External fixators have been used effectively for a variety of traumatic, congenital, and neuromuscular conditions of the lower extremity for many decades. The transition to management of Charcot foot and ankle deformities in the diabetic neuropathic patient is a logical application. External fixation can address the unique challenges in the Charcot foot, including osteoporosis, osteomyelitis, wound healing, and compliance issues. The author's experience has lead to abandoning other methods of fixation in favor of external fixation for most infected and noninfected Charcot deformities of the fo…

Charcot's neuroarthropathy is a relatively common disease in patients with diabetic neuropathy. If unrecognized or left untreated, Charcot's neuroarthropathy can result in a severely misshapen and unstable foot and ankle. Ulceration, soft-tissue infection, and osteomyelitis frequently ensue, and partial or complete amputation of the foot is not uncommon. A high index of suspicion and proper interpretation of clinical and diagnostic findings are essential to establish a timely and accurate diagnosis and to institute appropriate treatment. The pathogenesis of neuroarthropathy is reviewed and dia…

Chronic inflammation (CI) was studied immunologically and immunomorphologically on biopsy and surgical material: interstitial pulmonary diseases, chronic inflammatory processes of the uteri and ovaries, decubitus in the spinal cord damage, trophic ulcers of the limbs, osteomyelitis, chronic abscesses of soft tissues, coccygeal cysts and chronic paraproctitis. The following parameters determine the development of CI: persistence of the damaging agent, immunological insufficiency, mechanisms of autoimmunization, progression of CI according to the principle of vicious circle. CI looses its biolog…

Pasturella multocida is a well known potential cause of infection following bites or scratches by animals. The organism causes the usual clinical manifestations of a rapidly developing cellulitis at the site of injury. The resultant infection is dangerous and can progress on to a deep infection, osteomyelitis, and septicemia. In compromised patients, the source and etiology of the infection may be obscure making definitive diagnosis difficult. This paper reviews a very unusual case of a foot infection in a diabetic patient that was due to a domestic pet licking an excoriation on the foot.

The last decade has witnessed the refinement of microvascular tissue transplantation and its application in treating difficult open wounds. Nonetheless, severe type III tibial injuries continue to challenge reconstructive surgeons. The clinical presentation and surgical treatment of a type IIIB tibial fracture, complicated by long-standing chronic osteomyelitis, angulation deformity, and bone shortening, is presented. This case illustrates the benefit of combining microvascular transplantation of distant tissues with the Ilizarov technique of distraction osteosynthesis in the treatment of comp…

Rhinocerebral mucormycosis was diagnosed in a 75-year-old woman with a history of type II diabetes mellitus. This rare opportunistic infection is caused by fungi belonging to the order of Mucorales. The patient had a severe osteomyelitis of the base of the skull, resulting in complaints of headache and diplopia. She was treated with intravenous colloidal amphotericin B, surgical excision, and later with liposomal amphotericin B. She died of respiratory failure. Mucormycosis is usually a rapidly fulminant infection. This patient showed a remarkably chronic course.

Most of the periapical tissue lesions appear as radiolucent areas in a reontgenogram. However, several lesions of pulpal and periodontal origin as well as lesions caused by benign or malignant tumors appear in the roentgenogram as radiopaque areas. This article deals in detail with the classification, differential diagnosis and treatment of oral diseases which appear radiopaque in an X-ray film. Special attention is given to apical condensing osteitis, hypercementosis, chronic osteomyelitis and odontogenic tumors. These lesions are fairly common and the general practitioner should be familiar …

We are seeing an increased number of complications in intravenous drug abusers who resort to injecting the groin for vascular access (the "groin hit"). Vascular complications include venous thrombosis, arteriovenous fistula, mycotic aneurysm, ruptured pseudoaneurysm, and dissecting hematoma. Soft tissue complications include cellulitis and abscess. The latter may dissect into the extraperitoneal space. Skeletal complications include osteomyelitis and septic arthritis. This paper illustrates the radiographic spectrum of these complications. An algorithm will illustrate the radiographic evaluati…

Ten patients - two with Salmonella septicaemia associated with AIDS; one each Salmonella excretors in the convalescence stage and with Salmonella osteomyelitis; three with abdominal typhoid, and three with Salmonella enteritis - were treated with the gyrase inhibitor ciprofloxacin. The drug was given for 3-33 days, at a dosage of 250-750 mg orally in most cases. Clinical and bacteriological cure was achieved in nine patients. There were no significant side effects. One patient developed a macular rash, which is within the usual side-effect rate.

