Surgical versus conservative treatment of acute rockwood type Ⅲ-Ⅴ acromioclavicular joint dislocation: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Acromioclavicular joint (ACJ) injuries, accounting for approximately 12 % of all shoulder ligament injuries, are common. Conservative treatment is recommended for Rockwood type Ⅰ and Ⅱ dislocations, while the optimal treatment for type Ⅲ remains debated. For Rockwood type Ⅳ, Ⅴ, and VI dislocations, most scholars advocate surgical treatment. However, other studies have found no significant difference in clinical outcomes between conservative and surgical treatments for Rockwood type Ⅴ dislocations. METHODS: This systematic review and meta-analysis, conducted following PRISMA guidel

Biomechanical Analysis of Anteroinferior Bankart Repair Anchor Types.

BACKGROUND: All-suture anchors and knotless anchors are increasingly used in the repair of anteroinferior labral tears in patients with shoulder instability. Optimal repair constructs may limit recurrent instability. PURPOSE: To perform a quantitative biomechanical comparison of 3 labral fixation devices for soft tissue Bankart lesions: knotless soft-body tensionable anchor (SB knotless), knotted soft-body anchor (SB knotted), and knotless hard-body PEEK (polyether ether ketone) interference anchor (HB knotless). STUDY DESIGN: Controlled laboratory study. METHODS: A total of 21 glenoid spec

Use of Synthetic Osteochondral Implants to Treat Bilateral Shoulder Osteochondritis Dissecans in a Dog.

CASE DESCRIPTION: An 8-month-old American Bulldog was presented for assessment of bilateral thoracic limb lameness. Computed tomographic imaging revealed large, deep osteochondritis dissecans lesions in both humeral heads. TREATMENT AND OUTCOME: The osteochondritis dissecans lesions were debrided and the exposed subchondral defects were prepared to receive synthetic grafts. Circular implants consisting of a surface layer of polycarbonate urethane and a deep layer of lattice-type titanium were implanted into the osteochondral defects to reconstruct the articular surface topography. Follow-up c

Valgus Osteotomy of the Proximal Humerus to Treat Humerus Varus in Children.

PURPOSE: Humerus varus is a rare disease in children that can cause limited active abduction and forward flexion of the shoulder as well as upper limb length discrepancies. Valgus osteotomy of the proximal humerus is an effective method to treat this disease, although internal fixation with a plate and screws is rarely reported. The purpose of this article is to investigate the clinical outcome and radiographic changes of valgus osteotomy of the proximal humerus through internal fixation with a plate and screws. METHODS: From April 2005 to August 2011, our group treated 5 patients including 1

Histopathologic evaluation of passive stabilizers in shoulder instability.

BACKGROUND: The macroscopic pathomorphology in recurrent shoulder instability has been described. However, less is known regarding the histopathologic details of the affected structures. This study evaluates different histopathologic stages of shoulder instability by assessing biopsy specimens of static stabilizers for possible correlations with clinical parameters. Our hypothesis was that clinical parameters of shoulder instability correlate with histopathologic findings. MATERIALS AND METHODS: Passive shoulder stabilizers (labrum, anterior bundle of the inferior glenohumeral ligament) were

Total reverse shoulder arthroplasty: European lessons and future trends.

In the late 1980s, Grammont introduced a new reverse total shoulder arthroplasty (TSA), with a hemisphere directly attached to the glenoid surface and with medial positioning of the center of rotation to overcome former shortcomings. Over the past few years, results from several mid- and long-term clinical studies of this new TSA have demonstrated that unexpectedly good functional outcomes and pain relief (Constant-Score, 60-69) could be achieved, even in patients with progressive superior migration, joint destruction, and rotator cuff deficiency. In all these studies, however, limited range o

The effect of scapula taping on electromyographic activity and musical performance in professional violinists.

Taping the scapula has been suggested as a method of improving both scapula position and muscular efficiency of the shoulder girdle. These factors have been linked to neck and arm problems in violinists. The purpose of this study was to evaluate the effects of taping the scapulae of violinists into a position that prevented excessive elevation and protraction whilst playing. Eight professional violinists played three different musical excerpts with and without scapula taping applied in random order. Electromyographic activity was recorded from the upper trapezii, the scapula retractors and the

Granulomatous osteonecrosis in Crohn's disease.

