Neurocognitive function in obstructive sleep apnoea: a meta-review.

Adult obstructive sleep apnoea (OSA) is associated with cognitive dysfunction. While many review articles have attempted to summarize the evidence for this association, it remains difficult to determine which domains of cognition are affected by OSA. This is because of marked differences in the nature of these reviews (e.g. many are unsystematic) and the many different tasks and domains assessed. This paper addresses this issue by comparing the results of only systematic reviews or meta-analyses assessing the effects of OSA on cognition, the relationship between OSA severity and cognition, and…

Mandibular advancement appliances for treating sleep apnoea/hypopnoea syndrome.

STUDY DESIGN: Randomised controlled crossover trial. INTERVENTION: Sixteen participants (13 men and three women) with mild to moderate OSAHS, diagnosed using polysomnography (PSG), were recruited from a sleep clinic. To be included, participants had to have 20 or more teeth (sufficient to provide retention for oral appliances), have had no previous OSAHS treatment, been unable to tolerate nasal continuous positive airway pressure and be free from caries, periodontal disease, temporomandibular joint (TMJ) pain or movement limitations. One-night mandibular advancement titration was carried out …

Obstructive Sleep Apnoea in Pregnancy - More Questions than Answers.

The role of obstructive sleep apnoea (OSA) in pregnancy is not well studied, but an increasing body of literature appears to indicate that there may be adverse maternal and foetal health effects of the disease. OSA is associated with a twofold risk of pre-eclampsia. The small size of the existing investigations still leave unanswered questions about the consequences of OSA as it relates to some other clinically relevant outcomes such as eclampsia, stillbirth and maternal mortality. A consistent body of literature has emerged demonstrating an increased risk of insulin resistance and diabetes as…

Craniofacial morphology of Croatian patients with obstructive sleep apnea.

Currently, there is no information available regarding craniofacial morphology of Croatian patients with obstructive sleep apnea (OSA). The aim of the study was to determine the craniofacial characteristics of patients with OSA and to assess the association of cephalometric and anthropometric variables related to craniofacial morphology with the apnea hypopnea index (AHI). Anthropometric measurements and upright lateral cephalometric radiographs were obtained from 20 male patients with OSA and 20 male controls. The 20 OSA patients were classified into two groups on the basis of body mass index…

Measuring individual vulnerability to sleep loss--the CHICa scale.

The aim of this project was to construct a psychometrically satisfying scale to describe subjective reactions to sleep deprivation. First, on the basis of a literature review, a list of items was generated which reflected the negatively affected mood and reduced wellbeing associated with sleep loss. Additionally, psychology students were asked to describe their cognitive and emotional symptoms following a night with curtailed sleep. As a result, 69 items were included in the experimental set. University students (n = 102, females, mean age 22.5 ± 1.9 years) completed the form several times dur…

Eyelid laxity, obesity, and obstructive sleep apnea in keratoconus.

PURPOSE: The association between floppy eyelid syndrome and keratoconus (KCN) has been well established, but the converse relationship has not yet been examined and is the objective of this study. The study also investigates the prevalence of obesity and obstructive sleep apnea (OSA) in KCN patients. METHODS: A prospective case-control study of KCN patients with age-, sex-, race-, and body mass index-matched controls was conducted at the Ross Eye Institute, Buffalo, NY. Fifteen patients were enrolled in each group. Extensive eyelid laxity measurements were performed on both groups. Complete m…

Oropharyngeal Dysphagia in patients with obstructive sleep apnea syndrome.

Although previous studies demonstrated that patients with obstructive sleep apnea syndrome (OSAS) may present subclinical manifestations of dysphagia, in not one were different textures and volumes systematically studied. The aim of this study was to analyze the signs and symptoms of oropharyngeal dysphagia using fiberoptic endoscopic evaluation of swallowing (FEES) with boluses of different textures and volumes in a large cohort of patients with OSAS. A total of 72 OSAS patients without symptoms of dysphagia were enrolled. The cohort was divided in two groups: 30 patients with moderate OSAS a…

Invited commentary: sleep disturbances--another threat to male fecundity?

In a cross-sectional study among 953 young Danish men (2008-2011), Jensen et al. reported that sleep disturbances showed inverse U-shaped associations with semen parameters and testis size (Am J Epidemiol. 2013;177(10):1027-1037). Sleep disturbances were associated with several factors likely to affect semen parameters (such as history of sexually transmitted infections) that cannot all be efficiently controlled for, leaving room for residual confounding. Future studies could adopt a longitudinal design and rely on objective personal measures of sleep quality and duration using accelerometers.…

Restless legs syndrome: impact on sleep-related breathing disorders.

