatological, hemato-oncologic and radiotherapeutical experts in Taiwan convened meetings in 2023 to review and discuss the latest evidence and updates regarding diagnosis

Understanding pediatric cervicofacial non-tuberculous mycobacterial infection.

Cervicofacial non-tuberculous mycobacterial infection should be a part of the differential diagnosis for immunocompetent children ages 1 to 5 years who present with painless submandibular or preauricular lymphadenopathy. Although a benign and self-limiting disease, patients can develop a chronically draining fistula if not diagnosed and treated promptly. The diagnostic process can be managed with a combination of microbiological studies, cytology, laboratory tests, and imaging studies. Surgical excision is the most definitive curative measure. Incision and curettage, medical management with an

Risks and safety of biologics: A practical guide for allergists.

Biologic agents are a rapidly expanding class of medications, and several options are now available for the management of allergic and immunologic disorders. The risks of biologic therapy need to be understood in order to adequately counsel patients and appropriately monitor for potential adverse events. We sought to provide a comprehensive review of the risks and adverse effects reported for the current FDA-approved biologics used in management of allergic and immunologic disorders, including omalizumab, benralizumab, dupilumab, mepolizumab, reslizumab, tezepelumab and tralokinumab. Occurrence of syphilis in the young patient who coinfected with human immunodeficiency virus: A dangerous combination case report.

Several complications can occur if syphilis is not treated at an early stage. Several developing countries recently remerged the high cases of syphilis with human immunodeficiency. We reported the case of 26-years-old male infected with syphilis with the human immunodeficiency virus (HIV). The patient sole and palm contain lesions. In our patient, HIV was diagnosed 2 years earlier in the prophylactic studies but had not taken any medical management. The penicillin G was taken to the patient to reverse the lesions, and it was successfully treated. The patient also was on antiretroviral therapy

Bruton's tyrosine kinase Inhibitors and Cardiotoxicity: More Than Just Atrial Fibrillation.

PURPOSE OF REVIEW: The purpose of this review is to summarize the epidemiology, mechanisms, and management of cardiovascular complications of Bruton's Tyrosine Kinase inhibitors (BTKIs). RECENT FINDINGS: Ibrutinib increases the risk of atrial fibrillation, bleeding, and hypertension compared with non-BTKI therapies. The evidence to support an association between ibrutinib and other cardiovascular complications including ventricular tachyarrhythmias or cardiomyopathy is limited. Ibrutinib metabolism can be inhibited by some medications used to treat cardiovascular complications. The cardiovasc

Coronary artery fistulae.

Coronary artery fistulae (CAF) are rare congenital or acquired in which a connection forms between one of the coronary arteries and a heart chamber or with other vessels. This paper describes three cases of CAF along with their initial presentation, imaging findings and management. The first case is a rare form of CAF in which the left circumflex coronary artery fistula empty into left ventricle. We discuss the different types of CAF along with their prevalence and the different imaging tools that could be utilized to identify CAF. There is no unifying consensus on treatment strategy for sympt

Caval Thrombus Management: The Data, Where We Are, and How It Is Done.

Thromboses of the superior and inferior vena cava, either isolated or associated with distal deep venous thrombosis, are uncommon, but confer potentially serious morbidity and mortality. Incidence is increasing, especially with the prominence of intravascular devices. The range of treatment options is also expanding to include medical management, surgery, and endovascular techniques which are now frequently considered first line therapy due to lower reintervention rates and decreased periprocedural morbidity. Currently, there are no official guidelines for screening or treatment. This article

Asymmetric glaucoma in pseudoplateau iris syndrome.

A 39-year-old Caucasian man with bilateral narrow angles, a plateau-like iris configuration on gonioscopy and elevated intraocular pressure (IOP) presented with significant asymmetric glaucoma, left eye affected more than right. Initial management with topical medical therapy, laser iridoplasty and peripheral iridotomy in the left eye was ineffective in lowering the IOP or opening the anterior chamber angle. Ultrasound biomicroscopy demonstrated bilateral ciliary body cysts. The patient ultimately required surgical management, consisting of cataract extraction and endoscopic cyclophotocoagulat

Medical management of blepharitis.

PURPOSE OF REVIEW: Blepharitis is one of the most common ocular pathologies encountered in the clinical setting. Despite its prevalence, successful treatment is often difficult. The purpose of this review is to provide an update on the medical management of blepharitis. RECENT FINDINGS: The available treatment options for blepharitis have expanded rapidly in recent years. Eyelid hygiene remains the foundation of most treatment regimens, but the addition of topical and oral antibiotics, steroids, and calcineurin inhibitors is showing promising results. Dietary considerations and interventional

Complex regional pain syndrome of the upper extremity.

The diagnosis and management of complex regional pain syndrome is often challenging. Early diagnosis and intervention improve outcomes in most patients; however, some patients will progress regardless of intervention. Multidisciplinary management facilitates care in complex cases. The onset of signs and symptoms may be obvious or insidious; temporal delay is a frequent occurrence. Difficulty sleeping, pain unresponsive to narcotics, swelling, stiffness, and hypersensitivity are harbingers of onset. Multimodal treatment with hand therapy, sympatholytic drugs, and stress loading may be augmented

The use of anti-tuberculosis therapy for latent TB infection.

Tuberculosis infection is of global public health significance, with millions of incident cases each year. Many cases, particularly in low-prevalence settings, result from the reactivation of latent tuberculosis infection (LTBI); potentially acquired years prior to active disease. Up to one-third of the world's population has been infected with LTBI, and so may be at risk for future active TB disease. A variety of antituberculosis medications and treatment regimens have now been evaluated in the management of LTBI, with the aim of eradicating tuberculosis bacilli and reducing the likelihood of