A 25-year-old white woman was diagnosed with Crohn's disease involving the small and large intestines. She had a complex clinical course that required treatment with multiple pharmacological agents, including intravenous, oral and rectal corticosteroids. She also received parenteral nutrition with lipid emulsions. Finally, repeated intestinal resections and drainage of perianal abscesses were required. Her disease was complicated by gallstones, urolithiasis and hip pain. After osteonecrosis was diagnosed, joint replacements were performed. Review of the pathological sections from the resected

Anterior glenohumeral stabilization factors: progressive effects in a biomechanical model.

The aim of this study was to evaluate the anterior stabilizing factors of the glenohumeral joint over a range of translations. The stabilizers examined included the capsular ligaments, the coracohumeral ligament, the rotator cuff muscles, and the long head of the biceps. Simulated muscle forces were applied to eight shoulder specimens to produce 90 degrees of total elevation of the arm in the scapular plane. Stability, defined as the force required to reach a specified subluxation, then was evaluated under varying configurations of capsule cuts, humeral rotation, and muscular loads. The overal

Arthroscopic versus open reconstruction of the shoulder in patients with isolated Bankart lesions.

We compared open and arthroscopic stabilizations of true Bankart lesions in patients with traumatic, unidirectional anterior glenohumeral dislocations. The 27 patients were men (age range, 18 to 56 years) who were involved in recreational sports. One group (15 patients) had elected an arthroscopic Bankart repair; the other group (12 patients) had chosen open stabilization with a standard deltopectoral approach. Patients were followed up 17 to 42 months after surgery by examination, radiographs, and interviews. In the open repair group, 1 of the 12 patients experienced a subluxation in the foll

The role of the long head of the biceps brachii in superior stability of the glenohumeral joint.

We studied seven patients who had isolated loss of the proximal attachment of the tendon of the long head of the biceps brachii, documented operatively or with magnetic resonance imaging, in order to identify and measure superior translation of the humeral head on the glenoid. Four true anteroposterior radiographs were made of both shoulders, before and after the operation, with 0, 45, 90, and 120 degrees of humeral abduction in the scapular plane. Four patients were managed with arthroscopic acromioplasty with an open biceps tenodesis; one, with open biceps tenodesis alone; and one, with debr

Reliability of radiographic assessment of acromial morphology.

The most widely used radiographic classification system for acromial morphology identifies three distinct acromial shapes: type I (flat), type II (curved), and type III (hooked). The purpose of this study was to measure the interobserver and intraobserver reliability of determinations of acromial morphology as defined by this system. Between 1990 and 1992, one hundred twenty-six supraspinatus outlet radiographs were obtained from 126 patients by technicians from Triangle Orthopaedic Associates in Durham, N.C. Six fellowship-trained shoulder surgeons independently reviewed each radiograph and c

Surgical versus non-surgical treatment for acute anterior shoulder dislocation.

BACKGROUND: Acute anterior shoulder dislocation is the commonest type of shoulder dislocation. Subsequently, the shoulder is less stable and more susceptible to re-dislocation, especially in active young adults. OBJECTIVES: We aimed to compare surgical versus non-surgical treatment for acute anterior dislocation of the shoulder. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group specialised register (August 2003), the Cochrane Central Register of Controlled Trials (The Cochrane Library Issue 3, 2003), MEDLINE (1966 to September week 3 2003), EMBASE (1988 to 2003 week 39

Effect of concentric exercise-induced fatigue on proprioception, motor control and performance of the upper limb in handball players.

BACKGROUND: The last phases of a competitive game are when shoulder injuries most commonly happen, and fatigue is thought to be a major contributing factor, perhaps because of reduced proprioception and motor control. The purpose of this study was to investigate the effect of concentric fatigue on proprioception, motor control, and performance of the upper limb in handball players. METHODS: Forty-six right-handed handball players (all males, age 26.1 ± 5.54 years) were included in this test-retest laboratory experiment. Proprioception was assessed using joint reposition sense (JRS), threshold

Acromioclavicular third degree dislocation: surgical treatment in acute cases.

BACKGROUND: The management of acute Rockwood type III acromioclavicular joint (ACJ) dislocation remains controversial, and the debate about whether patients should be conservatively or surgically treated continues. This study aims to compare conservative and surgical treatment of acute type III ACJ injuries in active sport participants (<35 years of age) by analysing clinical and radiological results after a minimum of 24 months follow-up. METHODS: The records of 72 patients with acute type III ACJ dislocations who were treated from January 2006 to December 2011 were retrospectively evaluated
