Polymethylmethacrylate membrane with a series of serendipity.

Forty years have passed since the polymethylmethacrylate (PMMA) membrane was first developed. This article reviews its history and explains its longevity. The membrane was developed through application of a stereocomplex phenomenon that is observed upon mixture of isotactic and syndiotactic PMMA polymers. Filtryzer(TM) B1 and B2 were approved in Japan in 1977. B1 was the pioneer high-performance membrane model and B2 was a model that simulated a low-flux cellulosic membrane. The development of B1 led to the development of the dialysis machine with an ultrafiltration rate (UFR)-controlling func

Fabry disease: results of the first UK hemodialysis screening study.

BACKGROUND: Fabry disease is an X-linked lysosomal storage disorder in which deficiency of α-Galactosidase A (α-Gal A), leads to accumulation of glycosphingolipids in the vascular endothelium, kidneys and heart. Males with classical disease present in childhood, however some individuals with low levels of α-Gal A activity present atypically with adult onset renal impairment. Screening studies in patients with established end-stage renal failure (ESRF) suggest that up to 1.5% of patients have sub-normal α-Gal A levels. We used the dried blood spot (DBS) enzyme activity test to screen for undiag

Improvement of hypertension and LVH in maintenance hemodialysis patients treated with sustained-release isosorbide mononitrate.

BACKGROUND: Hypertension and left ventricular hypertrophy (LVH) are independent predictors of morbidity and mortality in maintenance hemodialysis (MHD) patients. The level of serum asymmetric dimethylarginine (ADMA) is increased in MHD patients, which is a kind of uremic toxin. It can competitively inhibit nitric oxide production from L-arginine, which can increase vascular resistance, then lead to endothelium dysfunction, hypertension, LVH and decrease of ejection fraction. A sustained-release preparation of isosorbide mononitrate can lead to vasodilatation by releasing nitrogen monoxide. But

Treatment disparities in acute coronary syndromes, heart failure, and kidney disease.

It has been consistently observed that patients with renal dysfunction have more premature, severe, complicated, and fatal cardiovascular disease than age- and sex-matched individuals with normal renal function. There have been 4 major explanations for this finding: (1) positive confounding by third variables associated with chronic kidney disease (CKD), including diabetes mellitus and hypertension; (2) therapeutic nihilism or lesser use of beneficial therapies in CKD; (3) greater toxicities of therapies, such as bleeding from anticoagulants or contrast-induced kidney injury; (4) biological fa

Endothelin antagonism in patients with resistant hypertension and hypertension nephropathy.

Resistant hypertension is a failure to achieve a blood pressure (BP) goal of < 140/90 mm Hg despite treatment with at least three different antihypertensive medications classes at a maximally tolerated dose and including a diuretic. The most important systems that require alteration include the renin-angiotensin-aldosterone system, sympathetic nervous system, and (more recently) the endothelin (ET) system. To date, several clinical trials have assessed the effects of ET antagonism, both selective and nonselective, on BP control in humans. The nonselective ET antagonist bosentan was evaluated i

New insights into the role of calcium-sensing receptor activation.

The discovery of the calcium-sensing receptor (CaSR) prompted the identification of substances that affect its function. Cinacalcet, for example, is a drug that allosterically modifies the receptor so as to increase its sensitivity to circulating calcium (thus the name "calcimimetic") and in this way decreases parathyroid hormone secretion. Clinical use of cinacalcet is already approved for the treatment of primary and secondary hyperparathyroidism, but research is ongoing to identify further potential actions of this drug. The effects and functions of the CaSR have been evaluated in different

A study of maintenance therapy after intravenous maxacalcitol for secondary hyperparathyroidism.

AIM: Intravenous vitamin D therapy is an established treatment for secondary hyperparathyroidism (SHPT). However, no protocols have been established for maintenance therapy with intravenous or oral vitamin D after control of intact parathyroid hormone (iPTH) within the target range. METHODS: Step I. For patients with SHPT (200 ≤ iPTH ≤ 500 pg/ml), a dose of 2.5 mg maxacalcitol (OCT) was administered intravenously three times a week with oral sevelamer hydrochloride; the dose was increased to a 10 µg maximum three times a week to control iPTH to < 150 pg/ml. Step II. When iPTH reached the targ

Choice of modality with the use of high-performance membrane and evaluation for clinical effects.

The golden target for dialysis therapy should be to guarantee longer survival and to give a higher quality of life without dialysis-related complications. In order to achieve this target, the choice of dialysis modality and membrane is essential but we have not yet established what the best choice for a dialysis modality and membrane are. Generally, we choose a dialysis modality for better solute removal and better biocompatibility. In this issue we would like to propose that the patients' preference for dialysis therapy is a useful parameter in prescribing the dialysis modality. In our recent

No influence of haemodialysis on interferon production in the QuantiFERON-TB Gold-In-Tube test.

BACKGROUND: Immunodeficiency in end-stage renal disease (ESRD) can be aggravated by haemodialysis (HD). This results in an increased incidence of reactivation of tuberculosis (TB) in HD patients. The tuberculin skin test to detect a latent TB infection (LTBI) has its limitations in these patients because of a high rate of false negative results due to anergy of T cells. Data on the influence of HD on the performance of interferon-gamma release assays are limited. The aim of this study was to determine the effect of HD on the performance of the QuantiFERON-TB Gold (QFT-G) assay in ESRD patients

Membranous nephropathy.

The understanding of cellular and molecular mechanisms involved in the pathogenesis of membranous nephropathy (MN) has come from studies carried out in the Heymann nephritis model of MN in the rat, which closely resembles the clinical and pathologic features of the human disease. MN involves the in situ formation of subepithelial immune deposits as a result of circulating antibodies binding to podocyte antigens. Complement activation leads to assembly of C5b-9 on podocyte plasma membranes, and results in sublethal podocyte injury and proteinuria. The podocyte responds to sublethal C5b-9 attack

Focal and segmental glomerulosclerosis: multiple pathways are involved.

