Primary Sphenoidal Sinus Lymphoma with Initial Presentation as Unilateral Abducens Nerve Palsy Symptom.

A 48-year-old man presented with 3 days of mild horizontal diplopia in the left direction, followed by the onset of headache 17 days later. A physical examination revealed isolated left abducens nerve palsy. Head computed tomography (CT) and magnetic resonance imaging (MRI) scans revealed soft-tissue density neoplasms that occupied the sphenoidal sinus and further invaded to destroy the clivus. Immunohistochemical staining of neoplasms was performed from biopsies samples. The pathological diagnosis was extranodal natural killer (NK)/T-cell lymphoma (ENKL), nasal type, associated with Epstein-B

Anatomy of the lateral femoral cutaneous nerve relevant to clinical findings in meralgia paresthetica.

INTRODUCTION: Compression of the lateral femoral cutaneous nerve (LFCN), known as meralgia paresthetica (MP), is common. We investigated the topographic anatomy of the LFCN focusing on the inguinal ligament and adjacent structures. METHODS: Distances from various bony and soft-tissue landmarks to the LFCN were investigated in 33 formalin-embalmed cadavers. RESULTS: The mean distance from the anterior superior iliac spine (ASIS) to the LFCN was 8.8 mm. In approximately 90% of cases, the LFCN lay <2 cm from the medial tip of the ASIS, whereas, in 76% of cases, it was <1 cm away. The mean angle

Surgery for Primary Malignant Parotid Neoplasms.

The successful treatment of salivary gland malignancies originating in the parotid gland begins with rigorous and thorough surgical management coupled with the directed and appropriate potential adjuvant use of radiation therapy and chemotherapy. The anatomic complexity of the region in relation to the facial nerve and adjoining neurovascular and musculoskeletal structures requires sound surgical planning and decision making based on preoperative and intraoperative findings. The clinical presentation of parotid malignancy is summarized, as well as the further evaluations that are performed, in

Therapeutic options for cervicogenic headache.

The term cervicogenic headache (CeH) describes a syndrome originating from the cervical spine. There are a variety of therapeutic approaches used for the management of CeH, but scientific evidence of their effectiveness is scarce. No medication drug has proven to be effective. The evidence for greater occipital nerve blocks, cervical nerve blockades, facet joint injections and surgical procedures is limited. Several physical therapy interventions are proposed for CeH, with spinal manipulation and soft tissue interventions being the most commonly used. However, the lack of solid evidence of pos

External and internal influences on muscle pathology.

Three cases of different types of neuromuscular diseases demonstrate different muscle responses to external stress or intrinsic muscle abnormalities. The first muscle biopsy shows stenosis of its vessels causing acute muscle ischemia, stress from an external vascular disease. The muscle response is similar to the cellular necrosis seen in primary muscle diseases (myopathies), but the histologic pattern is more focal than most myopathies. The second muscle biopsy demonstrates the effects of external motor nerve injury or disease causing groups of muscle fibers to atrophy. If a nerve reinnervate

An inherited disorder with splenomegaly, cytopenias, and vision loss.

We describe a novel inherited disorder consisting of idiopathic massive splenomegaly, cytopenias, anhidrosis, chronic optic nerve edema, and vision loss. This disorder involves three affected patients in a single non-consanguineous Caucasian family, a mother and two daughters, who are half-sisters. All three patients have had splenectomies; histopathology revealed congestion of the red pulp, but otherwise no abnormalities. Electron microscopic studies of splenic tissue showed no evidence for a storage disorder or other ultrastructural abnormality. Two of the three patients had bone marrow exam

Endometriosis in the Lumbar Plexus Mimicking a Nerve Sheath Tumor.

Endometriosis consists of ectopic endometrial tissue outside of the uterine cavity. It is typically benign. It may cause neurological symptoms if involving the central or peripheral nervous system. We present in this report a 46-year-old Caucasian female with progressively worsening lumbar pain with radiation to her left anterior thigh. MR imaging showed an enhancing mass in the L4 neural foramen, intrepreted as a nerve sheath tumor. At operation the nerve showed extrinsic and intrinsic abnormality, proven to be endometriosis. Postoperatively, the patient reported relief from her radiculopathy

Multi-walled carbon nanotubes inhibit regenerative axon growth of dorsal root ganglia neurons of mice.

Recent observations have demonstrated that nanomaterials may be toxic to human tissue. While the ability of nano-scaled particulate matter is known to cause a range of problems in respiratory system, recent observations suggest that the nervous system may be vulnerable as well. In the current paper we asked whether exposure of primary neuronal cell cultures to nanoparticles might compromise regenerative axon growth. Regenerative response was triggered by performing a conditioning lesion of sciatic nerve five days prior to collection of dorsal root ganglia (DRG). DRG neurons were plated at a lo

Approach to vasculitic neuropathies.

Since vasculitic neuropathy is treatable and potentially debilitating, clinicians should develop an approach to neuropathy that increases the likelihood of uncovering existing systemic or nonsystemic vasculitis. The presence of a connective tissue disease, systemic vasculitis, asymmetric or non--length-dependent axonal polyneuropathy, or multiple axonal mononeuropathies should heighten suspicion, but vasculitic neuropathy can also present as a distal symmetric polyneuropathy with or without other organ involvement. Electrodiagnostic testing utilizing extensive nerve conductions may be helpful

Activation of p38 MAP kinase in the rat dorsal root ganglia and spinal cord following peripheral inflammation and nerve injury.

