The problem of unilateral and bilateral neck dissection.

270 unilateral and bilateral neck resections have been performed in different primary localisations of tumours in the head and the neck at the Ear, Nose and Throat Department of the Medical Faculty in Zagreb in the period 1960-70. Of 209 patients with unilateral resection, only 78 survived (37%), while only 3 patients (4.9%) survived of a total of 61 patients where bilateral resection was performed. An analysis is given separately of radical resection in carcinoma of the larynx, the hypopharynx and the skin. From this analysis and from immunologic considerations, the following conclusions are

Real-world study of antiresorptive-related osteonecrosis of jaw based on the US food and drug administration adverse event reporting system database.

Objective: This study aims to explore the risk signals of osteonecrosis of the jaw induced by antiresorptive drugs and provide references for the clinical safety application. Method: According to the FDA's Adverse Event Reporting System (FAERS), from January 2004 to September 2021, we chose "Osteonecrosis of the jaw (10064658)" and "Exposed bone in jaw (10071014)" as preferred terms, "antiresorptive drugs" as the target drugs, and primary suspect drug as the drug role code in the dataset. We evaluated the association between drugs and adverse events by using reporting odds ratio (ROR) based on

Lymphoepithelial Carcinoma of Salivary Gland EBV-association in Endemic versus Non-Endemic Patients: A Report of 16 Cases.

Lymphoepithelial carcinoma of salivary glands (LECSG) are rare neoplasms, reported in endemic populations (southeastern Chinese) with a strong Epstein-Barr virus (EBV) association. A retrospective series comparing EBV status within an ethnically diverse population (endemic vs. non-endemic patients) has not been reported. Sixteen LECSG were equally distributed between males (n = 8) and females (n = 8) with a median age of 54 years (range 18 to 85 years) at initial diagnosis. Ten patients were white, 4 Asian, and 2 black. The patients typically presented with swelling or mass for an average of 1

SATB2 Expression Distinguishes Ovarian Metastases of Colorectal and Appendiceal Origin From Primary Ovarian Tumors of Mucinous or Endometrioid Type.

The primary origin of some ovarian mucinous tumors may be challenging to determine, because some metastases of extraovarian origin may exhibit gross, microscopic, and immunohistochemical features that are shared by some primary ovarian mucinous tumors. Metastases of primary colorectal, appendiceal, gastric, pancreatic, and endocervical adenocarcinomas may simulate primary ovarian mucinous cystadenoma, mucinous borderline tumor, or mucinous adenocarcinoma. Recently, immunohistochemical expression of SATB2, a transcriptional regulator involved in osteoblastic and neuronal differentiation, has be

Adenocarcinoma ex-goblet cell carcinoid (appendiceal-type crypt cell adenocarcinoma) is a morphologically distinct entity with highly aggressive behavior and frequent association with peritoneal/intra-abdominal dissemination: an analysis of 77 cases.

High-grade versions of appendiceal goblet cell carcinoids ('adenocarcinoma ex-goblet cell carcinoids') are poorly characterized. We herein document 77 examples. Tumors occurred predominantly in females (74%), mean age 55 years (29-84), most with disseminated abdominal (77% peritoneal, 58% gynecologic tract involvement) and stage IV (65%) disease. Many presented to gynecologic oncologists, and nine had a working diagnosis of ovarian carcinoma. Metastases to liver (n=3) and lung (n=1) were uncommon and none arose in adenomatous lesions. Tumors had various histologic patterns, in variable combina

Clinicopathologic characteristics of 29 invasive carcinomas arising in 178 pancreatic mucinous cystic neoplasms with ovarian-type stroma: implications for management and prognosis.

Information on the clinicopathologic characteristics of invasive carcinomas arising from mucinous cystic neoplasms (MCNs) is limited, because in many early studies they were lumped and analyzed together with noninvasive MCNs. Even more importantly, many of the largest prior studies did not require ovarian-type stroma (OTS) for diagnosis. We analyzed 178 MCNs, all strictly defined by the presence of OTS, 98% of which occurred in perimenopausal women (mean age, 47 y) and arose in the distal pancreas. Twenty-nine (16%) patients had associated invasive carcinoma, and all were female with a mean ag

Carcinoma of the collecting ducts of Bellini and renal medullary carcinoma: clinicopathologic analysis of 52 cases of rare aggressive subtypes of renal cell carcinoma with a focus on their interrelationship.

Carcinoma of the collecting ducts of Bellini and renal medullary carcinoma are rare aggressive neoplasms of putative distal nephron origin. First described in 1949, case reports and review articles constitute a major source of information on collecting duct carcinoma, whereas Davis and colleagues and the pediatric tumor registry have contributed the seminal works on renal medullary carcinoma. Here we present a detailed study of collecting duct carcinoma (n=39) and renal medullary carcinoma (n=13), characterizing these rare neoplasms and analyzing their interrelationship. Both collecting duct c

Implications of Heterotypic Cell Fusion in Cancer.

