Postpartum Care for Patients With Diabetes: A Comprehensive and Whole-Life Approach.

Diabetes in pregnancy is associated with increased risk of cardiometabolic syndrome, mood disorders, intimate partner violence, and additional sequelae. These risks may persist beyond the pregnancy into the postpartum period. Comprehensive management and counseling require an understanding of the physiology of the puerperium, types of potential complications, and evidence behind management options. This review will discuss evidence-based immediate postpartum management, the impact of lactation on diabetes, and the long-term and transgenerational impact of diabetes in pregnancy.

A Systematic Review of the Cardiotoxic Effects of Targeted Therapies in Oncology.

Cancer therapy advancements have improved survival rates but also introduced significant cardiotoxic risks. Cardiotoxicity, a critical adverse effect of cancer treatments such as doxorubicin, trastuzumab, and radiotherapy, poses substantial challenges. This systematic review synthesizes findings from studies on cardiotoxicity induced by cancer therapies, focusing on detection and management. Key predictors of chemotherapy-induced myocardial toxicity (CIMT) include advanced age, hypertension, hyperlipidemia, diabetes, and elevated N-terminal pro-B-type natriuretic peptide levels. Musculoskeletal Issues in Children and Adolescents: Common Childhood Musculoskeletal Injuries.

Active children and adolescents have unique risk factors for musculoskeletal injuries compared with adults. Physes and developing bones are at higher risk of injury than tendons and ligaments. Children's bone remodeling is robust, allowing most clavicle fractures and torus fractures of the forearm to be managed conservatively. Radial head subluxation is managed with reduction. Apophyseal injuries are traction or overuse injuries that typically can be managed nonoperatively. Osteochondritis dissecans and other osteochondroses require frequent monitoring and occasionally surgical intervention.

Contemporary Role of Radiotherapy in the Management of Primary Penile Tumors and Metastatic Disease.

Squamous cell cancer of the penis is a radiocurable malignancy all too often managed solely by partial or total penectomy. Effective management of the primary tumor while preserving penile morphology and function is a priority. External radiotherapy and brachytherapy have a role to play in the definitive management of the primary tumor. Surgical nodal staging remains a cornerstone of management because it is the strongest predictor of survival, and inguinal status determines pelvic management. Postoperative radiotherapy of the regional nodes for high-risk pathology is indicated. Chemoradiother

[Lung Cancer with Pulmonary Embolism:
Evidence-based Progress in Anticoagulation Assessment and Management].

Lung cancer is the malignant tumour with the highest incidence and mortality rates globally. Patients with lung cancer face a high risk of developing venous thromboembolism (VTE), particularly pulmonary embolism (PE). This not only prolongs hospital stays and increases inpatient costs but also severely impacts patient survival and quality of life. Current risk assessment models for PE and bleeding risk in lung cancer patients remain limited in efficacy, posing challenges for clinicians in selecting anticoagulation or preventing bleeding. Additionally, anticoagulation management involves comple

Pediatric Central Diabetes Insipidus due to Arginine Vasopressin Deficiency in the Perianesthetic Period.

The purpose of this concise review is to provide a practical approach to inpatient management of pediatric patients with central diabetes insipidus due to arginine vasopressin deficiency (AVP-D), as well as provide recommendations specific to the perianesthetic period. AVP-D poses significant challenges in perianesthetic management. A multidisciplinary approach to the perianesthetic management of AVP-D can optimize patient outcomes and reduce the risk of complications. Implementing structured protocols that address fluid management, medication dosing, and postoperative care is essential for en

Body weight and the management of lymphoedema.

Overweight and obesity are known risk factors for the development of lymphoedema, and are also well-recognised factors complicating the management of lymphoedema. However, a degree of uncertainty remains regarding the way in which obesity impacts upon the condition, and there is little detail about the practical application of weight management strategies in the population with lymphoedema. This article provides the background for ongoing research into the association between lymphoedema management and obesity. It is hoped that this research will provide important insights into the long-term m

Nonsurgical treatment: swallowing rehabilitation.