Restless legs syndrome (RLS) is a common chronic sensory-motor neurological disorder that remains a clinical diagnosis. Most RLS patients present with sleep complaints in the form of initiation and/or maintenance insomnia as RLS has a circadian rhythmicity. An increased number of periodic leg movements during sleep (PLMS) is a supportive criterion in the diagnosis of RLS. Abnormalities in the central dopaminergic and iron systems are involved in the physiopathology of RLS. There is a higher prevalence of RLS and PLMS in sleep-disordered breathing patients, particularly those with obstructive s…

Clinical heterogeneity of patients with complex sleep apnea syndrome.

BACKGROUND: The definition of complex sleep apnea (CompSAS) encompasses patients with obstructive sleep apnea (OSA) who develop central apnea activity upon restitution of airway patency. Presence of arterial hypertension (HTN), coronary artery disease (CAD) and heart failure (HF) have been proposed as risk factors for CompSAS among OSA patients. Using our database of patients with CompSAS, we examined the prevalence of these risk factors and defined other clinical characteristics of patients with CompSAS. METHODS: Through retrospective search of the database, we examined the medical and clini…

Detection of sleep-disordered breating with Pressure Bed Sensor.

A Pressure Bed Sensor (PBS) can offer an unobtrusive method for sleep monitoring. This study focuses on the detection of the sleep related breathing disorders using a PBS in comparison to the methods used in a sleep laboratory. A newly developed PCA modeling approach for the eight sensor signals of the PBS is evaluated using the Reduced Respiratory Amplitude Index (RRAI) as a central measure. The method computes the respiration amplitude with the Hilbert transform, and then detects the events based on a 20% amplitude reduction from the baseline signal. A similar calculation was used for the sl…

Breastfeeding and infant sleep patterns: an Australian population study.

OBJECTIVE: Our purpose was to determine if babies breastfed at 6 months of age were more likely to wake at night and less likely to sleep alone than formula-fed babies. PATIENTS AND METHODS: Data were drawn from the first wave of The Longitudinal Study of Australian Children, an ongoing, nationally representative study of the growth and development of Australia's children. The 4507 participants met the criteria for this study. The measures examined infant sleep problems as the outcome and breastfeeding at 6 months of age as the exposure in addition to the demographic data, maternal mental hea…

Relationship between obstructive sleep apnea syndrome and thyroid diseases.

Obstructive sleep apnea syndrome (OSAS) is a widespread disorder characterized by recurrent, partial, or complete episodes of apnea due to upper airway tract obstruction during sleep. OSAS frequency is likely to increase in hypothyroidism because of obesity, macroglossia, dysfunctional upper respiratory tractus (URT) musculature, deposition of mucopolysaccharides in URT tissues, and decreased ventilatory control. This study examines the relationship between OSAS and thyroid disease in OSAS subjects. This study includes 150 polysomnographically diagnosed OSAS patients (50 mild, 50 moderate, 50 …

The link between vitamin D metabolism and sleep medicine.

Vitamin D is a hormone that interacts with intranuclear receptors to effect transcriptional changes in many cell types including those in gut, bone, breast, prostate, brain, skeletal muscle, and the immune system. Inadequacy of vitamin D is widely prevalent, and leads to the classic diseases of bone demineralization as well as to more recently recognized problems such as nonspecific pain and noninflammatory skeletal myopathy, which may disrupt sleep and directly cause daytime impairment. Emerging lines of evidence suggest that low vitamin D levels increase the risk for autoimmune disease, chro…

A solitary tonsil can cause severe obstructive sleep apnea.

Hypertrophy of the tonsils and adenoids is the most common cause of pediatric obstructive sleep apnea. Bilateral tonsillectomy, most commonly performed with adenoidectomy, is the accepted treatment for obstructive sleep apnea. We report the unusual case of a child who underwent unilateral tonsillectomy and adenoidectomy at another institution and subsequently presented to us with persistent severe obstructive sleep apnea and a diagnosis of attention deficit hyperactivity disorder. The adenoids were not obstructing the choanae. The remaining tonsil was removed and the patient's sleep apnea reso…
