Degenerative Zygapophyseal Joint Subluxation of the Lumbar Spine: A Modified Classification and Clinical Considerations.

Degenerative lumbar zygapophyseal (facet) joint subluxation without prior trauma, fracture, spondylolysis, or spondylolisthesis is extremely uncommon and represents a distinct clinical entity. This study aims to present rare cases of advanced degenerative facet joint changes combined with unilateral or bilateral facet joint subluxation in the absence of trauma. All patients experienced chronic symptoms unresponsive to conservative treatment. MRI findings demonstrated Grade 4 degeneration per Grogan's classification, along with distinct subluxation patterns not captured by existing grading syst

Surgical Management of Facet Fracture Dislocations of the Subaxial Spine: A Retrospective Cohort Study.

INTRODUCTION: Facet fracture dislocations of the subaxial spine pose notable risks of neurologic injury and spinal instability. The optimal surgical approach-whether anterior-alone, posterior-alone, or combined anterior-posterior-remains debated. The aim of this study was to evaluate the effectiveness, safety, and long-term outcomes of these surgical approaches. METHODS: A retrospective analysis of patients presenting with cervical facet fractures at a single level I trauma center was conducted. They were divided into anterior-alone, posterior-alone, and combined AP surgical groups. Primary o…

The Impact of Atlantoaxial Intra-Articular Fusion on Cervical Spine Curvature and Sagittal Balance.

AIM: To investigate whether atlantoaxial intra-articular fusion (AIF) can maintain sagittal balance stability in the cervical spine during follow-up. MATERIAL AND METHODS: The data of 39 patients with anterior atlantoaxial dislocation who underwent AIF and 21 patients who underwent structural bone grafting (SBG) fusion were retrospectively reviewed. Radiographic variables, including T1 slope (T1S), C1?C2 angle, C2?C7 angle, C2?C7 sagittal vertical axis (SVA), and lateral atlantoaxial joint space height (LAAJSH), were measured preoperatively, postoperatively, and at the last follow-up. Analyzi…

Computed Tomographic Evaluation of the Morphometry of the Intervertebral Foramina and Intervertebral Discs of the Cervical Spine.

BACKGROUND: The cervical spine comprises vertebrae and intervertebral discs that facilitate neck mobility and protect neurovascular structures. Intervertebral foramina serve as conduits for spinal nerves and vessels and may undergo morphological changes with aging and degeneration, potentially leading to neurological symptoms. The objectives of this study included performing a CT analysis of the morphometry of intervertebral foramina and the height of the intervertebral disc across different levels of the cervical spine, with comparisons based on age, vertebral level, gender, and laterality. …

Abscesses of the spine and iliac fossa caused by infection with the opportunistic bacterium Fusobacterium nucleatum.

We present a case of spinal and iliac fossa infection caused by Fusobacterium nucleatum in a 52-year-old male patient with unexplained back pain. Notably, the patient had no history of periodontal infections.Magnetic resonance imaging (MRI) revealed abscesses at the L3-4 vertebrae and hip, with dura mater and cauda equina compression.Next-generation sequencing (NGS) of the aspirate confirmed the presence of Fusobacterium nucleatum. The patient was treated with antibiotics (carbapenems, and metronidazole) and underwent surgical procedures for abscess drainage and internal fixation, resulting in

Effects of cholestasis and hyperammonemia on dendritic spine density and turnover in rat hippocampal neurons.

Adults and children with cholestatic liver disease are at risk for type C hepatic encephalopathy (HE) and may present lifelong neurocognitive impairment. While the underlying cellular and molecular mechanisms are still incompletely understood, ammonium and bile acids (BAs) seem to play a key role in this pathology, by crossing the blood-brain-barrier and modifying neuronal homeostasis and synaptic plasticity. This experimental study aimed to investigate the effects of ammonium and BAs on dendritic spines of rat hippocampal CA1 neurons. Taking advantage of the bile duct ligated (BDL) in vivo ra…

Assessment of minimum target dose as a predictor of local failure after spine SBRT.

