Surgery computer: a quiet revolution for general practice.

This paper shows how a well planned computer system can transform a practice's programme of prevention and surveillance, for long the Cinderella subjects in our Health Service. It was not necessary to employ specially trained staff to operate the computer and our receptionists adapted quickly and successfully: in fact, a new and interesting job coordinating the new routines has evolved which will include the preparation and presentation of a wide range of practice statistics. After initial hostility the computer has been accepted by the staff and its ability to increase the efficiency of the p

Establishing development strategies and improvement paths for decision coach competencies in shared decision-making using an integrated accessibility-performance analysis and network relation map approach.

BACKGROUND: A decision coach (DC) is a trained healthcare professional who provides non-directive support to patients in health-related decision-making. The decision coach role can be integrated into shared decision-making (SDM) processes, but is not an inherent component of SDM, as both may be applied independently depending on the clinical context. Numerous trained DCs are nurses who have transitioned from their roles as patient educators to knowledgeable, skilled decision facilitators. Thus, developing core competencies has become increasingly crucial for DC training, especially patient-cen

qSOFA and Lactate for Nurse-Led Identification of High-Risk Septic Patients: A Prospective Observational Study.

BACKGROUND: Early identification of patients with sepsis at risk of deterioration is essential to reduce mortality. The quick Sequential Organ Failure Assessment (qSOFA) score is a widely used bedside tool but has limited sensitivity. Emerging evidence suggests that combining qSOFA with lactate levels (qSOFA-lactate) may improve predictive performance. AIM: The aim of this study was to evaluate the prognostic accuracy of the qSOFA-lactate score in predicting 30-day all-cause mortality among hospitalised patients with sepsis and to assess its applicability in nurse-led clinical assessments. Clinical and Operational Outcomes Associated with the Adoption of a Digital Wound Care Solution in Home Health Settings.

INTRODUCTION: Wounds increase the risk of hospitalization in home health (HH) settings by up to 52%. They also consume a notable portion of HH budgets due to frequent nursing visits for wound assessment and care. To address these challenges, a U.S.-based HH enterprise adopted a Digital Wound Care Solution (DWCS) to enhance wound management and operational efficiency. This study examines the impact of integrating the DWCS into practice, focusing on clinical and operational indicators and potential cost savings. METHODS: This study employed a quasi-experimental pre-post design to evaluate the i

Enabling factors that facilitate recovery among survivors of gender-based violence.

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: Gender-based violence (GBV) has long-term devastating effects on psychological health, resulting in depression, anxiety and posttraumatic stress disorder. Beyond physical and mental health symptoms, GBV can affect survivors on many personal, social, and spiritual levels, impacting their ability to connect to themselves, others, and the world around them. While most research on recovery following GBV has focused on recognizing factors associated with distress or adverse outcomes, there is limited information about how they recover. WHAT THE PAPER ADDS

Health care provider attitudes and practices related to intrauterine devices for nulliparous women.

OBJECTIVE: To examine predictors of health care providers perceiving intrauterine devices (IUDs) as unsafe for nulliparous women and of infrequent provision of IUDs to nulliparous women. METHODS: We analyzed questionnaire data obtained during December 2009 to March 2010 from 635 office-based providers (physicians) and 1,323 Title X clinic providers (physicians, physician assistants, certified nurse midwives, nurse practitioners, and nurses). Using multivariable logistic regression, we estimated adjusted odds ratios (ORs) and 95% confidence intervals (CIs) of the associations between patient,

"Push" versus "Pull" for mobilizing pain evidence into practice across different health professions: a protocol for a randomized trial.

BACKGROUND: Optimizing pain care requires ready access and use of best evidence within and across different disciplines and settings. The purpose of this randomized trial is to determine whether a technology-based "push" of new, high-quality pain research to physicians, nurses, and rehabilitation and psychology professionals results in better knowledge and clinical decision making around pain, when offered in addition to traditional "pull" evidence technology. A secondary objective is to identify disciplinary variations in response to evidence and differences in the patterns of accessing resea

Evaluating effect of implementing Gibbs' reflective model on quality of nurses' documentation in emergency departments: a clinical trial.

BACKGROUND: Emergency nurses are responsible for providing and documenting immediate patient care. Documentation is a fundamental component of their professional practice; however, evidence indicates that documentation standards are often not adequately met. Therefore, innovative training approaches should be employed to improve the quality of nurses' documentation. The present study aims to evaluate the effect of implementing Gibbs' reflective model on the quality of documentation by emergency department nurses. METHODS: This cluster-randomized clinical trial was conducted on 68 nurses worki

A multiphase, multimethod protocol for evaluating air-in-line alarms and infusion-technology usability during antineoplastic drug administration.

Air-in-line alarms during intravenous administration of antineoplastic drugs can interrupt therapy, increase nursing workload, and affect the continuity and safety of treatment. Although advanced infusion technologies have been introduced to reduce these events, evidence comparing their performance in clinical practice remains limited. This protocol describes a multicenter, prospective, multiphase study, combining a controlled simulation phase with two observational phases, designed to investigate the occurrence and management of air-in-line alarms during antineoplastic drug administration. [Clinical practice guidelines for day surgery of tonsils and adenoids in children].

Day surgery is an important part of the comprehensive reform of public hospitals, which is conducive to improving hospital management level, enhancing medical service efficiency, and controlling medical expenses. The National Health Commission has included tonsillectomy and/or adenoidectomy in the recommended list of daytime surgeries. However, there is no unified understanding and clinical practice of tonsillar and/or adenoid day surgery in China. Currently, there is an urgent need to develop clinical practice guidelines for tonsillar and/or adenoid day surgery to standardize the procedure. T

Nurses Leading Safe Infant Sleep Initiatives in the Hospital Setting.

Every day, 10 otherwise healthy infants die from sleep-related deaths in the United States. These deaths, termed sudden unexpected infant death, remain the leading cause of post-neonatal death in the United States despite known modifiable risk factors and prevention recommendations. In birthing hospitals, many parents report being given incorrect and sometimes no information about infant sleep safety, which creates immediate and long-term safety concerns. In this article, we provide an overview of sudden unexpected infant death, including sudden unexpected postnatal collapse, and the latest sa

‘It’s a necessary evil’: Experiences and perceptions of mandatory reporting of child abuse in Victorian general practice.

BACKGROUND AND OBJECTIVE: General practitioners (GPs) and practice nurses (PNs) are mandated to report child abuse; however, only 2–4% of reports are made by Victorian health professionals. This is concerning, given that the estimated prevalence of physical child abuse alone in Australia is 5–18%. The aim of this study was to explore GPs’ and PNs’ experiences and perceptions of mandatory reporting of child abuse in Victoria. METHODS: Semi-structured interviews with 17 Victorian GPs and PNs were undertaken and thematically analysed. RESULTS: Participants had limited understanding of mandatory

Exchanging implements: the micro-materialities of multidisciplinary work in the operating theatre.

Surgical procedures rely upon an array of commonplace tools, implements and materials that mediate practice and disciplinary collaboration within the operating theatre. Substantial time is dedicated to the issue and provision of these artefacts and their timely exchange is critical to the successful accomplishment of surgical procedures. In this article, we consider the practice, knowledge and agency that informs how particular implements and materials are passed by the scrub nurse to the surgeon that in turn enables their deployment with regard to the particular procedure and the contingencie
