Bilateral, simultaneous pneumococcal septic arthritis of the knees: a normal immune system, an unknown source.

Pneumococcal septic arthritis is a rare clinical entity and is often associated with a systemic bacteraemia. A 60-year-old man was admitted with bilateral swollen, painful knees. He was feeling feverish with raised inflammatory markers. Joint aspiration yielded purulent fluid, which, when cultured, grew Streptococcus Pneumoniae bilaterally. The patient underwent repeated arthroscopic knee washouts and was treated with intravenous (IV) antibiotics. During his admission, various investigations and scans were undertaken to find an infective focus or signs of immunodeficiency; none were found. Aft…

Activity of ceftaroline and comparator agents tested against contemporary Gram-positive and -negative (2011) isolates collected in Europe, Turkey, and Israel.

The activity of ceftaroline was tested against 8233 isolates mainly collected from bloodstream, urinary, respiratory and skin and soft tissue specimens in European medical centers during 2011. This cephalosporin displayed activity against Staphylococcus aureus (MIC(50), 0·25 mg/l), with greater activity against MRSA (MIC(50), 1 mg/l) than other β-lactams tested. Against Streptococcus pneumoniae, including penicillin-resistant strains, other streptococcal groups and Haemophilus spp., ceftaroline was highly active and MIC90 values ranged from ≤0·015 to 0·12 mg/l. Ceftaroline, like other cephalos…

[Microflora of palatine tonsils in chronic tonsillitis].

The objective of the present work was to develop the standardized approaches to pharmacotherapy of chronic tonsillitis based on the data of regional specificity of the tonsillar microflora. The methods of investigations included the study of throat swabs obtained from 668 patients presenting with various forms of chronic tonsillitis (CT). Half of these patients were shown to be infected with Streptococcus pyogenes and 24.5% with Mycoplasma pneumoniae. The presence of concomitant pathology of the respiratory and digestive tracts in the patients with CT increases the probability of identificatio

Pneumococcal microbial surface components recognizing adhesive matrix molecules targeting of the extracellular matrix.

The attachment of bacteria to host cells and tissues, and their subsequent invasion and dissemination are key processes during pathogenesis. In this issue of Molecular Microbiology, Jensch and co-workers provide further molecular insight into these events during infection with the Gram positive bacterium Streptococcus pneumoniae. Their characterization of pneumococcal adherence and virulence factor B (PavB), a bacterial surface protein with orthologues in other streptococci, show that it binds to the extracellar matrix components fibronection and plasminogen by virtue of repetitive sequences-d…

Surveillance of antimicrobial resistance of invasive pathogens: the Estonian experience.

The aim of the present study was to evaluate the needs for surveillance of invasive Gram-negative pathogens in Estonia. The antimicrobial susceptibility data of invasive isolates of Acinetobacter baumannii, Pseudomonas aeruginosa, Klebsiella spp, Escherichia coli, Staphylococcus aureus, Streptococcus pneumoniae and enterococci were collected in accordance with EARSS (European Antimicrobial Resistance Surveillance System) protocols. Despite the higher rate of Gram positive pathogens, their resistance to antimicrobials was low in contrast to the elevated resistance established for Gram negative …

Disease transmission in horses.

Bacterial, viral and protozoal infections may cause severe reproductive losses. The present paper reviews the risk factors, clinical signs and preventive measures for the most important venereal or potential sexually transmitted diseases in horses. The stallion and use of semen for artificial insemination represent major risk factors for the transmission of bacterial contaminants of the penis, including Streptococcus equi subspecies zooepidemicus, Pseudomonas aeruginosa and Klebsiella pneumoniae, known to cause endometritis and infertility in the mare. The role of the stallion in disease trans

Faropenem medoxomil: a treatment option in acute bacterial rhinosinusitis.

Faropenem medoxomil is the first oral penem in a new class of beta-lactam antibiotics. Faropenem medoxomil has excellent in vitro activity against Streptococcus pneumoniae, Haemophilus influenzae and other key pathogens implicated in acute bacterial rhinosinusitis. Clinical studies have demonstrated that, in the treatment of acute bacterial rhinosinusitis in adults, 7 days of treatment with faropenem medoxomil is as clinically and bacteriologically effective as 10 days of treatment with cefuroxime axetil. One study showed faropenem medoxomil to be superior to cefuroxime axetil. Overall, the sa…

Perimenopausal pneumococcal tubo-ovarian abscess--a case report and review.

BACKGROUND: Genital tract infections in females secondary to Streptococcus pneumoniae (pneumococcus) are unusual. Tubo-ovarian abscess resulting from such an infection is a rare occurrence and diagnosis is not always easy. This report demonstrates the problems of recognizing this condition and summarizes the pathomechanism, investigations leading to a diagnosis and the subsequent management. CASE: A rare case of a tubo-ovarian abscess caused by pneumococcus, occurring in a previously healthy 48-year-old woman, is presented. The tubo-ovarian abscess may have developed insidiously and probably …

Pneumococcal conjugate vaccines: proceedings from an interactive symposium at the 41st Interscience Conference on Antimicrobial Agents and Chemotherapy.

Globally, Streptococcus pneumoniae is a leading cause of invasive and noninvasive disease in infants and young children. The emergence of antibiotic-resistant strains has increased interest in prevention through immunization. Currently, the only available conjugate pneumococcal vaccine is a seven-valent formulation, PNCRM7. This paper presents excerpts from a symposium that provided an update of ongoing surveillance data and clinical trials evaluating pneumococcal conjugate vaccines. The topics addressed included: (1) PNCRM7 postmarketing safety data; (2) the impact of PNCRM7 in premature infa…

[Antibiotic resistance: infections of the upper respiratory tract and bronchi. When are antibiotics necessary?].

Antimicrobial resistance among respiratory tract pathogens has become an increasing problem worldwide during the last 10-20 years. The wide use of antimicrobial agents in ambulatory practice has contributed to the emergence and spread of antibiotic-resistant bacteria in the community, namely Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis. The pneumococcus has developed resistance to most antibiotics used for its treatment. Classes with important resistance problems include the beta-lactams, the macrolides, the lincosamides, trimethoprim-sulfamethoxazole, and the tet…

Peripheral gangrene complicating idiopathic and recessive hemolytic uremic syndromes.

Three patients with hemolytic uremic syndrome (HUS) developed peripheral gangrene. Bilateral carotid artery thromboses occurred in one of these patients after recovery from HUS. One patient had a long history of juvenile rheumatoid arthritis. In the second patient, a flu-like illness preceded the onset of HUS. The third was one of two sisters, with the HUS appearing more than 1 year apart. None had evidence of disseminated intravascular coagulation or infection with Streptococcus pneumoniae. The patient with rheumatoid arthritis had renal cortical necrosis but recovered moderate renal function…

Management of acute exacerbation of chronic bronchitis.

In chronic bronchitis, a common respiratory illness marked by chronic productive cough and caused largely by cigarette smoking, bacterial exacerbations are thought to be a common cause of progressive airway damage. Common bacterial pathogens, found in 50% to 60% of episodes, include Haemophilus influenzae (the most common), as well as Haemophilus parainfluenzae, Streptococcus pneumoniae and Moraxella catarrhalis. Bacterial resistance to antibiotics, especially beta-lactam drugs such as amoxicillin, is increasingly common in these pathogens. In this illness, clinical risk stratification based o…