A case is presented of a child who sustained septic growth arrest of the distal radial epiphysis following osteomyelitis at 5 weeks of age. At age 8 years and 8 months radial lengthening using the Wagner apparatus was performed. Two and one-half centimeters of total length (18% of original length) was gained. No significant complications were encountered and normal function has been maintained. Although follow-up is short and additional lengthening is anticipated, this report illustrates the technical considerations for performing this procedure.

Tropical pyomyositis, a suppurative condition involving a group or groups of muscles, has only been described in the tropical areas of the world. An etiological factor in the genesis of this disease is closed muscle trauma causing bleeding, hematoma formation, muscle necrosis, and secondary blood borne infection; but this does not explain why the condition occurs or is contracted only in the tropical belt. The commonest organism present in the abscess is Staphylococcus aureus. Neglected or poorly treated tropical pyomyositis can give rise to chronic osteomyelitis or septic arthritis.

We report on a 63-year-old patient with rectal carcinoma that metastasized to a toe. Although bone metastases from malignant tumors are common, metastatic lesions of the small bones of extremities are very rare. We have found in the literature only 29 cases of carcinoma which have metastasized to the small bones of the feet. Twenty of these cases are verified histologically. The differential diagnosis includes osteomyelitis, gout, and Reiter's disease. The roentgenographic features and the possible pathogenetic mechanisms of peripheral metastases are discussed.

The diagnosis of thoracic actinomycosis is readily made when the patient presents with the constellation of poor oral hygiene, pulmonary infiltrates, empyema, sinus tracts, and osteomyelitis of the ribs. However, in the absence of rib and chest wall involvement, the disease may be overlooked. Three cases of primary pulmonary actinomycosis are presented showing three different pulmonary forms of the disease and comprising (a) subacute patchy pulmonary infiltrates, (b) a cavitary lesion simulating tuberculosis, and (c) a central mass lesion simulating bronchogenic carcinoma.

BACKGROUND: Diabetic hand infections are uncommon but potentially severe complications of diabetes mellitus. Because of the complex anatomy and functional importance of the hand, these infections may progress rapidly and result in serious morbidity, including osteomyelitis and amputation. Clinical data on this condition remain limited.
METHODS: This case series included patients hospitalized with diabetic hand infection at a tertiary care center between 2020 and 2026. Patients, ≥ 18 year-old, with confirmed diabetes mellitus and clinical, microbiological, and/or radiological evidence of hand …

BACKGROUND: Xylazine, a veterinary sedative not approved for human use, has emerged as a dangerous adulterant in the illicit opioid supply and is now linked to severe, progressive necrotic wounds. These lesions often occur at noninjection sites, exhibit atypical healing patterns, and are commonly complicated by infection, osteomyelitis, and delayed presentation. This study characterizes the clinical features, management strategies, and outcomes of xylazine-associated wounds to inform triage and harm-reduction efforts.
METHODS: A systematic review was conducted in accordance with preferred rep…

The present study aimed to elucidate the characteristics of imaging findings for medication-related osteonecrosis of the jaw (MRONJ) in patients with cemento-osseous dysplasia (COD). Images from patients with MRONJ and COD from among patients with osteomyelitis, including MRONJ, that were obtained from multidetector row computed tomography (MDCT), magnetic resonance imaging (MRI), panoramic radiographs, and dental radiographs, were retrospectively analysed. The characteristics of imaging findings for MRONJ in patients with COD were then analysed. Significant differences in the presence of sequ…