OBJECTIVES: Metastasis-directed stereotactic body radiation therapy (SBRT) has demonstrated robust clinical benefits in carefully selected patients, improving local control and even overall survival (OS). We assess a large database to determine clinical and dosimetric predictors of local failure after spine SBRT. METHODS: Spine SBRT treatments with imaging follow-up were identified. Patients were treated with a simultaneous integrated boost technique using 1 or 3 fractions, delivering 20-24 Gy in 1 fraction to the gross tumor volume (GTV) and 16 Gy to the low dose volume (or 27-36 Gy and 21-2…

Simulative investigation of the required level of geometrical individualization of the lumbar spines to predict fractures.

Injury mechanisms of the lumbar spine under dynamic loading are dependent on spine curvature and anatomical variation. Impact simulation with finite element (FE) models can assist the reconstruction and prediction of injuries. The objective of this study was to determine which level of individualization of a baseline FE lumbar spine model is necessary to replicate experimental responses and fracture locations in a dynamic experiment.Experimental X-rays from 26 dynamic drop tower tests were used to create three configurations of a lumbar spine model (T12 to L5): baseline, with aligned vertebrae…

Complementary effects of postoperative delirium and frailty on 30-day outcomes in spine surgery.

BACKGROUND CONTEXT: With an increasingly older population, the number of frail patients requiring surgical management for degenerative spine diseases is rapidly increasing. Older patients are at increased risk of developing postoperative delirium (POD), which increases the odds of postoperative morbidity and mortality in spine surgery patients. Therefore, frail spine surgery patients may be at greater risk of developing POD and subsequent adverse outcomes. PURPOSE: To understand the relationship between frailty and POD in spine surgery patients, and the effect of POD on nonfatal and fatal adv…

Injury of the Hyoid Bone as a Rare Complication in Cervical Spine Rehabilitation: A Case Report.

Neck pain is a common reason for primary care visits, and its differential diagnosis should consider various conditions. The reported incidence of hyoid bone fractures is extremely low, accounting for only 0.002% of all fractures. The most common causes of hyoid bone fractures include strangulation attempts and motor vehicle accidents. We report a case of an uncommon complication of manual therapy of the cervical spine. A 76-year-old woman complained of neck pain that worsened during speaking and swallowing, originating from a neck physiotherapy session. The otolaryngological examination revea…

Evaluation and Management of Thoracolumbar Spine Trauma in Pediatric Patients: A Critical Analysis Review.

» Pediatric thoracolumbar trauma, though rare, is an important cause of morbidity and mortality and necessitates early, accurate diagnosis and management.» Obtaining a detailed history and physical examination in the pediatric population can be difficult. Therefore, the threshold for advanced imaging, such as magnetic resonance imaging, is low and should be performed in patients with head injuries, altered mental status, inability to cooperate with examination, and fractures involving more than 1 column of the spine.» The classification of pediatric thoracolumbar trauma is based primarily on a…

Clinical recognition of the role of the cervical spine in signs and symptoms of altered sensorimotor control.

Together with visual and vestibular input the cervical spine is vital for sensorimotor control of head and eye movement control, general body postural stability adjustments and co-ordination. Altered cervical input in persons with neck disorders can lead to signs and symptoms of impaired sensorimotor control across and within several domains. Clinical assessment for differential diagnosis and to direct management of cervical related altered sensorimotor control is clearly required in many patients. This applies not only to patients with neck disorders but also in patients with, for example ves…

Melorheostosis of the cervical and cervicothoracic spine: review of the literature and presentation of 3 new cases.

Melorheostosis is a rare dysplastic bone disease that usually affects long bones at the upper or lower limbs. It is rarely seen at the spine and very rarely located at the cervical spine. To the best of our knowledge, there are currently 6 cases of melorheostosis described at the cervical spine. Here, we give an overview of the current literature of spinal melorheostosis with respect to number of cases and skeletal distribution, aetiology, radiographic appearance, symptoms and treatment. Finally, we describe 3 new clinical cases with manifestation of melorheostosis at the cervical spine and th…

Catfish Puncture Wound and Retained Spine Management in the ED Setting: A Case Report.

Fishing is a common recreational activity in the United States, with over 29 million registered fishers. Although not inherently dangerous, commonly seen injuries from fishing include embedded fishhooks and injury from flora and fauna. Emergency department (ED) physicians need a basic understanding of how to treat these less-than-frequent injuries. We present a case report of a patient who presented with a catfish spine lodged in her leg. These spines not only cause puncture wounds but can result in lacerations and venom release as well. Our patient presented 6 hours after the initial injury f…
