Mucous membrane graft for cicatricial ectropion in lamellar ichthyosis: an approach revisited.

A 26-year-old woman with congenital lamellar ichthyosis presented with bilateral cicatricial ectropion of the upper and lower eyelids. There was severe exposure keratopathy with corneal perforation in the left eye. The generalized skin involvement necessitated the use of an alternative to correct the cicatricial ectropion. Mucous membrane graft harvested from the oral cavity was used to correct the cicatricial ectropion of upper and lower eyelid of both eyes. Six months after treatment, the mucous membrane graft was keratinized and a significant reduction in lagophthalmos was seen. Mucous memb

Surgical treatment of vertical ocular motility disorders of supranuclear origin.

Vertical ocular motility disorders of supranuclear origin can be congenital or acquired later in life. They produce complex forms of strabismus that require specialized surgical management to restore ocular alignment, eliminate torticollis, treat diplopia, and restore normal lid position. For each condition, surgical management must be individualized to address multiple impediments to fusion. This article outlines the surgical management of three common types of supranuclear vertical strabismus.

Adenovirus advances: new diagnostic and therapeutic options.

PURPOSE OF REVIEW: Adenoviral infection is common, can be severe, and may cause significant morbidity. RECENT FINDINGS: Ophthalmologists and optometrists are often guilty of spreading adenovirus because it is highly contagious and has 53 serotypes with variable morphology. Adenovirus is often difficult to diagnose based on clinical appearance and, in the early stages, is associated with a red eye or superficial keratitis common to herpes and other infections. This difficulty results in the indiscriminate use of antibiotics, which are expensive and of no established value in treating a viral i

[Facial and eye pain - Neurological differential diagnosis].

Head and facial pain are common in neurological practice and the pain often arises in the orbit or is referred into the eye. This is due to the autonomic innervation of the eye and orbit. There are acute and chronic pain syndromes. This review gives an overview of the differential diagnosis and treatment. Idiopathic headache syndromes, such as migraine and cluster headache are the most frequent and are often debilitating conditions. Trigemino-autonomic cephalalgias (SUNCT and SUNA) have to be taken into account, as well as trigeminal neuralgia. Trigemino-autonomic headache after eye operations

Surgical management of adult onset age-related distance esotropia.

PURPOSE: To study the effects of bilateral medial rectus recession for the management of adult onset age-related distance esotropia. METHODS: Ten patients with adult onset age-related distance esotropia measuring 14 prism diopters or greater underwent bilateral medial rectus recession to eliminate the need for prism glasses. RESULTS: In all but one case, the diplopia completely resolved postoperatively, with a median residual deviation of 1 prism diopter esophoria for distance and 2 prism diopters exophoria at near. CONCLUSION: Bilateral medial rectus recession is a useful technique for the

High-resolution hyperspectral imaging of the retina with a modified fundus camera.

PURPOSE: to examine the practical feasibility of developing a hyperspectral camera from a Zeiss fundus camera and to illustrate its use in imaging diabetic retinopathy and glaucoma patients. METHODS: the original light source of the camera was replaced with an external lamp filtered by a fast tunable liquid-crystal filter. The filtered light was then brought into the camera through an optical fiber. The original film camera was replaced by a digital camera. Images were obtained in normals and patients (primary open angle glaucoma, diabetic retinopathy) recruited at the Manchester Royal Eye Ho

Functional benefits of adult strabismus surgery.

The most common functional benefits from strabismus surgery in the adult patient are restoration of binocular vision and elimination of diplopia and/or a compensatory head posture. Adult patients with chronic strabismus have been thought to have the potential to achieve only a cosmetic result from surgery. On the contrary, several studies have shown that unexpected sensory fusion is possible after excellent postoperative motor alignment in these patients. Other functional benefits of strabismus surgery in the adult patient include the expansion of binocular visual fields in patients with esotr

SD-OCT with prolonged scan depth for imaging the anterior segment of the eye.

