Biomechanical evaluation of temporomandibular joint implants and periprosthetic bone under unilateral and bilateral clenching.

To ensure the long-term success of temporomandibular joint (TMJ) implants, it is imperative to understand their biomechanical performances. This study aims to compare the biomechanical performance of two stock implants (narrow and standard) under unilateral and bilateral clenching during both osseointegrated and non-osseointegrated conditions. Finite element models of a human mandible were developed from QCT data, with the left TMJ being replaced by the implants. Six clenching tasks were simulated to evaluate stress and strain distributions in the mandible and implants. Ipsilateral clenching p

The Clinical Outcomes of a Bioinductive Collagen Implant in Bursal-Sided Partial-Thickness Rotator Cuff Tears.

Background and Objectives: Many middle-aged and older individuals experience shoulder pain, often due to partial-thickness rotator cuff tears (PTRCTs). If conservative treatment fails to relieve symptoms in a patient, surgical intervention may be necessary. In such cases, using a bioinductive collagen implant may offer a viable alternative to conventional rotator cuff repair. Most notably, it offers potential advantages, particularly in reducing postoperative pain and promoting faster recovery. Accordingly, this study aims to evaluate the clinical outcomes of treating bursal-sided partial-thic

A case of persistent pneumothorax after bronchoscopic lung volume reduction successfully treated with video-assisted thoracic surgery without removal of endobronchial valves.

With proper patient selection excluding collateral ventilation, bronchoscopic lung volume reduction (BLVR) with endobronchial valves (EBVs) offers significant benefits but increases pneumothorax risk. Prioritized management of persistent pneumothorax post-BLVR remains unclear, with both EBV removal and video-assisted thoracic surgery (VATS) considered options. We report a 76-year-old man with severe emphysema who developed persistent pneumothorax and subcutaneous emphysema after left upper lobe BLVR. Following the patient's wishes, VATS was performed instead of EBV removal, resulting in a favo

Antibacterial abilities of Vancomycin interactions in PDEGMA brushes fabricated by interface-mediated PET-RAFT polymerization on implant surfaces.

The proliferation of pathogenic bacteria and biofilm formation on implantable material surfaces causes negative results in many medical treatments and infections. Despite the measures taken against unwanted bacteria with modern and advanced sterilization, infections due to contamination during the storage of implants are still an important problem. In order to overcome these problems, it has become necessary to develop synergistic systems to increase antibacterial performance and prevent biofilm formation. Polymer brush systems are among the strategies that can prevent bacteria and biofilm for

An Antimicrobial and Antifibrotic Coating for Implantable Biosensors.

Biofouling and foreign body responses have deleterious effects on the functionality and longevity of implantable biosensors, seriously impeding their implementation for long-term monitoring. Here, we describe a nanocomposite coating composed of a cross-linked lattice of bovine serum albumin and pentaamine-functionalized reduced graphene that is covalently coupled to antibody ligands for analyte detection as well as antibiotic drugs (gentamicin or ceftriaxone), which actively combats biofouling while retaining high electroconductivity and excellent electrochemical immunosensor behavior. Sensors

Acromial Cupping with Lodged Bioinductive Collagen Implant Following Rotator Cuff Repair: A Case Report.

CASE: We report a 39-year-old man with a supraspinatus retear treated with a Regeneten implant. Six months postoperatively, magnetic resonance imaging (MRI) revealed a foreign body lodged at the inferior acromion margin, causing acromial cupping, remodeling, and fibrosis. At the 12-month follow-up, the patient presented symmetrical strength and full range of motion with no pain. CONCLUSION: We report the first case of Regeneten implant migration with acromial lodging. Despite MRI findings, the patient remained asymptomatic with no functional limitations. Close postoperative follow-up is essen

A Novel Implantable Inferior Vena Cava Sensor: Validation of Remote Monitoring Derived Area with Computerized Tomography.