This article provides an overview of nonsurgical and nonpharmacologic treatments for oropharyngeal dysphagia. The speech-language pathologist (SLP) is the primary member of the swallowing management team who will provide this type of dysphagia management. The primary focus of the SLP for dysphagia management is first to eliminate or reduce aspiration risk, as well as to improve or restore swallowing function. Ultimately, the management plan will depend on the physiologic underpinnings of the disorder and patient variables such as cognition, motivation, and ability to attend therapy sessions or

Revascularization Is Associated With a Reduced Stroke Risk in Patients With Sickle Cell-Associated Moyamoya Syndrome.

BACKGROUND: Moyamoya syndrome refers to a progressive stenosis of the internal carotid arteries and can be associated with sickle cell disease. These codiagnoses result in severe risk for stroke, even in patients on optimal medical management. Surgical revascularization has been shown to be safe in small case series. OBJECTIVE: To evaluate the efficacy of revascularization with direct comparison to a medically managed control group within a single institution. METHODS: A retrospective cohort study of medically managed vs surgically revascularized patients with moyamoya syndrome and sickle ce

Post-Transplant Management and Complications of Autoimmune Hepatitis, Primary Biliary Cholangitis, and Primary Sclerosing Cholangitis including Disease Recurrence.

Autoimmune liver diseases have unique post-transplant considerations. These recipients are at increased risk of rejection, and recurrent disease may also develop, which can progress to graft loss and increase mortality. Monitoring for and managing these complications is therefore important, though data on associated risk factors and immunosuppression strategies has in most cases been mixed. There are also other disease-specific complications that require management and may impact these decisions, including inflammatory bowel disease in PSC. Further work to better understand the optimal managem

[Physical therapy in vascular diseases].

Regular physical exercise reduces the risk of developing cardiovascular diseases. Along with cardiovascular risk factors management, physical exercise is considered as one of the main treatment components in people with cardiovascular diseases. The therapeutic role of physical exercise in patients with peripheral artery disease has been highlighted for decades. However, the role of physical exercise in the management of patients with venous, lymphatic, or rare vascular diseases remains poorly investigated. This article discuss the potential benefits of physical exercise in the therapeutic care

Sunitinib treatment of metastatic renal cell carcinoma in von Hippel-Lindau disease.

Von Hippel-Lindau disease (VHL) is a major heritable renal cell cancer (RCC) syndrome. 24-45% of VHL patients have a cumulative risk to develop RCC. The management of VHL has changed significantly as clinicians have learned how to best balance the risk of cancer dissemination while minimizing renal morbidity. Despite the improving management of this disease, there are no evidence-based guidelines of the treatment of localized and advanced RCC in VHL. Here, we describe a 39-year-old patient with VHL disease who developed metastatic RCC and received sunitinib with complete remission.

Ruptured penetrating ulcer of the ascending aorta with pulmonary artery compression.

Pulmonary artery involvement has been reported in various degrees of complicated dissection of the ascending aorta. The prognosis remains poor without high-risk surgical intervention, but conservative management can be considered in high-risk cases. We report a case of nonoperative management of an octogenarian who presented with a contained rupture of his proximal ascending aorta, likely from a penetrating atherosclerotic ulcer. It was complicated by extrinsic compression of the pulmonary trunk and transient pulmonary hypertension without features of acute right heart failure. He remained ali

Oncological emergencies: tumor lysis syndrome.

Tumor lysis syndrome (TLS) occurs as a result of massive lysis of malignant cells and release of intracellular contents into the systemic circulation. It can lead to hyperuricaemia, hyperkalaemia, hyperphosphataemia and hypocalcaemia. TLS is most commonly present after initiation of anticancer therapy but it can also develop spontaneously (STLS--spontaneous tumor lysis syndrome). In the article, pathophysiology, classification, risk factors and recommendations of management in TLS, with a special focus on solid tumors, are discussed. The keys to the identification of high-risk patients, preven