The intrathecal administration of p38 MAP kinase (p38) inhibitor has been shown to reduce hyperalgesia. In the present study, we investigated the activation of p38 in the rat dorsal root ganglion (DRG) and spinal cord following peripheral tissue inflammation and nerve injury immunohistochemically. Peripheral inflammation and chronic constriction injury (CCI) of the sciatic nerve induced a significant increase in the percentage of phosphorylated (P-) p38-immunoreactive (IR) neurons, primarily small sized ones in bilateral DRGs. In contrast, following axotomy, a significant decrease in the perce

Apoplexy of a pituitary macroadenoma with reversible third, fourth and sixth cranial nerve palsies following administration of hypothalamic releasing hormones: MR features.

Pituitary apoplexy in patients with pituitary macroadenomas can occur either spontaneously or following various interventions. We present a case of a 71-year-old woman who developed third, fourth, and sixth cranial nerve palsies following administration of the four hypothalamic releasing hormones for routine preoperative testing of pituitary function. The MR examination showed interval tumor growth with impression of the floor of the third ventricle. There were also changes in signal intensity characteristics of the mass, suggestive of intratumoral bleeding. A transsphenoidal surgery with subt

Metastasis in a benign duodenal stromal tumour.

Gastro-intestinal stromal tumour (GIST) is increasingly recognized as a distinct entity within the group of soft tissue tumours. Mostly, GIST arises from the muscular components of the stromal layer, but the tumour may also originate from the autonomic nerve system, recently designated as gastro-intestinal autonomic nerve tumour (GANT). The majority of GIST is located in the stomach and small intestine; only 4% of GIST is found in the duodenum. Clinical and pathological criteria to differentiate benign from malignant GIST are not well established. Tumour size and mitotic activity are commonly

Histology of posterior scleral foramen in normal eyes: its possible role in optic nerve damage in chronic open angle glaucoma.

We examined the histologic features of the posterior scleral foramen in 27 enucleated eyes (16 during autopsy and 11 with choroidal malignant melanoma). The histologic findings of the posterior scleral foramen were similar to those in previous reports. The shape of the scleral foramen was either cylindrical or truncated, the latter occurring when the inner margin of the sclera projected with a crest-like edge pressing into the optic nerve. This configuration was encountered more frequently in the upper and lower temporal aspects of the scleral foramen in 10 eyes, while the nasal rim was free o

Barbed needle and inexplicable paresthesias and trismus after dental regional anesthesia.

The tips of 100 needles that had been used clinically for the administration of mandibular block anesthesia (50 by students and 50 by staff) were examined individually under a dissecting microscope. Sixty percent of these needles were found to be barbed. With the use of an animal laboratory simulation, it has been shown that the tips of standard needles as used in general dental practice will barb if allowed to touch bone (medial aspect of the mandibular ramus) during the administration of a mandibular block for dentistry using the direct approach and that a relationship exists between the pat

Experimental allergic encephalomyelitis. I. Optic nerve and central nervous system manifestations.

Experimental allergic encephalomyelitis (EAE) was produced in six adult rhesus monkeys. The animals were evaluated serially by ocular, ophthalmoscopic, fluorescein fundus angiographic, pupillary, visual evoked potential, neurologic, cerebrospinal fluid (CSF), and hematologic examinations and by postmortem detailed histopathologic examination. All the animals developed acute EAE. Four of the monkeys, surviving longer than 1 month, developed chronic relapsing EAE and were sacrificed 3 to 14 months after sensitization. All 12 eyes developed acute optic neuritis (with variable degrees of optic dis

Associations of soft contact lens wear duration and stromal neuromediator profiles with recovery after SMILE: A prospective cohort study.

BACKGROUND: To investigate the associations of soft contact lens (SCL) wear duration and stromal neuromediator profiles measured in small incision lenticule extraction (SMILE)-derived lenticules with postoperative recovery after SMILE. METHODS: This prospective observational cohort study enrolled 90 patients undergoing SMILE, stratified by SCL wear duration: long-term (> 10 years, n = 25), short-term (≤ 10 years, n = 35), and non-wearers (n = 30). Corneal stromal lenticules obtained during SMILE were analyzed for nerve growth factor (NGF), calcitonin gene-related peptide (CGRP), substance P (

Neuroma Prevention by Nerve to Artery Coaptation: A Pilot Study.

PURPOSE: Neuroma formation is a notable complication that can occur following amputation. Newer techniques are proving to be successful in preventing neuroma formation by providing the proximal nerve segment with an end organ to help with organized growth. Redirecting arterial blood flow to the nerve endings may be an effective mechanism of preventing neuroma formation. The aim of this study was to demonstrate that by coapting the proximal end of a transected nerve with a functional artery as a vascular end organ, neuroma formation can be prevented. METHODS: Six Sprague-Dawley rats were used

The Rare Large Common Peroneal Nerve's Schwannoma-A Case Report and Literature Review.

Schwannoma in the popliteal fossa is still rare, often diagnosed late because it grows slowly and has no symptoms. It is often misdiagnosed with connective tissue tumors or with neurological disorders originating in the spine or disorders of the peroneal nerve. Schwannoma within the common peroneal nerve is still rare in the popliteal fossa, with most tumor sizes around 2 cm in diameter due to their smaller size of nerve but can cause neurologic disturbance, especially when it is large. And over a long time, it can cause serious complaints like neurological deficits and make surgery difficult
