Orbital volume analysis: validation of a semi-automatic software segmentation method.

PURPOSE: The purpose of this study was to validate a quick, accurate and reproducible (semi-) automatic software segmentation method to measure orbital volume in the unaffected bony orbit. Precise volume measurement of the orbital cavity is a useful addition to pre-operative planning and intraoperative navigation in orbital reconstruction. METHODS: In 21 CT scans, one unaffected orbit was selected to compare manual segmentation (gold standard) with three segmentation methods using iPlan software (version 3.0.5; Brainlab, Feldkirchen, Germany): automatic (method A), automatic minus bone/air ma…

Is frosting effective? The role of retention sutures in posttraumatic orbital reconstruction surgery.

BACKGROUND: Plastic surgeons, otolaryngologists, and maxillofacial surgeons routinely perform periorbital fracture fixation in the setting of acute trauma. One of the most common complications of periorbital surgery continues to be lower-lid retraction. The Frost suture has been widely adopted as one of the methods of reducing the frequency of this complication, although there are no compelling data on its efficacy. MATERIALS AND METHODS: A retrospective chart review was performed on trauma patients who had undergone periorbital fracture fixation by one of three reconstructive surgeons at a s…

Surgical treatment of orbital floor blowout fracture in children: A case report.

INTRODUCTION: Orbital floor blowout fracture is a common traumatic lesion of the craniofacial complex, but rarely in children population, consequently representing challenge in surgical treatment. Timely diagnosis and surgical treatment prevent the probability of the occurrence of the functional complications. CASE REPORT: We presented surgical treatment of on 8-year-old girl with a blowout orbital floor fracture one month after the injury. The predominant symptoms were: ocular bulb motility disorder with consecutive strabismus and double vision. Orbital floor reconstruction was made by an au…

The last ride of Henry II of France: orbital injury and a king's demise.

Jousting was a popular pastime for royalty in the Renaissance era. Injuries were common, and the eye was particularly at risk from the splinters of the wooden lance. On June 30, 1559, Henry II of France participated in a jousting tournament to celebrate two royal weddings. In the third match, Gabriel de Montgomery struck Henry on the right shoulder and the lance splintered, sending wooden shards into his face and right orbit. Despite being cared for by the prominent physicians Ambroise Paré and Andreas Vesalius, the king died 10 days later and was found to have a cerebral abscess. The wound wa…

An unexpected cause of orbital apex syndrome in an immune-competent elderly male.

Invasive aspergillosis causing orbital apex syndrome (OAS) in an immune-competent individual is a very rare phenomenon, scarcely reported in medical literature. A 68-year-old male presented with progressive loss of vision in the right eye, starting after a cataract surgery. Neurological examination suggested OAS. Imaging was suggestive of mass lesion causing destruction of ethmoid bone. Biopsy of the lesion could not be done initially in view of its proximity to the major neuro-vascular bundle in the orbital apex and cavernous sinus and the major risk involved in the procedure relative to its …

Incidence of depression of the medial orbital wall without a recent trauma event.

The aim of this study was to see how frequently depression of the orbital medial wall can be found on computed tomographic (CT) imaging of patients without a recent trauma event. A total of 14,628 CT scans (12,515 brain CT scans and 2113 three-dimensional facial CT scans) of patients with no recent trauma event (defined as a facial trauma within 1 mo) were reviewed. If there was a depression of more than 3 mm on the medial wall compared with the contralateral side in the axial view, the scan was included in the "depression or asymmetry" group. In the depression or asymmetry group, old fracture…

Incidental finding of an orbital foreign body in a child with microcephaly and developmental delay.

A 3-year-old boy underwent investigations for microcephaly and global developmental delay. An MRI scan identified an ill-defined enhancing lesion in the left superolateral orbit. On subsequent questioning his parents reported that he had been admitted to a neighbouring hospital 2 months earlier with left-sided preseptal cellulitis following a fall onto a plastic toy. Following the episode of cellulitis he developed intermittent mild erythema and oedema of the left upper eyelid for which his parents had not sought further medical attention. The child was admitted for an anterior orbitotomy via …

The value of three-dimensional printing modelling for surgical correction of orbital hypertelorism.

Orbital hypertelorism is defined as an increased distance between both medial and lateral sides of the orbits. Most common causes are frontonasal malformations, craniofacial fissures, encephalocele and a miscellaneous group of various syndromic or chromosomal disorders. Surgical correction of orbital hypertelorism is still challenging. The present report describes a case of severe orbital hypertelorism of an 11-year-old boy, where surgical correction was planned using three-dimensional printing modelling. This approach allowed reducing time of surgery, accurately planning the location of the o…

Defining pediatric orbital roof fractures: patterns, sequelae, and indications for operation.

BACKGROUND: There are few published data addressing pediatric orbital roof fractures, particularly regarding long-term outcomes and indications for operative intervention. To provide evidence-based guidelines for the management of these injuries, the authors examined a large cohort of patients with this injury pattern. METHODS: Institutional review board approval was obtained to review pediatric patients presenting to Johns Hopkins Hospital from 1991 to 2011. All patients with a diagnosis indicating orbital or skull base fractures were reviewed to determine orbital roof involvement. Patients …

The impact of suction drainage on orbital compartment syndrome after craniofacial surgery.

PURPOSE: Postoperative orbital compartment syndrome is a potentially blinding complication of surgery in the orbital region. We describe the technique of orbital drain placement as a method of preventing vision loss resulting from orbital compartment syndrome. METHODS: We present a retrospective case series of 29 patients who underwent orbital fracture, facial fracture, and orbital implant removal from 7/4/2008 to 5/3/2013 by the same craniofacial surgeon. An orbital drain was placed in each patient. The drainage was recorded daily until drain removal. Criteria for removal included less than …

Abnormal anatomy of inferior orbital fissure and herniation of buccal fat pad.

The anatomy of the inferior orbital fissure has been well studied, and its reported dimensions vary little. It is encountered during exploration of the orbital floor and when possible is not disturbed. We describe a case of herniation of buccal fat through the inferior orbital fissure that was found during exploration and repair of the orbital floor.