Focal segmental glomerulosclerosis (FSGS) is not a disease but a clinicopathologic entity. The term FSGS itself is a misnomer because its lesions are not always focal, segmental, or sclerotic. Its clinical expression also widely varies and is nonspecific. Confronted with such diversity, one cannot but translate the title of this contribution into a unifying version focusing on the podocyte, initial culprit, or victim of multiple processes leading to FSGS. Some have been identified in human glomerulopathies and/or in animal or cell culture models, and are classified as secondary. Genetic forms,

The 50th anniversary of long-term hemodialysis: University of Washington Hospital, March 9th, 1960.

The first 6 months of 1960 saw the development of the shunt that first made long-term hemodialysis possible for patients dying from chronic kidney failure. A brief account of hemodialysis for acute kidney failure prior to 1960 is followed by a description of the work of Belding Scribner, Wayne Quinton and David Dillard at the University of Washington in Seattle. Scribner had the idea of a shunt connecting indwelling arterial and venous cannulas in the forearm between dialyses, to maintain patency of the cannulas, Quinton used Teflon tubing to make the device, and Dillard was the surgeon who im

Ferumoxytol: a new era of iron deficiency anemia treatment for patients with chronic kidney disease.

Ferumoxytol is a new product approved for intravenous use by the US Food and Drug Administration (FDA) in the treatment of iron deficiency anemia in adults with chronic kidney disease. This approval was based on data from 3 open-label, randomized, controlled clinical trials. In all of these trials, ferumoxytol was well tolerated, and hemoglobin levels were significantly increased compared with those achieved by orally administered iron. Ferumoxytol, a superparamagnetic iron oxide coated with a carbohydrate shell, is also used as a magnetic resonance imaging (MRI) agent due to its magnetic prop

Simultaneous clinical resolution of focal segmental glomerulosclerosis associated with chronic lymphocytic leukaemia treated with fludarabine, cyclophosphamide and rituximab.

BACKGROUND: Although renal involvement in advanced haematological malignancies is common, glomerulonephritis associated with lymphoproliferative disorders is rare, and the related pathogenetic mechanisms are still poorly understood. We present a rare case of chronic lymphocytic leukaemia(CLL)-associated focal segmental glomerulosclerosis with nephrotic-range proteinuria. CASE PRESENTATION: A 53-year-old Caucasian man, previously healthy, with no history of hypertension, alcohol use or smoking presented with rapid weight gain, massive peripheral oedema, and hypertension. Models of preventable disease: contrast-induced nephropathy and cardiac surgery-associated acute kidney injury.

In order to prevent a disease, its temporal nature (or at least when it starts) needs to be clearly defined. In acute kidney injury (AKI), this is usually not possible because the current diagnostic criteria are retrospective. Contrast-induced nephropathy (CIN) and cardiac surgery-associated acute kidney injury (CSA-AKI) are both thought of as potentially preventable acute renal lesions because the timing of the insult is known precisely. While this is true, knowledge of the exact biological processes that give rise to each condition is lacking, although there are several common themes - notab

Glomerulonephritis emerging shortly after Puumala hantavirus infection: a report on 7 patients.

AIMS: Nephropathia epidemica (NE) is mild type of hemorrhagic fever caused by Puumala (PUU) hantavirus. Renal biopsy typically shows acute tubulointerstitial nephritis and complete recovery is the usual outcome. We previously described 5 patients with membranoproliferative glomerulonephritis (MPGN) after acute NE. We now report on 7 more patients who developed biopsy-confirmed glomerulonephritis (GN) during the convalescent phase of NE. MATERIAL AND METHODS: We present case histories of 7 patients with nephrotic-range proteinuria concomitant with hematuria after serologically verified NE. RE

Vitamin D receptor activation: clinical outcomes.

Vitamin D has been recognized for years as a key player in the control of bone metabolism through the regulation of calcium and phosphate homeostasis. Vitamin D deficiency appears to be common in the general population as a result of decreased dietary intake, diminished absorption, or limited exposure to sunlight. Vitamin D deficiency results in increased circulating levels of parathyroid hormone, which has several disadvantageous effects on bone metabolism. The intake of ordinary doses of vitamin D supplements appears to be associated with a decrease in total mortality rates. In the chronic k

Utilization of hemodiafiltration as treatment modality in renal replacement therapy for end-stage renal disease patients--a global perspective.

The worldwide demand for renal replacement therapy (RRT) is constantly rising. In order to support both medical communities and healthcare authorities in their decision-making processes, it has become increasingly interesting to provide quantitative overviews on the global development of the number of end-stage renal disease (ESRD) patients, as well as information on the quantitative development of individual treatment modalities. Worldwide, numerous registries and studies contribute to establishing a reliable base for understanding ESRD patient numbers. Supplementary, Fresenius Medical Care m

Vitamin D supplementation and recombinant human erythropoietin utilization in vitamin D-deficient hemodialysis patients.

INTRODUCTION: We sought to examine the impact of ergocalciferol (ERGO) on recombinant human erythropoietin (EPO) use in a cohort of 25-OH vitamin D (25-D)-deficient hemodialysis (HD) patients. METHODS: Baseline 25-D levels were obtained for all patients who received HD >6 months in our unit. Patients with levels between 10 and 30 ng/mL received ERGO 50,000 IU x 4 doses and patients with levels <10 ng/mL received 50,000 IU x 6 doses over a 4-month period. Monthly dose of EPO was recorded at baseline and after ERGO supplementation. RESULTS: Baseline 25-D levels were <30 ng/mL in 89% of tested