The interplay between discrete neoplastic cells and cells within the tumor microenvironment represents a critical component of cancer evolution that ultimately impacts patient outcomes. A newly identified neoplastic cell that is a product of cell fusion between neoplastic and immune cells has emerged as a key player in metastatic spread. Indeed, these hybrid cells are detected across the metastatic cascade: within the primary tumor, disseminated into the peripheral blood, and in metastatic sites. While multiple mechanisms of hybrid cells generation have been posited, cell fusion has the most r

Metastatic Trajectories in Non-Small Cell Lung Cancer Guide Local and Systemic Therapies.

Advances in systemic therapy have improved outcomes for metastatic non-small cell lung cancer (NSCLC), yet resistance and progression remain nearly universal. Local therapies such as radiotherapy, surgery, and image-guided ablation can extend disease control in selected patients, but existing classifications-including dynamic models of oligometastatic disease-assign a single state per patient and do not capture lesion-level heterogeneity. We introduce the concept of metastatic trajectories-the spatiotemporal dynamics of response and progression across lesions, organs, and patients-as a framewo

The critical intersection of survivorship and palliative care in people with metastatic breast cancer.

Metastatic breast cancer (MBC) remains incurable, but as survival improves, individuals are living longer and experience complex, evolving needs that span medical, psychosocial, and existential domains. These needs are often inadequately addressed by existing care models. Survivorship programs frequently exclude those with incurable disease, while palliative care is commonly delayed due to stigma, misperceptions, and systemic barriers. This paper explores the critical intersection of survivorship and palliative care in the context of MBC, advocating for an integrated, person-centered model tha

Metastatic breast cancer outcome in a real-world setting: analysis for long-term survival and curability in a prospective dataset.

BACKGROUND: Treatment advances in metastatic breast cancer (MBC) have led to prolongation in overall survival (OS), nevertheless MBC is considered an incurable disease. Real-world data provides an opportunity to assess outcomes in an unselected patient population receiving standard-of-care therapy and assess for potential curability. METHOD: Patient, tumor and treatment characteristics collected prospectively were analyzed. Univariate and multivariate analyses were performed for characteristics associated with OS ≥10-years. Time-to-next treatment (TTNT) and OS were compared to published clini

Best therapeutic approach in metastatic hormone-sensitive prostate cancer based on disease volume: a systematic review and network meta-analysis.

CONTEXT: With new treatment strategies approved in metastatic hormone-sensitive prostate cancer (mHSPC), heterogeneity across trials hinders the physicians' choice for first-line treatment. OBJECTIVE: We conducted a systematic review and network meta-analysis to assess the efficacy of currently approved treatments for mHSPC stratifying patients according to their disease burden (high- vs. low-volume as per CHAARTED criteria) and onset of metastatic disease (synchronous vs. metachronous). INTERVENTION: Eleven randomized controlled trials (RCTs) published until October 30, 2024 were included.

Cell chirality in cancer: Emerging roles in malignant progression and tumor metastasis.

Cell chirality is a newly discovered cellular characteristic that is phenotype-specific and is associated with cell migration, cell differentiation, organelle positioning, and organ asymmetry. The maintenance of normal cellular chirality is fundamental for preserving physiological functions in organisms. In recent years, cellular chirality has been increasingly investigated in tumor research, especially metastasis. During tumor metastasis, tumor cells acquire migratory and invasive capabilities through epithelial-mesenchymal transition (EMT), a process that is intricately linked to the dynamic

Key Updates on the Version 9 AJCC/UICC Staging System for Salivary Gland Carcinoma.

BACKGROUND: Multiple studies have identified limitations in the nodal (N) category definitions of the eighth-edition tumor-node-metastasis classifi cation (TNM8) for major salivary gland carcinoma (SGC). Minor SGCs have traditionally been staged according to site of origin despite distinct biology and patterns of spread, and the feasibility of a unified staging system for both major and minor SGCs had not been systematically evaluated. These shortcomings prompted a comprehensive reassessment of SGC staging. METHOD: A multidisciplinary international expert panel, in collaboration with the Amer

Distinct molecular and clinical aggressiveness in very early-onset metastatic colorectal cancer: survival and genomic divergence between patients aged 30-39 versus 40-49 years.

BACKGROUND: Early-onset colorectal cancer (EOCRC) is increasing worldwide and exhibits clinical heterogeneity. Patients younger than 40 years may constitute a biologically distinct subgroup within EOCRC. We investigated whether 'very early-onset' metastatic colorectal cancer (VEOCRC, ages 30-39) exhibits specific clinical and genomic features compared with EOCRC in patients aged 40-49 and whether these differences lead to variations in survival from the time of metastatic diagnosis. MATERIALS AND METHODS: We analysed the data of metastatic EOCRC patients in a multi-institutional database, div
