Anomalous Putamen Volume in Children With Complex Motor Stereotypies.

BACKGROUND: Complex motor stereotypies in children are repetitive rhythmic movements that have a predictable pattern and location, seem purposeful, but serve no obvious function, tend to be prolonged, and stop with distraction, e.g., arm or hand flapping, waving. They occur in both "primary" (otherwise typically developing) and secondary conditions. These movements are best defined as habitual behaviors and therefore pathophysiologically hypothesized to reside in premotor to posterior putamen circuits. This study sought to clarify the underlying neurobiologic abnormality in children with prima

Corporeal reflexivity and autism.

Ethnographic video recordings of high functioning children with autism or Aspergers Syndrome in everyday social encounters evidence their first person perspectives. High quality visual and audio data allow detailed analysis of children's bodies and talk as loci of reflexivity. Corporeal reflexivity involves displays of awareness of one's body as an experiencing subject and a physical object accessible to the gaze of others. Gaze, demeanor, actions, and sotto voce commentaries on unfolding situations indicate a range of moment-by-moment reflexive responses to social situations. Autism is associ

Pseudodyskinesis--a novel mechanism for abnormal motion of the heart's diaphragmatic wall.

We report a case of pseudodyskinesis, where there is dyssynchronous contraction of the heart's diaphragmatic wall despite normal wall thickening. This finding has previously been reported in a small group of patients with liver disease, and has been attributed to elevation of the diaphragm as a result of hepatomegaly and ascites. Our case demonstrates similar findings in a patient without liver disease, in whom the diaphragm was elevated secondary to volume loss in the chest. Our case supports the assertion that diaphragmatic elevation, regardless of cause, is indeed responsible for this proba

Limb shaking transient ischemic attack--an unusual presentation of carotid occlusive disease. A case report and review of the literature.

Limb shaking transient ischemic attack (TIA) is an uncommon presentation of carotid occlusive disease. This unusual form of TIA is not well recognized and may be mistaken for focal epilepsy, delaying proper diagnosis and treatment. In this communication, we present such a case, together with a review of the literature, a brief account of pathophysiology and an outline of appropriate clinical management.

EMDR and hypnosis in the treatment of phobias. Eye Movement Desensitization and Reprocessing.

Clinical hypnosis and EMDR have both been employed in the treatment of phobias. EMDR has been a controversial treatment method with the research showing mixed results concerning its efficacy. Many studies have shown the effectiveness of hypnosis in the treatment of phobias, but no studies have directly compared hypnosis to EMDR. This paper discusses each approach to treatment, with special emphasis on EMDR. Relevant research and current theories are reviewed along with questions raised and recommendations for future research.

Aggression and the termination of "rituals": a new variant of the escape function for challenging behavior?

Aggression and stereotyped behaviors are not uncommon among people with intellectual disabilities and they are often treated separately as operant behaviors. In this single case study, it is argued that the function of a young woman's aggressive behavior appeared to be that of avoiding or escaping the termination of a chain of complex stereotyped behavior (or "ritual"). She became aggressive only when this chain of stereotyped behavior was terminated and the aggression appeared to extinguish when it no longer led to escape from the termination of the "ritual". It is suggested that this is an e

The effects of the saddle seat on seated postural control and upper-extremity movement in children with cerebral palsy.

A repeated-measures experimental cross-over study was conducted with school-aged children with spastic cerebral palsy (spastic CP) to compare the effects of flat-bench versus saddle-bench seating on postural control and reaching motions. The saddle bench allowed significantly better postural control as measured by the clinical rating scale Sitting Assessment for Children with Neuromotor Dysfunction and by a study-defined variable, Spinal Extension. No statistically significant differences were found for any other variables using aggregate data-analysis techniques. However, the group results an

Gross motor disability and head growth in Rett syndrome--a preliminary report.

Growth of head circumference was measured in 99 RS girls and compared to gross motor function. The degree of head growth deceleration correlated to the severity of gross motor disability at 12 years of age.

Restrictive ophthalmopathy associated with linear scleroderma.

A patient with a coup de sabre lesion of the forehead developed progressive ipsilateral limitation of ocular motility, primarily involving adduction and depression. Investigation disclosed no other explanation for the ocular motility disturbance, which we suspect represents restrictive myopathy maximally involving ocular muscles immediately subjacent to the area of linear scleroderma.

[The influence of sleep on abnormal waking movements].

Sleep decreases considerably the abnormal movements seen during waking in the degenerative extra-pyramidal diseases. However, the electrophysiologic recordings reveal that muscular contractions are not completely abolished. This decrease parallels the reduction of the waking system which act likely as a non-specific system of amplification. One can notice that sleep modifies the characteristics of the parkinsonian tremor and that some palatal myoclonias persist during sleep.

Botulinum toxin treatment of supranuclear ocular motility disorders.

We treated one patient with bilateral internuclear ophthalmoplegia and another with skew deviation with extraocular muscle botulinum toxin injection. Both patients had pre-injection symptomatic diplopia in primary position, one for 1 month and the other for 12 months. Resolution of diplopia and complete and permanent binocular fusion in primary position was obtained within 3 to 4 days in both cases.

The oculocerebral syndrome in association with generalised hypopigmentation. A case report.

A 14-year-old girl with generalised hypopigmentation, mental retardation, abnormal movements, and ocular anomalies is described. It is suggested that she represents a further case of oculocerebral albinism, a rare autosomal recessive condition. Reference is made to previous similar cases.

Imbalanced adaptation of accommodation and vergence produces opposite extremes of the AC/A and CA/C ratios.

It is customary to describe abnormal interactions between accommodation and convergence according to the Duane-White classification of convergence excess and insufficiency or divergence excess and insufficiency. However, recent studies indicate that these extremely high and low categories of accommodative vergence may result from adaptive disorders of accommodation and convergence. The AC/A ratio is shown in this paper to vary inversely with adaptability of accommodation and to vary directly with adaptability of vergence. Extremely high AC/A ratios can result from an imbalance in which vergenc

Normal-pressure hydrocephalus. Onset of gait abnormality before dementia predicts good surgical outcome.

In 1977, Fisher reported that in patients with possible normal-pressure hydrocephalus (NPH), if the gait abnormality preceded dementia, surgery usually had a favorable outcome and vice versa. We studied this finding in 21 patients shunted for possible NPH. By evaluating serial videotapes of gait, neuropsychological tests, and Katz index ratings, preoperatively and at approximately two months and six months postoperatively, we judged 16 patients improved. In the improved group, the families reported that the gait abnormality preceded the dementia in 11 patients and occurred at the same time in

Divergence insufficiency: characteristics, diagnosis, and treatment.

Although the entity of divergence insufficiency was described as early as 1886 by Duane, it has received relatively little attention in the literature. Patients presenting with a greater eso deviation at distance than at near, a concomitant deviation, and diplopia pose a challenging and critical diagnostic and therapeutic puzzle for the clinician. Divergence insufficiency, a benign condition, must be differentiated from divergence paralysis and sixth nerve palsy, two conditions that present with somewhat similar findings yet have underlying etiologies of a serious nature. This paper reviews th