An 840-nm wavelength spectral domain optical coherence tomography (SD-OCT) with prolonged scan depth was developed and mounted onto a conventional slit lamp for imaging the anterior segment of the eye. X-Y cross aiming was applied to align the SD-OCT scanning position during imaging. An internal fixation target displayed on a miniature LCD monitor was provided. The SD-OCT instrument had an axial resolution of 6 μm and a prolonged scan depth of 7.2 mm. High-quality SD-OCT images, consisting of 2,048 × 2,048 pixels, were acquired of the entire anterior chamber and entire crystalline lens from a

Study on the treatment of traumatic orbital apex syndrome by nasal endoscopic surgery.

Orbital apex syndrome (OAS) is a complex disease caused by a variety of pathological factors, and trauma is one of the main factors/causes. Clinical data of 17 cases of traumatic OAS treated by nasal endoscopic surgery in our department from January 2002 to April 2009 were gathered and reviewed. Among them, the six patients presented with OAS after injury to the lateral wall of orbital apex. Seven other patients exhibited OAS after injury to the medial wall of orbital apex, two displayed OAS after zygomatic trauma, while OAS manifested in the other two patients with craniocerebral trauma 3 day

[Periocular surgery and the impact of split-thickness skin grafts].

Oculoplastic surgery has to consider the specific characteristics of the periocular region and possible resulting functional consequences. Motility of eyelids, limited skin thickness with excellent wound healing conditions, lacrimal duct system and eyelid ligaments are important criteria for the individual choice of the surgical procedure. The use of split-thickness skin grafts represents an established method to repair periocular defects. Important medical conditions for split-thickness transplantation are colour of the skin, skin structure, photodamage of the skin, hair growth and other dege

Orbital lesions: surgical management within a multidisciplinary treatment.

AIM: The aim of this review was to establish the timing and the role of surgery within the multidisciplinary treatment of orbital lesions. METHODS: It was retrospectively analysed a consecutive series of 50 orbital lesions, surgically treated in the Department of Neurosurgery of the Udine University Hospital (Udine, Italy) between 1998 and 2007, and reviewed the current literature on this subject. RESULTS: This series of 50 consecutive symptomatic patients surgically treated (32 total resection, 10 subtotal removal and 8 biopsies ) was first evaluated, with the aim of comparing our experienc

Further implications of probable changes in medial rectus muscle innervation after surgery for infantile esotropia.

INTRODUCTION: The conclusion that setting the eyes of an infantile esotrope (ET) straight with a successful bilateral medial rectus muscle reseccion (MROU) reduces the preoperative hyperinnervation of these muscles almost to normal may have special implications for the best principles of management of infantile ET not detailed in a recent paper by Mimms, III, Miller and Schoolfield. METHODS: The same 113 infantile esotropes who provided data for the exoshift under anesthesia study had previously provided data for a dose-response curve. A simple geometric calculation was done to determine the

The effects of CAM vision stimulator for bilateral amblyopia of different etiologies.

BACKGROUND: To evaluate the association between bilateral amblyopia and the effects of CAM vision stimulator. METHODS: This retrospective study was carried out between January 1994 and July 2004. The 105 children enrolled were all younger than 7 years old and had bilateral amblyopia. All children wearing full-correction glasses regularly received CAM vision stimulation once per week. The patients' age, initial best corrected visual acuity (BCVA), final BCVA, and the number of CAM sessions needed to achieve a visual acuity (VA) > or = 0.8 were recorded. RESULTS: The major cause of bilateral a

The pathogenesis of pterygium: current concepts and their therapeutic implications.

Pterygium is a disease of the ocular surface that is associated with chronic UV exposure and is characterized by proliferation, inflammatory infiltrates, fibrosis, angiogenesis and extracellular matrix breakdown. Although pterygium is not fully understood, significant progress has been made toward understanding the mechanisms involved in its pathogenesis. In this review, we provide an update on the signaling pathways activated by UV light that result in induction of mediators responsible for the growth of pterygium. We also review the recent genetic studies on hereditary factors and provide a