Intravascular volume shifts assessed by inferior vena cava (IVC) size are associated with adverse outcomes and hospitalization in patients with heart failure (HF). A novel implantable IVC sensor was developed to provide remote daily assessment of IVC area and its collapsibility index (in response to respiration) as a measure of volume status in patients with HF. Daily assessment of these biomarkers of volume status in HF enables early detection of fluid overload and proactive HF management. This study was designed to validate the accuracy of IVC area derived from the sensor as adjudicated agai

Delayed targeted atelectasis in a case of bronchoscopic lung volume reduction with endobronchial valves.

A woman in her late 60s with severe chronic obstructive pulmonary disease (COPD) and emphysema underwent bronchoscopic lung volume reduction (BLVR) with endobronchial valves (EBV) to address hyperinflation. The initial EBV placement has led to partial lobar atelectasis of the left lower lobe and resulted in significant improvement in the patient's symptoms and lung function. However, valve migration occurred later due to pneumothorax unrelated to valves, leading to suboptimal clinical improvement. The patient achieved delayed full lobar atelectasis 21 months after EBV placement, which led to a

Is Polyetheretherketone an Effective Alloplastic Material in Comparison to Titanium in Calvarial Reconstruction.

INTRODUCTION AND AIM: Polyetheretherketone (PEEK) and titanium (Ti) cranioplasty implants ideal outcomes are good esthetics, long-term stability, and protection of the fragile brain tissue. However, it is unclear whether PEEK implants can offer an equal alternative to Ti implants. This work aimed to critically review papers and case series published on both Ti and PEEK Cranioplasty regarding complications, clinical outcomes, ease of use, esthetics, manufacture and availability, cost and time-saving factors, postoperative quality of life (QOL), as well as their suitability for the fronto-orbito

Non degradation of chitosan and initial degradation of collagen nerve conduits used for protection of nerve coaptations.

BACKGROUND: Nerve conduits are either used to bridge nerve gaps of up to 3 cm or to protect nerve coaptations. Biodegradable nerve conduits, which are currently commercially available, include Chitosan or collagen-based ones. As histological aspects of their degradation are highly relevant for the progress of neuronal regeneration, the aim of this study was to report the histopathological signs of such nerve conduits, which were removed during revision surgery. MATERIALS AND METHODS: Either Chitosan (n = 2) or collagen (n = 2) nerve conduits were implanted after neuroma resection and nerve gr

Long-term cost-effectiveness analysis of sacral neuromodulation in the treatment of severe faecal incontinence.

AIM: The aim of this study was to evaluate the long-term cost-effectiveness of sacral neuromodulation in the treatment of severe faecal incontinence as compared with symptomatic management. METHODS: In the public health field, a micro-costing evaluation method was conducted from the perspectives of the health system and the society. The incremental cost-effectiveness ratio was used as a decision index, and we considered various scenarios to evaluate the impact of the cost of symptomatic management and percutaneous nerve evaluation success rate in its calculation. Clinical data were retrieved

A pilot study of mitochondrial response to an in vivo prosthetic joint Staphylococcus aureus infection model.

Prosthetic joint infections (PJI) are associated with orthopaedic morbidity and mortality. Mitochondria, the "cell's powerhouses," are thought to play crucial roles in infection response and in increased risk of sepsis mortality. No current research discusses PJI's effect on mitochondrial function and a lack of understanding of immune-infection interactions potentially hinders patient care. The purpose of this pilot study was to evaluate the impact of simulated PJI on local tissue mitochondrial function. Using an established prosthetic implant-associated in vivo model, tissues were harvested f

Controlled release of low-molecular weight, polymer-free corticosteroid coatings suppresses fibrotic encapsulation of implanted medical devices.

Inflammation-driven foreign body reactions, and the frequently associated encapsulation by fibrogenic fibroblasts, reduce the functionality and longevity of implanted medical devices and materials. Anti-inflammatory drugs, such as dexamethasone, can suppress the foreign body reaction for a few days post-surgery, but lasting drug delivery strategies for long-term implanted materials remain an unmet need. We here establish a thin-coating strategy with novel low molecular weight corticosteroid dimers to suppress foreign body reactions and fibrotic encapsulation of subcutaneous silicone implants.
