[Practice-oriented overview - current state of rhinosinusitis in adults based on the latest consensus papers and publications].

Over the past two decades, a paradigm shift has occurred in the understanding and management of rhinosinusitis, leading to new therapeutic options, necessitating a revised diagnostic and therapeutic approach. Nevertheless, avoidable errors are still frequently made in clinical practice. Rhinosinusitis ranks among the most prevalent upper respiratory tract conditions globally and is associated with a significant individual and socioeconomic burden. To prevent overdiagnosis and promote effective treatment, it is essential to understand the current classification, pathophysiology, and etiology of

Impact of Sinus Surgery on Nasal Discharge in Chronic Rhinosinusitis: A Systematic Review and Meta-Analysis.

BACKGROUND: Nasal discharge is one of the cardinal symptoms of chronic rhinosinusitis, impacting over 50% of patients. For patients with symptoms refractory to standard medical therapy, endoscopic sinus surgery is an option. The objective of this study is to characterize how nasal discharge improves after surgery in patients with chronic rhinosinusitis. METHODS: The literature was searched for articles reporting nasal discharge symptom data both at baseline and after surgery. Specific symptoms of interest on the 22-item sinonasal outcome test (SNOT-22) were "need to blow nose," "runny nose,"

Enhanced Therapeutic Options for Budesonide Nebulisation via Pulsating Nebuliser in Upper Respiratory Tract Diseases.

Introduction: The primary treatment method for T2 immune response-related airway inflammations, such as eosinophilic inflammation, is topical glucocorticosteroid therapy. However, its effectiveness is limited by the challenge of drug penetration into the perinasal cavity. The introduction of a new generation of pulsating nebulizers has partially addressed this issue. Pulsating nebulizers (vibrating aerosol) enhance the penetration of nebulized medication into the paranasal sinuses, thereby opening a new therapeutic option for the treatment of chronic sinusitis. Consequently, a new regulatory i

[Monoclonal antibodies in the treatment of recurrent polyposis rhinosinusitis. Literature review].

This article is devoted to the problem of the effectiveness of therapy of recurrent polyposis rhinosinusitis and the choice of the most effective drug from the group of monoclonal antibodies. The wide prevalence of the disease, as well as the low level of effectiveness of existing treatment methods, such as glucocorticosteroid (topical and topical) therapy, antibacterial therapy and available surgical methods of treatment (polypotomy, endoscopic polysinusitomy) allow us to conclude that the existing methods of treatment for recurrent polyposis process do not allow to achieve a stable positive

The burden of systemic corticosteroids in patients with chronic rhinosinusitis with nasal polyps.

Background: Systemic corticosteroids (SCS) are widely used to treat patients with chronic rhinosinusitis with nasal polyps (CRSwNP) that is insufficiently controlled with first-line treatments. However, such treatment must be balanced against the risk of adverse effects with protracted or repeated use. Increasing awareness of these adverse effects and the introduction of biologics are changing established management approaches. Objective: The objective was to review the role of SCS in the management of CRSwNP in the evolving treatment landscape. Methods: A literature search was conducted for s

Management of Chronic Rhinosinusitis with or Without Nasal Polyposis.

Chronic rhinosinusitis (CRS) is a complex disease with multiple phenotypes. The primary phenotypic division is CRS with nasal polyps (CRSwNP) and CRS without nasal polyps (CRSsNP). The majority of patients with CRSwNP have type 2 inflammatory disease characterized by eosinophilic inflammation and type 2 cytokines (primarily interleukin-4, 5, and 13). CRS is not primarily an infectious problem, although viral and bacterial infections or dysbiosis may exacerbate the disease. There is an association of CRS with asthma, particularly CRSwNP. Treatment of the CRS benefits asthma.

[Different immunological types of CRSwNP in the context of the new European EAACI nomenclature : Part 2: Hypersensitivity reactions of type V (epithelial barrier defects)].

BACKGROUND: Chronic rhinosinusitis (CRS) has a prevalence of up to 11% in Europe and the United States, making it one of the most common chronic diseases. Classification according to immunological endotypes is increasingly being incorporated into disease definitions, particularly for chronic rhinosinusitis with nasal polyps (CRSwNP). Genetic and epigenetic alterations within the mucosal immune system play an important role in this context. Identification of endotypes may help to elucidate disease heterogeneity and guide development of personalized therapeutic approaches. In part 1 of this publ

[Advances in next-generation sequencing technology to analyze the microbiome of patients with chronic sinusitis].

Chronic rhinosinusitis (CRS) is a chronic inflammatory disease of the sinus mucosa, and the pathogenesis of CRS has not been fully elucidated, and the impact of dysbiosis of the microbiome in the nasal cavity and even in the gut on the pathogenesis of CRS remains controversial. Next-generation sequencing technology, a culture-independent high-throughput sequencing method, contributes to a comprehensive understanding of the CRS microbiome. This article reviews the progress of research on the relevance of bacteria and other microorganisms to CRS and the microbial characteristics of the sinus and

Tissue Eosinophilia is Superior to an Analysis by Polyp Status for the Chronic Rhinosinusitis Transcriptome: An RNA Study.

OBJECTIVE: RNA sequencing (transcriptomics) is used to study biological pathways. However, the yield of data depends on comparing well-characterized cohorts. We compared tissue eosinophilia versus nasal polyp (NP) status as the metric to characterize transcriptomic mechanisms at play in eosinophilic and non-eosinophilic chronic rhinosinusitis (CRS) versus controls. METHODS: RNA sequencing was conducted on sinonasal tissue samples of CRS and controls. Analyses were conducted based on polyp status [with nasal polyps (CRSwNP) and without nasal polyps (CRSsNP)] as well as tissue eosinophil levels
